Maintenance disclosures
DURING PREFLIGHT INSPECTION, FLIGHT CREW REPORTED THAT THE TAIL CONE LIGHT WAS INOPERATIVE. ADDITIONALLY, WHEN THE EMERGENCY LIGHTING SYSTEM WAS ACTIVATED, MULTIPLE CEILING LIGHTS DID NOT ILLUMINATE AS REQUIRED. THE EMERGENCY EXIT LIGHT AT THE L2 DOOR WAS ALSO FOUND INOPERATIVE. THE DISCREPANCIES WERE IDENTIFIED ON THE GROUND AND RECORDED IN THE AIRCRAFT MAINTENANCE LOG. MAINTENANCE PERFORMED TROUBLESHOOTING AND CORRECTIVE ACTIONS IN ACCORDANCE WITH APPLICABLE PROCEDURES, AND THE SYSTEM WAS RETURNED TO SERVICE AFTER SATISFACTORY OPERATIONAL CHECKS.
DURING DAILY CHECK, LOW PRESSURE INDICATION WAS NOTED ON THE L2 EVACUATION SLIDE. TROUBLESHOOTING CONFIRMED EVACUATION SLIDE PRESSURE WAS BELOW ACCEPTABLE LIMITS. THE L2 EVACUATION SLIDE WAS REMOVED AND REPLACED WITH A SERVICEABLE OVERHAULED UNIT IN ACCORDANCE WITH MD-80 AMM 25-62-00. AIRCRAFT WAS RETURNED TO SERVICE FOLLOWING INSTALLATION, FUNCTIONAL CHECKS, AND RII INSPECTION. NO IN-FLIGHT EMERGENCY OCCURRED.

1. RIGHT ENGINE FAILURE DURING CRUISE, FOLLOWED QRH PROCEDURES AND DIVERTED TO BRO, DECLAIRED EMERGENCY UNEVENTFUL LANDING. 2. DURING WALK-AROUND, DISCOVERED PASSENGER CABIN WINDOW AT ROW 11 R/H WAS DAMAGED. 1. DURING T/S FOUND #2 ENGINE HAD CONSIDERABLE FOD DAMAGE. R/R # 2 ENG IAW W.O. #4863, TC 71-900-01-01. PERFORMED ADJUSTMENT/TEST ENG GROUND RUN-UP ENG CHANGE IAW TC 71-900-02-01. ITEMS S,A,B, +C, SYNCHRONIZATION SYSTEM TEST 6, ELECTRICAL SYSTEM CHK + ITEM, ENGINE GENERATOR FIELD DISCONNECT + OPERATIONAL CHECK COULD NOT BE ACCOMPLISHED DUE TO R/H GENERATOR FAILURE TO COME ONLINE. ADJUSTED PART POWER TRIM AND ENG LOW IDLE TRIM IAW MD80 AMM 71-00-00. ALL PARAMETERS WITHIN LIMITS. C/W BOEING TC 76-021-01-02, ENG EMERGENCY SHUTDOWN SYSTEM. OPS CHK IS SATISFACTORY. 2. R/R PASSGENER CABIN WINDOW AT ROW 11 R/H IAW MD80 AMM 56-20-00.

PERFORMED INSPECTION AND CHECK OF LT SECONDARY HEAT EXCHANGER AND FOUND CRACKED. R&R LT SECONDARY HEAT EXCHANGER. OPS CHECK NORMAL. NO LEAKS NOTED.
ON TAKE OFF RT ENGINE APPROACHED CAUTION ZONE EGT. AFTER TAKE OFF RT ENGINE EGT 75 DEGREES CELSIUS HIGHER THAN LT ENGINE 560 DEGREES CELSIUS OIL TEMP EXCEEDED 130 DEGREES CELSIUS. FUEL FLOW RT ENGINE 1000 PPH HIGHER THAN LT ENGINE
DURING CLIMB THRU FL270 TAIL COMPARTMENT TEMP HIGH ANNUNCIATOR LIGHT ON. FOUND PRIMARY HEAT EXCHANGER DUCT LEAKING. R&R PRIMARY HEAT EXCHANGER DUCT. NO LEAKS NOTED. OPS CHECK NORMAL.
AFT CENTER FUEL PUMP IS NOT PASSING TEST DURING PREFLIGHT.
SMOKE REPORTED IN CABIN. FOUND ROW 2AB SIDEWALL UPPER LIGHT WALL SOCKET HAD AN ELECTRICAL ODOR AND SMOKE. REMOVED A BURNED OUT BULB AT SECTION 2AB IAW 33-21-03. SMOKE WAS CLEARED AND NO ODOR.
FWD & AFT CENTER FUEL TANK BOOST PUMP CIRCUIT BREAKER TRIPPED DURING FLIGHT. TROUBLESHOT THE FUEL PUMP CONTINUITY AND RESISTANCE, CHECKED UNSATISFACTORY IAW AMM 28-20-07. REMOVED AND REPLACED THE FUEL BOOST PUMPS FWD AND AFT IAW AMM 28-20-07. OPS CHECKED THE BOOST PUMPS AND FOUND NORMAL IAW AMM 28-20-07.
FWD & AFT CENTER FUEL BOOST PUMP CIRCUIT BREAKER TRIPPED DURING FLIGHT. TROUBLESHOT THE FUEL PUMPS CONTINUITY AND RESISTANCE CHECKED UNSATISFACTORY IAW AMM 28-20-07. REMOVED AND REPLACED THE FUEL BOOST PUMPS FWD AND AFT IAW AMM 28-20-07. OPS CHECKED NORMAL IAW AMM 28-20-07.
AFTER LANDING AIRCRAFT WITH APU RUNNING AND 1 PACK ONLINE (RIGHT), SMOKE WAS PRESENT IN CABIN. APU TEMPERATURE RED LINE. REMOVED AND REPLACED APU IAW AMM 49-10-00. OPS CHECK NORMAL.
AIRCRAFT FAILED TO PRESSURIZE AFTER TAKE OFF. FOUND TO BE THE CABIN PRESSURE CONTROLLER.

CARGO DETECTION SUPPRESSION PANEL, ALL LIGHTS ILLUMINATED. PERFORMED TROUBLESHOOTING OF SMOKE DETECTION, FIRE SUPPRESSION SYSTEM IAW AAELTD SMOKE DETECTION STC DRAWINGS FC03-0282 AND -0283 AND MD80AMM 26-10-00. FOUND PIN NR 7 GROUNDED. RESEATED THE PIN IAW SWPM 20-31-00 AND PERFORMED OPS TEST. OPS TEST NORMAL, NO FAULTS NOTED.
DURING CLIMB DFGP INOP, FIRST OFFICER FLIGHT MODE ANNUNCIATOR INOP, AUTO THROTTLE SYSTEM INOP.
DURING CRUISE AT FL290, CABIN ALT STARTED TO CLIMB, RT PACK PRESS BEGAN OSCILLATING, FLOW LIGHT ILLUMINATED, PERFORMED QRH PROCEDURE AND BEGAN AND EMERGENCY DECENT. TROUBLE SHOT SYSTEM TO A DEFECTIVE RIGHT COOLING TURBINE. REMOVED AND REPLACED RT COOLING TURBINE IAW AMM 21-51-01 OPS REVEALED LOW AIR FLOW. REMOVED AND REPLACED RIGHT COOLING FAN IAW AMM 21-51-01 LEAK CHECK AND OPS CHECK IAW AMM 21-00-00. NO LEAKS NOTED OPS CK NORMAL AT THIS TIME.
WHILE ON APPROACH, AFTER SELECTING FLAPS 11, THE AIRCRAFT WENT INTO A FLAP ASYMMETRIC CONDITION. EXTENDED THE DOWNWIND LEG WHILE REVIEWING THE QRH. A SLAT ONLY LANDING WAS EXECUTED WITH NO FURTHER ISSUES.
AT FL350 CABIN PRESSURE WAS LOST AND CABIN ALTITUDE ANNUNCIATOR ILLUMINATED.
FIRE DETECTOR LOOP-B FAULT, FIRE DETECTOR LOOP-A NORMAL. TS ENGINE FIRE DETECTION SYSTEM IAW AMM 26-10-00. FOUND SENSING ELEMENT ON ENGINE NR 2 GROUNDED, SPACED ELEMENT AND PERFORMED OPS TEST IAW AMM 26-10-00. OPERATIONAL TEST NORMAL.
AT FL 320, CABIN BEGAN TO FILL WITH SMOKE. DECLARED AN EMERGENCY FOLLOWED QRH, DIVERT. PERFORMED EXTENSIVE T/S IAW AMM 49-90-00, FOUND LINE LOOSE AT APU OIL PRESSURE SENSOR. RESECURED OIL LINE IAW AMM 20-12-01, OPS CHECKED NORMAL IAW AMM 21-20-00, NO ODOR NOTED IN CABIN.
RIGHT ENGINE FAILURE AFTER TAKEOFF. EVENT HAPPENED ABOUT 1000 FT. ENGINE MADE A LOUD BANG, SUSPECTED CATASTROPHIC FAILURE. THRUST LOST IMMEDIATELY. R & R RT ENGINE. ACCOMPLISHED LEAK CHECK AND RUN-UP, NO LEAKS NOTED. CHECKED NORMAL IAW T/C 71-900-01-01 AND AMM 71-00-00 MD-80, T/C 71-900-02-01.

DOORWAY L-1 FLASHLIGHT NOT WORKING. REMOVED AND REPLACED FLASHLIGHT IAW MM 33-53-00. OPS CHECKED NORMAL.
NLG FAILED TO RETRACT AFTER TAKEOFF, AN AIR TURNBACK WAS EXECUTED. TROUBLESHOOTING FOUND NLG STRUT FLUID LOW. R & R SERVICING VALVE, SERVICED FLUID AND NITROGEN IAW AMM 32-21-02. OPS CHECKED NORMAL.

DURING PREFIGHT WITH ACFT POWERED WITH EXTERNAL BATTERY SWITCH ON AND EXTERNAL A/C ON, CABIN FILLED WITH BLACK SMOKE. PERFORMED OPS CHECK AND INSPECTED BATTERY SYSTEM AND EXTERNAL POWER, NO FAULTS FOUND IAW AMM 24-00-00, 24-30-01 AND 24-30-01. VERIFIED GROUND AC UNIT BURNING EXCESSIVE OIL.
WHILE STARTING NR 1 ENGINE ON TAXI WAY, TOWER REPORTED FLAME FROM TAIL PIPE. RETURN TO GATE. INSPECTED NR 1 ENGINE INLET AND EXHAUST, NO DISCREPANCIES NOTED AT THIS TIME, AMM 72-00-00.
MAIN CABIN EMERGENCY LIGHTS ROW 5 AND 6 INOPERATIVE. R & R MAIN CABIN EMERGENCY BATTERY PACK IAW AMM 33-53-00. OPS NORMAL.
RIGHT GENERATOR WENT OFF LINE DURING CLIMB, ATTEMPTS TO RESET WERE UNSUCCESSFUL, APU GENERATOR CAME ON LINE. INSPECTED RT GENERATOR RESET SWITCH FOR OPS, FOUND NORMAL IAW 30-31-00. TROUBLESHOT TO ELECTRICAL SYSTEM FOUND NORMAL IAW 24-20-01, OPS CHECKED SYSTEM FOUND NORMAL IAW 24-20-00 AND 24-10-00. INSPECTED APU RESET SWITCH FOR PROPER OPERATION FOUND NORMAL IAW SWPM 20-31-00.
DURING CLIMBOUT WHEN POWER REDUCED TO CLIMB THRUST VIBRATION ON RT ENGINE FOLLOWED BY FLAME OUT. PERFORMED GENERAL VISUAL INSPECTION OF RT ENGINE FOUND PRESSURE SENSE LINE FUEL CONTROL AT MOISTURE TRAP NOT SECURED. SECURED LINE AT MOISTURE TRAP IAW AMM 73-20-03 PERFORMED ENGINE RUN AND TEST ALL PARAMETERS NORMAL IAW AMM 72-00-00, TABLE 511.514.535
DURING SERVICE CHECK, FOUND LT AND RT OVER WING EMERGENCY EXIT LIGHTS INOPERATIVE. R & R BATTERY POWER SUPPLY IAW AMM 33-53-00, CHECKED NORMAL.
UNABLE TO RETRACT GEAR, RETURNED TO DEPARTURE. INSPECTION FOUND GROUND SENSING MECHANISM OUT OF ADJUSTMENT. ADJUSTED GROUND SENSING MECHANISM IAW AMM 32-23-01. PERFORMED EXTENSION AND RETRACTION TEST IAW AMM 32-30-00, CHECKED NORMAL.
DURING CRUISE AT FL 320 BOTH AUTO CONTROLS FAILED, UNABLE TO CONTROL CABIN, DECLARED AN EMERGENCY FOLLOWED QRH PROCEDURES. AIRCRAFT WAS READIED FOR MX FERRY FROM SJU TO MIA FOR REPAIRS. MX REMOVED AND REPLACED PERISEALS ON THE RIGHT SIDE PACK TURBINE INLET CONNECTOR ALSO REMOVED AND REPLACED NR 1 PACK ACM. PRESSURIZATION SYSTEM OPS CHECK AND LEAK CHECK NORMAL IAW AMM 21-20-15 & 21-50-10.
LIGHT SMOKE & HASE IN CABIN WAS REPORTED.
SMOKE IN COCKPIT, DECLARED EMERGENCY, DIVERTED. DISASSEMBLED, CLEANED AND INSPECTED UNIT IAW 27-60-07 REV NR 6, DATED 8/15/06. FOUND MOTOR ASSY DAMAGED, VARIOUS HARDWARE DAMAGED, CAPACITOR LEAKING, RECTIFIER OPEN, BEARINGS WORN, GEAR TRAIN LUBRICATION DRY, DATA PLATE UNREADABLE.
AFTER SLAT EXT DURING THE APP, LT HYD SYS PRESSURE CYCLES TO ZERO PRESS, QTY NORMAL.
AFTER TAKE OFF, LANDING GEAR DID NOT RETRACT, A/C WOULD NOT PRESURIZE.
