ON SELECTION OF GEAR UP AFTER TAKEOFF, THE LANDING GEAR FAILED TO RETRACT AND THE NR 2 HYD ISO VLV CAUTION LIGHT ILLUMINATED. EMERGENCY DECLARED AND AIRCRAFT RETURNED TO OGG. (MX) REPLACED THE NLG DOOR SOLENOID SEQUENCE VALVE. NOTE: 2 OF 4 HOUSING BOLTS ON THE SOLENOID VALVE FAILED.
ON APPROACH, "SMOKE" WARNING LIGHT CAME ON, REMAINED ON FOR 3 SECONDS THEN WENT OUT. AFTER LANDING LIGHT CAME ON AGAIN THEN WENT OUT AFTER A FEW SECONDS. VISUALLY INSPECTED NR 1 AND NR 2 SMOKE DETECTORS, NO DEFECTS NOTED. PERFORMED OPS CHECK OF SMOKE DETECTION SYSTEM GOOD IAW MM 26-12-00, PG 201. (K)

ADDITIONAL NOTES: TWO DIFFERENT P/N SMOKE DETECTORS ON BOARD THIS AIRCRAFT. NR 1 - P/N 30-231-3, S/N 35007 TIME SINCE REPAIR 174.8 HR. NR 2 - P/N 473054, S/N 0054W, TIME SINCE REPAIR 1326.8 HR. EXPERIENCED IN THE PAST THAT CELL PHONE USAGE TRIGGERS THE SMOKE WARNING AND BELIEVE THIS TO BE A DESIGN ISSUE. THEREFORE, DON'T KNOW WHICH DETECTOR (OR BOTH) MAY HAVE GIVEN THE FALSE WARNING, SO ONLY A FUNCTIONAL TEST ON THE DETECTORS WAS PERFORMED.

RE-INSTALLED IGNITER IAW DHC-8 MM 72-01-20 SEC 5B PG 402. ALSO FOUND INBOARD NACELLE AFT DOOR BURNED. REMOVED AND REPLACED DOOR IAW DHC-8 MM 71-10-00 PAR 16 PG 206. NOTE: TEMPORARY RD8-54-2383 INITIATED FOR CONTINUED AIRWORTHINESS UP TO 5000 HOURS WITH REPETITIVE INSPECTIONS EVERY 100 HOURS ON INBOARD ENGINE SUPPORT STRUT P/N 85410029-053, S/N 0118.
NOSE GEAR DOWN AND LOCK LIGHT DID NOT COME ON INTO LNY. FLOOR DOWN AND LOCKED INDICATOR SHOWED NOSE GEAR DOWN AND LOCKED. REMOVED AND REPLACED LANDING GEAR PANEL AND PERFORMED OPS CHECK IAW DHC-8 MM 32-30-05 PG 201-202.
GEAR UNSAFE LIGHT ON AFTER GEAR UP SELECTION. CLEARED VIA CHECKLIST, FOUND ALTERNATE RELEASE DOOR NOT FULLY CLOSED. VERIFIED WITH CAPTAIN THAT LANDING GEAR ALTERNATE RELEASE DOOR NOT FULLY CLOSED DURING FLIGHT. NO DEFECTS NOTED.

LANDING GEAR MALFUNCTION, FAILED TO DEPLOY, GEAR CYCLED 3 TIMES, PROCEEDED WITH ALTERNATE EXTENSION, SUCCESSFUL.
LOSS OF NR 1 HYD FLUID ON APPROACH. MX FOUND BAD O-RING ON SPU NR 1 OUTLET UNION. REPLACED O-RING IAW DHC -8 MM CH 29-00-00 PG 401. LEAK CHECK GOOD IAW DHC -8 MM CH 29-00-00 PG 501.
AFTER T/O APPROX 500FT AGL, NR 2 ENGINE TQ DROPPED TO 11 PERCENT, ALL OTHER INSTRUMENTS INDICATING NORMAL. ENGINE BEGAN TO SURGING AND UNABLE TO CONTROL WITH THRUST LEVERS. COMMENCED WITH ENGINE SHUTDOWN PROCEDURES. DECLARED EMERGENCY WITH APPROACH. LEVELED OFF AT 1000 FT MSL TO STAY VFR, LANDED WITHOUT INCIDENT.
SMOKE WARNING LIGHT ILLUMINATED IN FLIGHT. MAINTENANCE INSPECTED AFFECTED SYSTEM WITH NO FAULTS FOUND.
AFTER GEAR UP SELECTION, RED NOSE GEAR LIGHT ILLUMINATED AND GEAR HANDLE ILLUMINATED AND STARTED FLASHING ON AND OFF. NO OTHER CAUTION LIGHTS ILLUMINATED. (MECHANIC) INSPECTED AND CLEANED NOSE GEAR PROXIMITY SENSOR AND PERFORMED TEST PROCEDURE IAW MM 62-61-00 PG. 202 SECTION D. NO DEFECT NOTED.
SMOKE SMELL IN COCKPIT AND ROW 1. COULD NOT DETERMINE SOURCE. COMPLIED WITH A WALK AROUND INSPECTION AND HAVE DETERMINED THIS AIRCRAFT TO BE IN A SAFE CONDITION FOR A FERRY FLIGHT FROM JHM-HNL ONLY. SOURCE DETERMINED TO BE FROM ACM. ACM FOUND FROZEN. REMOVED AND REPLACED ACM IAW DHC MM 21-50-11 PG 206-210. LEAK AND OPS CHECK GOOD.

DURING CLIMB-OUT, 'AFTER T/O' CHECKLIST, WHEN AUTO-FEATHER DESELECTED ARM LIGHT REMAINED ILLUMINATED. COMPLETED A NORMAL APPOACH AND LANDING, TAXIED TO TERMINAL. MAINTENANCE T/S SYSTEM BY SWAPPING THE NR 1 AND NR 2 TSCU'S, DID NOT SOLVE PROBLEM REPLACED THE ARM CONTROL RELAY (6121-KS). REMOVED AND REPLACED BULB IN BEACON LAMP ASSY WITHIN THE SMOKE DETECTOR ASSY IAW PYROTECTOR INC COMPONENT MM 26-10-21 PAGE 7. REINSTALLED NR 1 SMOKE DETECTOR BY REINSTALLING 6 SCREWS AND CONNECTOR PLUG. OPS CHECK GOOD IAW DHC-8 MM 26-12-00. RESET CIRCUIT BREAKER AND REMOVED PLACARDS.

EMERGENCY FLOOR PATH LIGHTING ASSEMBLY AT ABOUT SEAT 6CD, INOPERATIVE. INSPECTED LIGHTING ASSEMBLY AND FOUND BROKEN WIRE AT THE ABOVE MENTIONED LOCATION. REMOVED AND REPLACED LIGHTING ASSEMBLY. OPERATIONAL CHECKS COMPLETED IAW MM. THERE WERE NO DEFECTS NOTED. SYSTEM RETURNED TO SERVICEABLE CONDITION.

NR 2 ENGINE TORQUE BEGAN SURGING/FLUCTUATING +-15 PERCENT AT TAKE OFF. CONTINUED FOR A COUPLE OF MINUTES WITHOUT OTHER GAUGE INDICATIONS. TRIED MANUAL FUEL CONTROL (DROPPED AS IT SHOULD TO 68 PERCENT TORQUE), RESELECTED ECU. FO COULD FEEL SURGES. AFTER STEADYING OUT FOR A COUPLE OF MINUTES, SURGES BECAME +- 20 PERCENT AND WE BROUGHT POWER TO IDLE, PROP MIN. - STABLE AFTER THAT. INSPECTED AND CHECKED NR 2 ENGINE POWER AND CONDITION LEVERS. FOUND NR 2 ENGINE OUT OF RIG. RIGGED NR 2 ENGINE POWER AND CONDITION LEVERS IAW DHC 8 ENGINE RIGGING MANUAL. GROUND RUN CHECK GOOD.
NOSEWHEEL ALT EXTENSION HANDLE/CABLE BROKE. REMOVED AND REPLACED NOSEWHEEL ALT EXTENSION HANDLE CABLE, RIGGED NLG ALTERNATE RELEASE CONTROL IAW MM. OPS CHECK OF ALT EXTENSION SYSTEM GOOD IAW MM.
NR 2 ADI PITCH/ROLL MISMATCH, AHRS DATA INVALID, YAW DAMPNER FAILED ON BOTH CHANNELS. FLAPS VERY SLOW MOVING AND STUCK AT 10 PERCENT. FLAPS WOULD NOT RETRACT ABOVE 5 DEGREE. DECLARED EMERGENCY AND RETURNED TO AIRPORT. LANDED WITHOUT INCIDENT. CORRECTIVE ACTION: 1) PERFORMED GMT TEST FOR BOTH NR 1 AND NR 2 AHS SYSTEM. TESTED GOOD - 2) PERFORMED GMT TEST FOR YAW DAMPNER SYS.(BOTH L & R CHANNELS). TESTED GOOD. 3) BLED NR 1 MAIN HYDRAULIC SYSTEM AND FPU. (B) FUNCTIONAL TEST OF WING FLAP SYSTEM GOOD. NO DEFECTS NOTED.
