PILOT HEARD A POP DURING GEAR EXTENSION. PILOT RETRACTED AND EXTENDED GEAR AND NOSE GEAR WAS NOT DOWN AND LOCKED. NO GREEN LIGHT, GEAR TRAILING AT A 45 DEGREE ANGLE. UPON LANDING, THE NOSE GEAR FOLDED AFT. INVESTIGATION REVEALED THAT THE BELLCRANK NEAR THE AFT END OF THE NOSE GEAR WELL HAD FAILED AT THE FORK BOLT. NO EVIDENCE OF PRIOR CRACK ON THE BELLCRANK. THIS AIRCRAFT HAD EXPERIENCED A BELLY LANDING WHERE IT APPEARED THAT THE GEAR WAS BEGINNING TO EXTEND WHEN THE RUNWAY WAS CONTACTED. THAT BELLY-IN WAS JUST HOURS PRIOR TO THIS FAILURE. THE ABOVE BELLCRANK WAS INSPECTED VISUALLY, AND NOT REPLACED DURING REPAIRS. SUSPECT STRESS OF BELLY-IN CAUSED FAILURE. SUGGEST BETTER INSPECTION.

LEFT MLG INBD DOOR CAME OPEN AFTER TAKEOFF, REPORTED BY CONTROL TOWER. LANDING MADE WITHOUT INCIDENT. FND GEAR RETRACT IDLER BELLCRANK BROKEN 2 INCHES FROM END OF ARM THAT RUNS GEAR DOOR EXTEND/RETRACT LINKAGE. BELLCRANK SHOWS EVIDENCE OF HAVING BEEN CRACKED ONE-THIRD OF ITS THICKNESS FOR SOME TIME, MONTHS PERHAPS. THE RT BELLCRANK ON ACFT CRACKED AND LATER BROKE IN SIMILAR FASHION ABOUT 1 YEAR PRIOR. SUSPECT PRIOR IMPROPER RIGGING AND TOO MUCH GEAR DOOR TENSION WHEN GEAR DOWN. SUGGEST CLOSE INSPECTION AS THIS BELLCRANK IS IN SUCH A LOCATION THAT IT IS DIFFICULT TO INSPECT THOROUGHLY. SUGGEST CLOSE ADHERANCE TO CESSNA RIGGING PROCEDURES. CRACK BEGAN AT TOP.

EXCESS WIND NOISE FORM EMERGENCY EXIT WINDOW HAS BEEN A RECURRING PROBLEM. EACH TIME MAINTENANCE PERSONNEL HAVE TO RESECURE FORWARD PIN WHICH IS CONNECTED TO WINDOW RELEASE CABLE, THE CABLE HAS BEEN TOO SHORT AND THE PIN WOULD BARELY REACH. CLOSER INVESTIGATION REVEALED THE HAT SECTION WHICH HOLDS THE RELEASE CABLE HANDLE WAS INSTALLED UPSIDE DOWN APPARENTLY FROM THE FACTORY. NO EVIDENCE OF EVER BEING REMOVED. THE RELEASE CABLE HAD TO PASS BELOW THE MS20392-2 FLAT HEAD PIN WHICH GIVES IT AN 'S' TURN WHEN HAT SECTION 0812500-1 IS UPSIDE DOWN. HAT SECTION WAS TURNED OVER, NO NEW HOLES REQUIRED, ONLY DRILLING RIVETS OUT AND REPLACING, AND CABLE LENGTH BECAME ADEQUATE.
