CREW NOTICED A LANDING GEAR DOOR UNSAFE INDICATION IN FLIGHT. THE GEAR WAS LOWERED WITH GEAR DOWN AND LOCKED INDICATIONS NORMAL. INSPECTION REVEALED THAT THE OB RT GEAR DOOR HINGE WAS CRACKED WHICH CAUSED THE DOOR TO NOT FULLY CLOSE. THE CONDITION WAS AGRIVATED BY HIGHT AIRSPEED. (K)

THE AIRCRAFT WAS EN ROUTE TO PICK UP PASSENGERS IN SARASOTA, FL PASSING THROUGH 41,000 FEET HEADING FOR 43,000 FEET, THE LEFT ENGINE FLAMED OUT. PILOT'S DESCENDED TO 25,000 FEET AND RELIT THE ENGINE ACCORDING TO CHECKLIST PROCEDURES. ACFT WAS LANDED WITHOUT INCIDENT. MECHANIC REMOVED COWLINGS AND INSPECTED ENGINE. NO DISCREPANCIES NOTED AND THE ENGINE WAS GROUND RUN TO CHECK PERFORMANCE. FINDING ALL PERFORMANCE PARAMETERS NORMAL, ACFT WAS FLOWN BACK TO HOME BASE AT 33,000 FEET WITHOUT FURTHER INCIDENT.

PILOT FELT LOSS OF ELEVATOR AUTHORITY DURING TAKEOFF ROLL. TAKEOFF WAS ABORTED WITHOUT INCIDENT AND AIRCRAFT WAS TURNED OVER TO MAINTENANCE. THE RIGHT AFT ELEVATOR CONTROL CABLE WAS FOUND FRAYED AND BROKEN BETWEEN THE TWO UPPER PULLEY SETS. INSTALLATION OF A NEW CABLE USING SAME ROUTING REVEALED THAT CABLE COULD CHAFE ON A BOLT THAT RETAINS THE YAW DAMPENER SERVO. FURTHER INSPECTION SHOWED THE RUDDER CABLE (RIGHT) AND ELEVATOR CABLE CROSSED BETWEEN THE UPPER PULLEY SETS. CORRECTION OF CABLE ROUTING ELIMINATED CHAFE SITUATION. RUDDER CABLE WAS NOT CHANGED WITH ELEVATOR CABLE. THIS AREA IS DIFFICIULT TO INSPECT DUE TO BLEED AIR PLUMBING.
