Maintenance disclosures
DURING CRUISE AT FL410, THE FLIGHT CREW NOTED THAT THE COCKPIT PAC HAD BECOME UNCONTROLLABLE IN BOTH AUTO AND MANUAL MODES. THE CREW RAN THE APPROPRIATE CHECK LIST AND WITHIN 5 MINUTES, A CABIN PAC O'TEMP MESSAGE WAS RECEIVED. THE FIGHT CREW DONNED THEIR OXYGEN MASKS AND STARTED AN EMERGENCY DESCENT AFTER OBSERVING UNCONTROLLABLE CABIN RATE PRESSURE INCREASE. ATC WAS NOTIFIED OF THE EMERGENCY. DURING THE DESCENT, A RED CABIN ALTITUDE MESSAGE POSTED ALONG WITH THE AMBER AVN HOT BAGGAGE MESSAGE. DURING THIS TIME, THE PASSENGER OXYGEN MASKS HAS ALREADY DEPLOYED. THE CREW DIVERTED TO CLE AND LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE WAS SECURED TO INSPECT AND REPAIR THE AIRCRAFT. EXTENSIVE TROUBLESHOOTING WAS PERFORMED. THE LEFT HIGH/LOW PRESSURE VALVE WAS REPLACED (P/N 9914536-5), THE RIGHT HIGH/LOW PRESSURE VALVE WAS REPLACED (P/N 9914536-5), THE TT011 CABIN PAC DUCT TEMP SENSOR (ENVIRONMENTAL CONTROL SENSOR) WAS REPLACED AND THE LEFT AND RIGHT INTERGRADED AVIONICS COMPUTERS WERE SWAPPED FOR TROUBLESHOOTING. THE PASSENGER OXYGEN MASKS WERE RE-PACKED AND THE AIRCRAFT READIED FOR A MAINTENANCE FLIGHT TO TAKE PLACE AFTER FILING OF THIS REPORT. A SUPPLEMENTAL SDR TO BE FILED IF ADDITIONAL MAINTENANCE ACTION IS REQUIRED.
DURING CRUISE AT FL450, FLIGHT CREW RECEIVED A CABIN PAC OâTEMPâ CAS MESSAGE, AND NOTED THE CABIN SMELLED OF BURNING RUBBER. PER THE CHECKLIST, CREW SELECTED THE PAC OFF AND DESCENDED TO FL410, OPERATED THE PAC AGAIN, WHICH IT OPERATED NORMALLY FOR A FEW MINUTES THEN THE CABIN PAC OâTEMP MESSAGE RETURNED. WHILE COMMUNICATING WITH MX CONTROL, THE REMAINING COCKPIT PAC FAILED. AT THIS POINT THE CABIN ALTITUDE BEGAN TO CLIMB RAPIDLY AND UNCONTROLLABLY. FLIGHT CREW DONNED THEIR O2 MASKS AND BEGAN AN EMERGENCY DESCENT TO FL100. NOT KNOWING HOW LONG THE CABIN PRESSURE WOULD HOLD, CREW ATTEMPTED TO DEPLOY THE PASSENGER/CABIN O2 MASKS, BUT THE SYSTEM FAILED TO DEPLOY. CREW ISSUED A MAYDAY CALL AND DIVERTED TO KDSM, WHERE THEY LANDED WITHOUT FURTHER INCIDENT. ONCE ON THE GROUND, BOTH THE CABIN PRESSURIZATION SYSTEM AND PASSENGER O2 MASKS WERE DEFERRED (PLACED ON MEL). THE AIRCRAFT WAS THEN FLOWN TO MX FOR TROUBLESHOOTING THE ISSUES. MX FOUND A FAULTY OXYGEN CONTROL VALVE (P/N: 9914067-19 S/N: 2916). A NEW VALVE WAS INSTALLED AND FUNCTIONAL TEST WAS NORMAL OF THE CABIN O2 MASKS. WHILE TROUBLESHOOTING THE PRESSURIZATION ISSUE, MX FOUND THE CABIN PAC ASPIRATOR HOSE CLOGGED AND THE COCKPIT ASPIRATOR HOSE TO BE LEAKING. BOTH CABIN AND COCKPIT PAC ASPIRATOR HOSES WERE REPLACED WITH NEW. AIRCRAFT PRESSURIZATION CHECK AND FUNCTIONAL TEST OF THE CABIN AND COCKPIT PACâS WERE NORMAL. AIRCRAFT WAS RETURNED TO SERVICE.
WHILE CONDUCTING A FLEET WIDE FIRE LOOP INTERFERENCE INSPECTION, THE FOLLOWING ITEM WAS FOUND ON ENGINE NR 2. REPLACED FUEL LINE (P/N 23056762) DUE TO CHAFFING BEYOND LIMITS. NO DEFECTS WERE NOTED ON THE NR 1 ENGINE.
DURING CLIMB OUT, THE FLIGHT CREW RECEIVED A RED RT BLEED LEAK CAS MSG. THIS WAS A REPEAT MSG. THE DECISION WAS MADE TO DIVERT THE ACFT FOR MX. IT WAS DETERMINED THAT THE FWD HORIZONTAL STABILIZER TH002 & THE AFT HORIZONTAL STABILIZER TH004 BLEED TEMPERATURE AIR SENSORS WERE FAULTY PN S101816PG. BOTH SENSORS WERE REPLACED & OPERATIONAL CHECKS WERE ACCOMPLISHED WITH NO FAULTS NOTED. THE ACFT WAS RETURNED TO SERVICE WITH NO FURTHER ISSUES.
