DURING THE APPROACH TO LAND, LDG GEAR HANDEL FAILED FOLLOWED BY 3 UNLOCK LIGHTS ON BOTH SYSTEMS; LDG GEAR GRAVITY EXTENSION HAS BEEN USED FOR LANDING. PROBLEM WAS FOUND TO BE A FAILURE OF THE LANDING GEAR SELECTOR VALVE. A SERVICABLE VALVE WAS INSTALLED AND AIRCRAFT WAS RETURED TO SERVICE.
DURING CLIMB OF FLIGHT 7129 POS-BQN, AIRCRAFT 800FX INCURRED A PITCH DISCONNECT MASTER WARNING ALERT. THE CREW PERFORMED THE QRH PROCEDURES, DECLARED AN EMERGENCY, AND RETURNED TO POS UNEVENTFULLY. CONTRACT MAINTENANCE WAS NOTIFIED AND TROUBLESHOT THE ISSUE. NO FAULTS WERE FOUND, AND AIRCRAFT WAS OPS CHECKED GOOD. FLIGHT 7129 DEPARTED A SECOND TIME BUT THE SAME ISSUE OCCURRED. THE CREW RETURNED TO POS WITH NO FURTHER ISSUES.
DURING CLIMB, CREW RECEIVED AN AUTO PRESSURIZATION FAULT AND AN EXCESS CABIN ALTITUDE ALERT. CORRECTIVE ACTION IN WORK.
DURING CRUISE ON FLIGHT, WITH THE AUTOMATIC PRESSURIZATION ALREADY DEFERRED, THE CREW RECEIVED AN EXCESSIVE CABIN ALTITUDE WARNING AND CONTINUOUS CHIME. AFTER CONSULTING THE QRH, THE CREW DECLARED AN EMERGENCY AND RETURNED TO INDIANAPOLIS WITHOUT FURTHER INCIDENT. MAC MAINTENANCE PERFORMED A SUCCESSFUL OPERATIONAL CHECK OF THE MANUAL PRESSURIZATION CONTROL.
DURING THE PRE-FLIGHT OF AIRCRAFT N800FX, FLIGHT 8327, IND-LCK, THE FLIGHT DECK EMERGENCY ESCAPE ROPE WAS DISCOVERED TO BE IMPROPERLY INSTALLED. ALTHOUGH THE ESCAPE ROPE WAS PRESENT, THE ATTACH RING WHICH SECURES THE ROPE TO THE AIRCRAFT STRUCTURE WAS MISSING. A REPLACEMENT ATTACH RING WAS LOCATED, INSTALLED IAW WITH AMM PROCEDURES, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
#2 DC GENERATOR FAULTED, WOULD NOT RESET. FOLLOWED BY AHRS 1 NORM SUPPLY CB POPPED, AHRS 1 AUX SUPPLY CB POPPED. THE CREW REPORTED AN ELECTRICAL SMELL IN THE COCKPIT AND DONNED O2 MASKS. MAINTENANCE REPLACED #1 AHRS COMPUTER AND #2 STARTER GENERATOR WITH A SUCCESSFUL OPERATIONAL CHECK.
LANDING GEAR HANDLE IS STUCK IN THE GEAR DOWN POSITION, COULD NOT RETRACT LANDING. MAINTENANCE REPLACED THE LANDING GEAR CONTROL HANDLE.
WHILE CLIMBING THROUGH 16,000 FEET NR 1 ENGINE FAILED WITH A LOSS OF OIL PRESSURE INDICATED ON THE INDICATOR AND LOW OIL PRESSURE WARNING AND UNABLE TO FEATHER ENGINE. THE ENGINE WAS MANUALLY SHUT DOWN AND AN EMERGENCY WAS DECLARED. THE AIRCRAFT RETURNED TO DEPARTURE AIRPORT AND LANDED WITHOUT FURTHER INCIDENT. REPLACED NR 1 ENGINE TURBO MACHINERY AND REDUCTION GEAR BOX. TEARDOWN CONDITION REPORT ON REMOVED ENGINE PENDING.
AFT CARGO DOOR BOTTOM CORNER HAS A DENT. MAINTENANCE COMPLIED WITH AFT CARGO DOOR REPAIR IAW M7 ICAW EO 72R47001-07 REV (IR) DTD JULY 31 COCO AND M7 DWG#72R47001-07 REV (IR) DTD JULY 31 2020.
AFTER TAKEOFF FLAPS ASSYMETRY LIGHT ILLUMINATED ALONG WITH MASTER CAUTION FLIGHT CONTROL. AIRCRAFT RETURNED AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED #1 MULTI-FUNCTION COMPUTER (MFC)AND OPERATIONALLY CHECKED GOOD.
DURING LANDING FLARE NR 2 FIRE WARNING ACTIVATED. WARNING STOPPED UPON LANDING BEFORE ANY ACTION COULD BE TAKEN.
ON SHORT APPROACH, THE CREW WAS ALERTED WITH A MASTER WARNING, CONTINUOUS REPEATING CHIME AND A NR 2 FIRE INDICATION. THE ALARM SYSTEM ACTIVATED FOR APPROXIMATELY 15 SECONDS AND THEN CEASED BEFORE TOUCHDOWN. THE FLIGHT LANDED WITHOUT INCIDENT AND NO EMERGENCY WAS DECLARED. LEAK CHECKED BOTH BLEED AIR DUCTING AND DE-ICING DUCTING IAW AMM JIC 36-22-00-CHK-10010,FOUND COMPENSATED FLEX SLIDE DUCT TO BE LEAKING AT SLIDE AND CLAMPS. DE-ICE DUCTING CHECK GOOD. REPLACED BOTH CLAMPS, SEALS, AND COMPENSATED FLEX SLIDE DUCT IAW AMM JIC 36-11-00-RAI-10005. RETESTED FOR LEAKS. NO LEAKS FOUND.
WHILE ON A VISUAL APPROACH, AT ABOUT 800FT AGL, THE NR 2 ENGINE FIRE WARNING ACTIVATED. DUE TO BEING ON SHORT FINAL, THE CREW DECIDED TO CONTINUE THE APPROACH, DECLARED AN EMERGENCY AND WHEN ON THE GROUND INITIATE THE ON GROUND ENGINE FIRE CHECK LIST. THE NR 2 ENGINE WAS INSPECTED WITH NO EVIDENCE OF FIRE. SYSTEM TROUBLESHOOTING WAS COMPLIED WITH AND A CONTAMINATED CONNECTOR WAS FOUND AT FIN 426VC. THE CONNECTOR WAS CLEANED IAW SPM 20-55-00. AN OPERATIONAL TEST WAS SUCCESSFULLY PERFORMED IAW JIC 26-12-00 OPT 10000.
NR 2 ENGINE FIRE INDICATION ON LANDING. TROUBLESHOT SYSTEM IN ACCORDANCE WITH FAULT ISOLATION MANUAL 26-12 FIGURE 104. INSPECTED FIRE LOOPS FOR CHAFFING, MISSING BUSHINGS AND QUICK RELEASE CLIPS. INSPECTED FOR BLEED LEAKS. NO LEAKS OR EVIDENCE OF LEAKS THROUGH ITT OR MARKS AROUND BLEED LINES. RAN LT AND RT ENGINES IN ACCORDANCE WITH JIC 72-00-00-ERU-10030-008 AND 72-00-00-ERU-10040-004. FOR PRECAUTIONARY REASONS, REMOVED AND REPLACED ENGINE FIRE DETECTION CONTROL UNIT IN ACCORDANCE WITH 26-12-81 RAI 1000-001. OPS CHECKS GOOD IN ACCORDANCE WITH ATR72 JIC 26-12-00 OPT 1000-001.
MAIN CARGO DOOR PULL ROPE ANCHOR PLATE PULLED THROUGH ITS ATTACHMENT TO CARGO DOOR FRAME.
ON FINAL APPROACH, THE NR 2 ENGINE FIRE WARNING ILLUMINATED FOR APPROXIMATELY THREE SECONDS AND THEN EXTINGUISHED. THE AIRCRAFT LANDED WITHOUT INCIDENT. THE NR 2 FIRE HANDLE WAS REPLACED, OPS CHECKED GOOD, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
WHILE ON APPROACH, THE NR 2 ENGINE FIRE WARNING ANNUNCIATED. THE CREW DECLARED AN EMERGENCY. THE AIRCRAFT LANDED WITHOUT INCIDENT. A DVI OF THE FIRE-DETECTION SYSTEM WAS PERFORMED, AND THE SENDING ELEMENTS WERE CHECKED FOR RESISTANCE AND CLEANED. THE NR 2 ENGINE FIRE-DETECTION CONTROL UNIT WAS REPLACED, OPS CHECKED GOOD, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
WHILE ON APPROACH, THE NR 2 ENGINE FIRE WARNING ANNUNCIATED. DECLARED AN EMERGENCY, BUT TOOK NO FURTHER ACTIONS UNTIL LANDING. THE AIRCRAFT LADED WITHOUT INCIDENT. AFTER LANDING, THE FIRE WARNING EXTINGUISHED. AS A PRECAUTION, THE CREW RAN THE QRH ON GROUND ENGINE FIRE CHECKLIST AND SHUT THE AIRCRAFT DOWN ON THE RUNWAY. THE NR 2 ENGINE WAS INSPECTED WITH NO EVIDENCE OF FIRE. THE AIRCRAFT WAS TAXIED TO THE GATE VIA THE NR 1 ENGINE. THE FIRE LOOP WAS REPLACED, THE SUBSEQUENT OPERATIONAL CHECK WAS GOOD, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FINAL APPROACH, THE NR 2 ENGINE FIRE WARNING ANNUNCIATED. AN EMERGENCY WAS DECLARED. JUST BEFORE TOUCHDOWN, THE WARNING EXTINGUISHED. THE AIRCRAFT LANDED WITHOUT INCIDENT. THE NR 5 ENGINE FIRE LOOP CONNECTOR ON THE NR 2 ENGINE WAS INSPECTED AND CLEANED. THE SUBSEQUENT OPERATIONAL CHECK WAS GOOD, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING TAXI, THE COPILOT'S LT BRAKE PEDAL BOTTOMS OUT WITH MINIMAL BRAKE PRESSURE. THE MECHANIC REPLACED THE MASTER CYLINDER IAW JIC 32-42-13, RAI 1000. THE SYSTEM CHECKED GOOD AND THE ACFT WAS RETURN TO SERVICE.
LEFT WING FLAP BEAM LATERAL LINK FITTING LUG, OVERSIZED AT RIB 13. LUG OVERSIZED AND BUSH INSTALLED IAW SRI DT/S-223/14-21209.
AT 50 FEET BEFORE LANDING, NR 2 FIRE WARNING ILLUMINATED THEN OFF, LASTED 5 SECONDS. TROUBLESHOT AND REPLACED THE FIRE DETECTION CONTROL UNIT FOR NR 2 ENGINE IAW JIC 26-12 81 RAI 10000. THE SYS CHECK GOOD AND THE ACFT WAS RETURNED TO SERVICE.
RIGHT INBOARD SEAT TRACK HAS PITTING CORROSION OUT OF LIMITS. REPLACED SEAT TRACK P/N S53678001228.
DURING INITIAL CLIMBOUT OF REPOSITION FLIGHT 7559 MSN-CID, MADE A PRECAUTIONARY AIR TURNBACK INTO MSN DUE TO ABNORMAL PITCH FORCES. INSPECTION OF AIRCRAFT FOUND ICE ON THE RIGHT ELEVATOR. REMOVED ICE AND PERFORMED VISUAL INSPECTION OF ELEVATOR TRIM IAW ATR72 JIC-30-00 DUE 10000 AIRCRAFT RETURNED TO SERVICE.
DURING TAKEOFF ROLL OF FLIGHT 7499 CRW-MEM, THE PILOTS AIRSPEED AND ALTIMETER FLAGS APPEARED. THE FLIGHT CREW ABORTED THE TAKEOFF AND RETURNED TO THE GATE. AFTER TROUBLESHOOTING THE MECHANIC REPLACED THE DADC IAW ATR72 JIC 34-11-81 THE SYSTEM CHECKED GOOD AND THE AIRCRAFT WAS RETURN TO SERVICE.
