Maintenance disclosures
AIRCRAFT EXPERIENCED A HYDRAULIC B LEAK AND AN ASSOCIATED HYD LOW VOL AMBER CAS MESSAGE. THE FLIGHT CREW ELECTED TO RETURN TO LNK. AN EMERGENCY WAS DECLARED, AND THE AIRCRAFT LANDED UNEVENTFULLY. MAINTENANCE WAS CONTRACTED TO INSPECT AND REPAIR THE AIRCRAFT. TECHNICIANS FOUND AN O-RING PACKING (P/N NAS1612-10) ON THE R/H HYDRAULIC LOW-PRESSURE SWITCH (ST016) TO BE PROTRUDED FROM THE UNION TEE FITTING. THE FAILED PACKING WAS REMOVED AND REPLACED. HYDRAULIC SYSTEM LEAK CHECKS WERE COMPLETED WITH NO DEFECTS NOTED. THE AIRCRAFT WAS SUBSEQUENTLY RETURNED TO SERVICE WITH NO FURTHER DISCREPANCIES.
WHILE CONDUCTING A FLEET WIDE FIRE LOOP INTERFERENCE INSPECTION, THE FOLLOWING ITEM WAS FOUND ON ENGINE NR 1. REPLACED OIL LINE (P/N 23056412) DUE TO CHAFFING BEYOND LIMITS. NO DEFECTS WERE NOTED ON THE NR 2 ENGINE.
WHILE ON DEPARTURE FROM KLRD, FLIGHT CREW RECEIVED AN AMBER PITCH FEEL FAIL CAS MESSAGE, THE CHECK LIST WAS THEN ACCOMPLISHED WHICH STATES TO LAND AS SOON AS POSSIBLE. CREW ELECTED TO RETURN TO KLRD, DECLARED AN EMERGENCY AND LANDED WITHOUT INCIDENT. DURING TROUBLESHOOTING, MAINTENANCE FOUND A FAILED RT ELEVATOR LINEAR ACTUATOR P/N 9914558-10 S/N 112U AND A FAILED ELEVATOR PITCH NK122 MODULE P/N 6718476-7 S/N 0217. BOTH THE ACTUATOR AND PCB WERE REPLACED, ALONG WITH THE LT ELEVATOR LINEAR ACTUATOR, THIS WAS REPLACED OUT OF PRECAUTION. A SATISFACTORY PITCH FEEL SYSTEM FUNCTIONAL TEST WAS ACCOMPLISHED.
WHILE LEVELING OFF AT FL410, THE FLIGHT CREW RECEIVED AN AMBER, CABIN ALTITUDE, CAS MESSAGE. THE CREW DONNED THEIR O2 MASKS AND REQUESTED FL380; AT THIS ALTITUDE IT WAS NOTED THAT THE CABIN PRESSURE WAS NOT STABILIZING, SO AN EMERGENCY DESCENT WAS INITIATED. THE CREW DECLARED AN EMERGENCY AND DIVERTED, WHERE THEY LANDED WITHOUT INCIDENT. MAINTENANCE TECHS REPLACED A FAULTY PRESSURE CONTROLLER, PN 2119862-3 AND ACCOMPLISHED OPS CHECKS, WITH NO DEFECTS NOTED. ACFT WAS RETURNED TO SERVICE.
WHILE IN CRUISE FLIGHT, THE FLIGHT CREW NOTED AN ELEC SMELL AS THE MICROWAVE OVEN WAS IN OPERATION. NO PHYSICAL INDICATIONS OF FIRE OR SMOKE WAS NOTED. AS A PRECAUTION, THE FLIGHT CREW ELECTED TO DIVERT. THE MICROWAVE WAS SUBSEQUENTLY REMOVED FROM THE ACFT FOR REPAIR & THE ACFT RETURNED TO SERVICE.
DURING CLIMB OUT & EN ROUTE, FLIGHT CREW RECEIVED AN AMBER SLAT ANTI-ICE COLD RT ALONG WITH WING ANTI-ICE HOT RT CASS MESSAGES. ACFT RETURNED FOR MAINTENANCE. THE RT WING ANTI-ICE BLEED AIR TUBE D-RING ROLLED. D RING WAS R & R, LEAK & OPS CHECKED NORMAL.
FIGHT CREW REPORTED THAT ON LIFT OFF, PILOT RECEIVED A STICK SHAKER AND RED AIRSPEED TAPE STALL INDICATION THAT LASTED 2-3 MINUTES. LEVELED OFF AT 2,500 - 3,000 FT AND DECLARED AN EMERGENCY. RETURNED AND LANDED UNEVENTFULLY. FUTHER INVESTIGATION FOUND THE NR 1 ADC CONNECTOR WAS NOT FULLY ENGAGED AND LOCKED. ENGINE FADECS DOWNLOADED AND CIRCUIT BREAKERS RESET TO RETREAVE TROUBLSHOOTING DATA. CONNECTOR WAS SECURED AND ADC RE-RACKED. SYS OPS CHECKS WERE PERFORMED AND ENGINE RUNS WERE PERFORMED WITH NO FURTHER FAULTS FOUND. A SUCCESSFUL FLIGHT WAS PERFORMED. THE ACFT WAS RETURNED TO SERVICE WITH NO FURTHER ISSUES.
