ENROUTE KHVN TO KRDU AT FL260 AN EICAS âR GEN TIE OPENâ ILLUMINATED. FOLLOWED THE CHECKLIST AND CLEARED EICAS. AFTER ABOUT 4 MINUTES âR GEN TIE OPENâ AGAIN ILLUMINATED FOLLOWED BY âR GENâ ANNUNCIATOR. THE âR GENâ EICAS L/R âFIREâ ILLUMINATED IN THE TORQUE GAUGES FOR ONLY A FEW MOMENTS THEN EXTINGUISHED, THERE WAS NO SECONDARY INDICATIONS OF FIRE. EMERGENCY WAS DECLARED AND DIVERTED TO KACY. LANDING WAS UNEVENTFUL. MAINTENANCE ISOLATED THE DISCREPANCY TO A RIGHT ENGINE STARTER GENERATOR ARMATURE FAILURE. SG WAS REPLACED. PERFORMED PWC ENGINE REQUIREMENTS FOR SG FAILURE. AIRCRAFT WAS RETURNED TO SERVICE.
ON APPROACH FLAPS WOULD NOT EXTEND PAST THE APPROACH POSITION. EMERGENCY DECLARED. LANDING WAS UNEVENTFUL. MAINTENANCE REPLACED THE RIGHT WING FLAP SAFETY SWITCH. OPERATIONAL CHECKED OKAY. AIRCRAFT WAS RETURNED TO SERVICE.
RUDDER TRIM FROZE DURING CRUISE FLIGHT. OPERATED NORMAL AT LOWER ALTITUDES. MAINTENANCE PERFORMED THE REQUIREMENTS OF NTO LETTER FOR TRIM ACTUATOR LUBE. OPS CHECK FLIGHT WAS UNSUCCESSFUL. AIRCRAFT DIVERTED. MAINTENANCE REPLACED THE RUDDER TRIM ACTUATOR.
DURING SCHEDULED ROUTINE RIGHT ENGINE MAINTENANCE BORESCOPE INSPECTION, REVEALED FOD DAMAGE. ENGINE WAS REMOVED AND REPLACED WITH A RENTAL UNIT. A SUCCESSFUL OPERATIONAL CHECK FLIGHT WAS PERFORMED. AIRCRAFT WAS RTS.
WHEN LT ENG START SWITCH WAS PLACED INTO, IGNITION & ENG START POSITION, NO N1 ROTATION WAS OBSERVED & VOLTAGE ON THE VOLTAGE SELECTOR DROPPED TO ZERO. APPROX THREE SECONDS INTO THE START SEQUENCE, SMOKE WAS OBSERVED BELLOWING OUT FROM TOP OF ENG COWL VENT. AN IMMEDIATE ENG SHUTDOWN SEQUENCE WAS INITIATED & ACFT WAS EVACUATED. CREW & PAXS EVACUATED THE ACFT WITHOUT INCIDENT. AFTER SHUTDOWN THERE WAS NO MORE SMOKE OBSERVED FROM ENG COWL. REPLACED THE LT STARTER GEN & A SUCCESSFUL OPERATIONAL CHECK WAS PERFORMED. ACFT WAS RETURNED TO SERVICE.
