REMOVED AND REPLACED #3 MLG WHEEL ASSY PER AMM 32-45-11. ANTI-SKID CONTROL SYSTEM CHECKED, FAILED LOCKED WHEEL PROTECTION TEST INTERMITTENTLY. ITEM TRANSFERRED MEL 32-02-01, CAT C AUTH # D32JAN0326-1, EXP. 01/13/26. PER AMM, THE OPPOSITE WHEEL ON THE SAME AXLE REQUIRES REPLACEMENT. THE #4 TIRE WAS PLACED ON EO B737-32-45-01, WHEEL REPLACEMENT AT FIRST AVAILABLE MX OPPORTUNITY.
DURING TAKE-OFF FO NOTICED HYDRAULIC SYSTEM A "LOW PRESSURE" LIGHT ILLUMINATED. THEN CHECKED SYSTEM A HYDRAULIC QUANTITY WHICH WAS FOUND TO BE AT 0. PILOTS IMMEDIATELY DECLARED EMERGENCY.
ON LANDING AS THE NOSE TOUCHED DOWN THE AIRCRAFT STARTED VEERING TO THE LEFT. BOTH REVERSERS WERE NORMAL AND SPEED BRAKE UP. USED DIFFERENTIAL BRAKING BUT WOULD NOT HOLD ON CENTER LINE. USED RIGHT TILLER TO ASSIST BUT IT SUDDENLY JAMMED TO THE LEFT FORCEFULLY. A/C WENT OFF THE RUNWAY THROUGH THE GRASS ONTO A TAXIWAY.

BLEED TRIP OFF LIGHT ILLUMINATED VARIOUS TIMES WITHIN SECONDS. RAN QRH PROCEDURE TO RESET UNTIL IT WOULD NOT RESET ANYMORE, THEN TURNNED LT PACK OFF PER QRH AND CABIN ALT ROSE ABOVE 10. THE WARNING HORN ACTIVATED AND WENT TO MANUAL AC, CABIN ALT AND PRESS CHECKLIST AND INITIATED EMERGENCY DESCENT. PERFORMED T/S AND FOUND LOOSE DUCT CLAMP AT LT WIND TAI DUCT. SECURED CLAM, IAW B737 AMM 36-19-15 & 20-10-44. OPERATIONAL TEST NORMAL, NO LEAKS OR FAULTS FOUND.
R-1 EMERGENCY EXIT SIGN FAILS TO ILLUMINATE. CORRECTIVE ACTION: REMOVED AND REPLACED R-1 EMERGENCY LIGHT POWER SUPPLY AT STA.330L EQUIP #1133. OPS CHECK GOOD IAW B737-400 AMM 33-51-12

PERFORMED FUNCTIONAL TEST FLIGHT, NO DISCREPANCY NOTED.
WHEN LEAVING, LANDING GEAR POSITIONED UP, HANDLE JAMMED, HAD TO PULL THE TRIGGER. NOSE GEAR AND RT MLG INDICATION REMAINED RED. CONTINUED FOR PROPER MX SUPPORT. PERFORMED QRH PROCEDURE. SUPPLEMENTAL INFORMATION: ACCOMPLISHED AC JACKING IAW AMM 07-11-11, ACCOMPLISHED GEAR EXTENSION AND RETRACTION TEST IAW AMM 32-32-00 AND 32-33-00. OPS CHECK NORMAL AT THIS TIME IAW AMM 07-11-11, LIGHT INDICATION NORMAL AT THIS TIME.
DISCREPANCY: AT TOP OF DESCENT, AUTO-THROTTLES MOVED THROTTLES TO IDLE, NR 1 ENGINE MADE "POP" NOISE AND SHUTDOWN. PRIOR TO TOP OF DESCENT, ENGINE INDICATIONS WERE NORMAL. PERFORMED ENGINE FAILURE QRH. WAS ABLE TO RESTART ENGINE AND PERFORM NORMAL LANDING. SUBMITTED ENGINE TREND REPORT IN FLIGHT. CORRECTIVE ACTION: PERFORMED TROUBLESHOOTING IAW AMM 71-00-42. REMOVED AND REPLACED NR 1 ENGINE FUEL FILTERS IAW AMM 73-11-02, NO FINDINGS BORESCOPE OF NR 1 ENG ACCOMPLISHED IAW AMM 72-00-00 STAGES 1, 3, AND 8 BY BSI. RESULTS ON FILE. REMOVE AND REPLACE NR 1 ENG MEC IAW AMM 73-21-01-401, NO LEAKS FOUND ON MEC OR NR 1 ENG FUEL FILTERS. ACCOMPLISHED IDLE CHECKS, ACCEL/DECEL CHECK IAW AMM 71-00, OK FOR SERVICE AT THIS TIME.
DISCREPANCY: AIRCRAFT WILL NOT PRESSURIZE AUTO/STANDBY OR MANUAL CORRECTIVE ACTION: PERFORMED DETAILS VISUAL INSP OF CABIN OUTFLOW VALVE, PRESSURE CONTROLLER AND RACK CONNECTORS. VERIFIED AIR/GROUND LOGIC AND PERFORMED SATISFACTORY PRESSURIZATION OPS CHECK. NO FAULTS NOTED, ALL WORK IAW AMM 21-31-21, 21-31-00, AND 20-60-06, WDM 35-51-01
FWD CARGO BAY STA 500E HAS CRACK INDICATION AT 1.04" HOLE BETWEEN STR 20R AND STR 21R. REPAIRED FRAME AT STA 500E BETWEEN STR 20R AND STR 21R IAW SRM, REPAIR 13.
EMERGENCY LIGHTS INSIDE AND OUTSIDE OF CABIN ARE INOPERATIVE. REMOVED AND REPLACED BATTERY PACKS AT BS 753L AND 598R. OPERATIONAL CHECKED GOOD.
RTO TAKEOFF WARNING DURING INITIAL T/O ROLL. PERFORMED AURAL WARNING OPS TEST IAW AMM 31-26-00, 172 SYS TEST SATISFACTORY. NO HIGH ENERGY STOP.
