(REPEAT EVENT) FOLLOWED FIM 21-20-00-810-801-A PER WORKSCOPE PROVIDED, A/C HAD COMPRESSOR WASH DONE TO E1 ON 5/19, PERFORMED ENGINE RUN FOR 15 MINS WITH DOORS OPEN PER FIM. PERFORMED ENGINE RUN UPS TO 70% WITH CREW AS REQUESTED BY MOD. NO SMELLS NOTED. PERFORMED BORESCOPE INSPECTION OF LH ENGINE HPC STAGES 1, 2, AND 3 IAW GE CF34-8E LMM 72-00-00. NO SMELLS NOTED. UNABLE TO REPLICATE SMELL ON GND.
UNIDENTIFIED ODOR IN FLIGHT DECK AND CABIN. ODOR STARTED DURING DEPARTURE SHORTLY AFTER TAKEOFF, STOPPED AFTER 3 MINS. ODOR RETURNED AT TOP OF DESCENT. USING QRH FUMES, ODOR ISOLATION CHART, SWEET, WET DOG, RUBBER TYPE ODOR, CAT A. PERFORMED BORESCOPE INSPECTION ON LT ENG STAGES 1,2, & 3 OF HPC LOOKING FOR OIL RESIDUE, OIL WETNESS DUE TO OIL ODOR, FUMES, SMOKE IN THE CABIN OR COCKPIT. REFER TO SUBTASK 72-00-00-810-021. FOUND OIL RESIDUE ON IGV & STAGE 1. ACCOMPLISHED E1 COMPRESSER WASH IAW LMM 72-00-00 CLEANING 002. NO ODOR OR FUMES NOTED. ENG: GE: CF348E, SER: 902528.
DURING TAKEOFF, THE AIRCRAFT PUTS OUT A BAD ODOR INTO JUST THE COCKPIT. THE FLIGHT ATTENDANT IN THE BACK SAYS THERE'S NO SMELL TO THEM. IT STARTS GIVING THE SMELL AGAIN DURING THE DESCENT AROUND 18000 FEET BUT NOT NEAR AS SEVERE DURING THE TAKEOFF. DURING TAKEOFF IT TAKES ABOUT 10 MINUTES FOR THE SMELL TO FULLY DISSIPATE OUT OF THE COCKTAIL. THE SMELL IS BAD ENOUGH THAT YOU NEARLY NEED TO PUT ON THE OXYGEN MASK. ACCOMPLISHED DUAL HEAT EXCHANGER CLEANING IAW EJ-2100-006. NO FURTHER DAMAGES NOTED. MEL RESTORED.
