Maintenance disclosures
DURING TAXI IN FROM FLT NR 4453 - THE CAPTAINS RIGHT RUDDER PEDDLE ARM BROKE OFF. MAINTENANCE WAS DISPATCHED TO THE AIRCRAFT, AND FOUND ARM HAD FAILED AT PEDAL PIVOT POINT. THE ARM WAS REPLACED AND THE AIRCRAFT WAS RELEASED TO SERVICE. (M)
DURING INSPECTION THE LEFT ENGINE NACELLE OUTBOARD UPPER KEEL WEB (P/N 114-980022-1) WAS FOUND TO HAVE TWO CRACKS ON THE FORWARD FLANGE. BOTH CRACKS RAN VERTICALLY ON THE FLANGE. ONE CRACK WAS BETWEEN THE TWO CENTER ATTACH RIVETS APPROXIMATELY 1 INCH IN LENGTH. THE 2ND CRACK WAS LOCATED NEXT TO THE THREE LOWER HI-LOCK ATTACH FASTENERS APPROXIMATELY 3 INCHES IN LENGTH. THE KEEL WAS REPAIRED IAW AERODESIGN ENGINEERING REPORT NR 1507, DATED 6/2/95 AND FAA FORM 8110-3. THE KEEL WEB IS TO BE REPLACED NO LATER THAN 1/4/96 WITH REINSPECTIONS EVERY 45 DAYS UNTIL REPLACEMENT. (M)
PVD - ON TAKEOFF ROLL, THE LT ENGINE EXPERIENCED A LOSS OF POWER AND OIL PRESSURE. THE TAKEOFF WAS ABORTED, ENGINE WAS SHUTDOWN AND ACFT RETURNED TO GATE. INITIAL ON SITE INVESTIGATION REVEALED LOSS OF POWER TURBINE BLADES, PUNCTURED EXHAUST DUCTS AND DEBRIS IN EXHAUST AREA. INSPECTION OF MAIN ENGINE OIL FILTER REVEALED HEAVY METAL PARTICLE CONTAMINATION. COMPLETE ENGINE CHANGE OUT WAS ACCOMPLISHED ON SITE, WITH FAILED ENGINE AND PROPELLER RETURNED TO PSM. ENGINE TO BE DIRECTED TO O/H FACILITY FOR FURTHER INVESTIGATION. S/D - THE POWERPLANT REFERENCE ABOVE EXPERIENCED A POWER SECTION FAILURE OF THE SUNGEAR. (Z)
