Part: SPAR — CRACKED (HORIZ STAB)
DURING ROUTINE 500 HOUR INSPECTION A CRACK WAS FOUND RADIATING FROM AN ATTACHMENT FITTING SCREW BORE TO THE EDGE OF THE SPAR. A NEW SPAR WAS MATCH DRILLED AND INSTALLED IAW EUROCOPTER REPAIR SCHEME.
Part: FREQ ADAPTER — CRACKED (MAIN ROTOR HUB)
DURING ROUTINE INSPECTION 1 OF 3 FREQUENCY ADAPTERS FOUND TO HAVE A CRACK IN THE ELASTOMER NEARING ITS MAXIMUM ALLOWABLE LIMITATION OF 10MM. A NEW KIT OF 3 MATCHED ADAPTERS WERE INSTALLED.
Part: STARFLEX — DEFECTIVE (MAIN ROTOR)
DURING ROUTINE INSPECTION WITH ROTOR HEAD SLEEVE ASSY REMOVED ONE OF THE STAR ARM END BUSHINGS WAS FOUND TO TURN SLIGHTLY WITH HAND PRESSURE APPLIED. THE STARFLEX WAS REMOVED FOR REPAIR OF BONDING DEFECT. THIS IS A COMMON FAILURE FOR THIS PART. THE MFG CLAIMS TO HAVE A REPAIR FOR THIS DEFECT BUT OFFERS ONLY AN EXCHANGE ASSY. SUGGEST A NEW ADHESIVE SYS OR PART DESIGN BE TRIED TO REDUCE THIS TYPE OF DEFECT FOR AN EXPENSIVE PART. A SLIPPAGE MARK PAINTED ACROSS THE EDGE OF THE BUSHING AND ONTO THE STAR ADHESIVE BEAD HELPS IN DETECTION OF THIS DEFECT WHILE IN SERVICE BY INDICATING THE BUSHING HAS TURNED. (K)

DURING ROUTINE INSPECTION EXCESSIVE OIL LEAKAGE WAS NOTED AT THE MODULE 5 OUTPUT MAGNETIC SEAL. CLOSE INSPECTION OF THE REMOVED SEAL REVEALED NO OBVIOUS DEFECT. A NEW SEAL WAS INSTALLED WITH NO FURTHER LEAKAGE NOTED. (K)

(REF 0421083949) THE TAIL ROTOR DRIVE FREE WHEEL ASSEMBLY OUTPUT SHAFT SEAL OF MODULE 5 BEGAN TO WEEP AND PROGRESSIVELY THE WEEP BECAME A LEAK. A NEW SEAL WAS INSTALLED AND NO FURTHER LEAKAGE WAS NOTED. (K)

PILOT REPORTED THAT WHEN ADJUSTING POWER FOR CRUISE FLIGHT THE ENGINE HAD FLUCTUATIONS WHEN PASSING THROUGH A CERTAIN COLLECTIVE POSITION THAT WERE REPEATABLE IF SELECTING THE POSITION IN QUESTION. UPON SHUTDOWN A FAULT CODE WAS GENERATED INDICATING THE COLLECTIVE POTENTIOMETER WAS FAULTY. A NEW POTENTIOMETER WAS INSTALLED AND NO FURTHER DEFECT NOTED. THIS IS A VERY COMMON FAULT FOR THIS PART THAT EFFECTS THIS MODEL AIRCRAFTS DISPATCH RELIABILITY. (K)

PILOT REPORTS AMBER GOVERNOR LIGHT CAME ON IN FLIGHT. NO FAULT CODE WAS GENERATED INDICATING A POTENTIOMETER DEFECT HOWEVER CIRCUMSTANCES REPORTED POINTED TO THIS. A NEW POTENTIOMETER HAS BEEN INSTALLED. PROBLEM FIXED THIS PART HAS LASTED MUCH LONGER THAN TYPICAL THIS IS A RECURRING PROBLEM FOR THIS PART THAT HAS EFFECTED THE DISPATCH RELIABILITY OF THIS MODE AC. THIS PART CAN ON OCCASION ALSO CAUSE ENGINE FLUCTUATIONS IN FLIGHT WHEN THE RESISTANCE VALUE SIGNAL IS DISRUPTED FOR A LITTLE AS 1/20TH OF A SECOND, WHICH IS THE SAMPLE RATE FOR THE ENGINE CONTROL SYS. AN ADDITIONAL FACTOR FOR THESE TYPE OF FAULTS IS NO RESET FUNCTION AVAILABLE TO THE PILOT TO DIFFERENTIATE BETWEEN A MOMENTARY OR AN ACTIVE FAULT. ONCE THE AMBER WARNING LIGHT IS ILLUMINATED IT REMAINS ON UNTIL THE FLIGHT IS TERMINATED EVEN IF SIGNAL DISRUPTION WAS ONLY FOR 1/20TH OF A SECOND DURING AN IN-FLIGHT TURBULENCE EVENT. (K)

NR (ROTOR RPM) MAGNETIC SENSING PICKUP LOOP BROKEN CAUSING INTERMITTENT NR INDICATION FLUCTUATIONS DURING FLIGHT. PROBABLE CAUSE IS THAT WIRE AND FRAGILE COIL WIRE IS HAIR THIN. SUGGEST A STRONGER WIRE HARNESS BE USED. (K)
