AT 133 KNOTS THE NR 1 ENGINE SURGED WITH FLAME-OUT, THE FRONT AND BACK. INSPECTION IN THE FRONT AND BACK, LOOKS OK. DUE TO AD 2002-21-10, THIS IS A GROUP 3 TAKEOFF SURGE AND THE ENGINE WILL BE CHANGED.
L2 DOOR SMELLS LIKE ELECTRICAL BURNING AND WILL NOT OPERATE ELECTRICALLY. REPLACED LOCK/RELEASE SOLENOID PER M/M. OPS GOOD. NO MORE BURNING SMELL.
STRONG ELECTRICAL BURNING ODOR DURING DESCENT, JUST AFTER WINDSHIELD HEAT WAS TURNED ON. CREW DEACTIVATED WINDSHIELD HEAT, TURNED ECON OFF AND ISOLATED THE NR 1 PACK. FUMES DISSAPATED. CREW COULD HEAR MOTOR FAN THAT SOUNDED AS IF IT WAS BINDING. FUMES SEEM STRONGEST AROUND C/B PANEL. FOUND AVIONICS COOLING FAN NR 3 WITH C/B POPPED. REPLACED FAN. FAN OPS NORMAL. C/B REMAINS CLOSED.
GEAR FAILED TO RETRACT ON FIRST ATTEMPT. SECOND ATTEMPT SUCCESSFULL. PERFORMED GEAR SWING TO DUPLICATE PROBLEM. SWUNG GEAR 4 TIMES. NO PROBLEMS NOTED. SYSTEM CHECKS GOOD. DURING GEAR SWING, FOUND LT GEAR OVERCENTER LOCK ACTUATOR LEAKING . R & R LT GEAR OVER CENTER LOCK ACTUATOR PER M/M.
AT CRUISE FLT ATTDS INFORMED US THERE WAS A STRONG ELECTRICAL SMELL IN THE AFT GALLEY CHILLER #8 RED FAIL LIGHT WAS FLASHING. SHUT DOWN CHILLER #8 AND #7. GALLEY POWER WAS ALSO TURNED OFF. DIVERTED TO JFK. SMELL SEEMED TO DISSAPATE. CORR ACTION: FOUND NBR 7 CHILLER FAN GRINDING. CHECKED AREA OF CHILLERS NO EVIDENCE OF ELECT WIRING CHAFFED OR BURNT. CHECKED AFT COOLING FAN FOUND OK #8 CHILLER OPS NORMAL. RPLD #7 CHILLER. PERFORMED OVER WEIGHT LANDING INSPECTION PER M.M.

ATB AURAL WARNING WITH NO VISUAL CAUSE, UNABLE TO SILENCE HORN. REPLACED CAWS UNIT.
PAX AT 3B NOTICED A HOT WIRE SMELL. THE CREW ALSO NOTICED THE SMELL. LATER F/A AND PASSENGER COMPLAINED OF HEADACHES INSPECTED CEILING FWD CABIN FOR BURNING WIRE CK OK. CHECKED SIDEWALL OF FWD CRAGO COMP FOUND NR 1 WATER COMP BEARING FAILURE. REPLACED NR 1 WATER COMP. CKS GOOD.
THERE WAS A SMALL FIRE OR SMOLDER IN 4R LAV TRASH BIN, IT WAS PUT OUT PROMPTLY BY THE FLIGHT ATTENDANT. NO DAMAGE VISABLE IN CAN. THE SMOKE ALARM DID NOT GO OFF. TESTED SMOKE DETECTOR ON ARRIVAL, CHECKED OK. NO EVIDENCE OF FIRE OR BURNED MATERIAL. TRASH COMPARTMENT AND CONTAINER IN SERVICABLE CONDITION. REPLACED SMOKE DETECTOR PRECAUTIONARY.
ODOR DETECTED IN AREA OF MONITOR 40J. MONITOR HAD BEEN FLICKERING, NO PICTURE. POWER REMOVED FROM MONITOR, ODOR DISAPPEARED. REPLACED MONITOR AND POWER SUPPLY.
FWD CABIN EMERGENCY LIGHT INOP. REMOVED AND REPLACED N01 BATTERY PACK IN THE NR 1 BATTERY POWER SUPPLY, GRD OPS CK OK.
IN CRUISE, FOUND CREW 02 BOTTLE EMPTY. FOUND FLEX LINE TO OBSERVER 02 REGULATOR LEAKING BADLY. REPLACED LINE AND REPLACED 02 BOTTLE. LEAK CK GOOD.
FIRE EXTINGUISHER BOTTLE AT 4L LOW PRESS. REPL BOTTLE.
THE RT FUSELAGE SKIN SURROUNDING THE FWD AND AFT LAV VACUUM WASTE AIR EXHAUST PORTS WERE FOUND CORRODED ON THE OUTER SURFACE AROUND THE AFT EDGES OF THE PORT CUTOUTS AT FS 1937 AND 1959 RESPECTIVELY. THE CORROSION BLEND OUT ON THE FWD PORT HOLE REQUIRED A MAX DEPTH OF 0.016. THE AFT PORT HOLE REQUIRED THAT THE SKIN BE CUTOUT IN THE DAMAGED AREA. FS 1937 - 0.063 EXT 2024-T3 REPAIR DOUBLER; FS 1956 - 0.071 2024-T3 EXT DOUBLER; GENERAL - 2 INTERNAL FINGER DOUBLERS - SEAL AND TREAT.
WITH A/P ON, WHEN FLIGHT SPOILERS WERE EXTENDED TO FULL, THE AIRCRAFT ROLLED LEFT TOWARD 30 DEGREE BANK. SPOILERS DON'T APPEAR TO EXTEND AT 1/3 AND 2/3 POSITION. AILERON TRIM WAS JAMMED DURING APPROACH, WILL NOT MOVE FROM ZERO POSITION. GROUND SPOILERS DID NOT EXTEND ON LANDING. FOUND ICE BUILDUP IN RIGHT WHEEL WELL IN AREA OF CONTROL CABLES. WATER LEAK FROM LAV -M ABOVE. REPLACED WATER FAUCET IN LAV-M.

ON TAKEOFF LOUD BANG LEFT SIDE, VIBRATION, ABORTED T/O. A/C REPORTED SMOKE AND FIRE, FIRED NR 1 AGENT BOTTLE. SHUTDOWN PER CHECKLIST. FOUND COMPRESSOR HAD FAILED, METAL IN TAILPIPE AND BELOW 2.5 BLEED OPENING. MTC IN PROGRESS.
NR 1 ENGINE FAILED DURING T/O RUN. LOUD BANG FROM LEFT SIDE OF A/C HEARD, T/O ABORTED AT 50 KNOTS. A/C REPORTED SMOKE AND FIRE. BLEW NR 1 FIRE BOTTLE AND SHUT DOWN ENGINE. NO EVIDENCE OF FIRE FOUND DURING INSPECTION. CHANGED ENGINE, METAL FOUND IN BLEED VALVE.
BOTH A/P INOP, NO CURRENT FAULTS. ACFT RETURNED ATL. PERFORMED RTS ON BOTH FCC, ALL NORMAL.
AFTER TAKEOFF AT 2,600 FT AND 270 KIAS, NR 2 ENGINE FAILED. A/C SHUDDERED, RPM ROLLED BACK AND TEMP WENT TO 754, FIRED EXT AGENT FOR NR 2 ENG. FLIGHT RETURNED TO LAX WHERE MAINTENANCE FOUND METAL IN TAIL PIPE. REPLACED ENGINE AND ROUTED TO SHOP.
