SLICK MAG MODEL NUMBER M3637 CONTACT POINTS BROKE. 10 HOURS SINCE NEW.
INVESTIGATION OF MAGNETO FAILURE FOUND THAT THE CONTACT ARM OF THE PRIMARY POINT SET HAD SNAPPED OFF. THE BREAK OCCURED AT THE AREA WHERE THE STIFFENER FLANGES FLATEN OUT. JUST AT THE EDGE OF THE NYLON BLOCK. UNABLE TO DETERMINE CAUSE OF FAILURE. (AL05200402443)
THIS RECEIVER WAS INSTALLED AS PART OF THE EFIS-SV SYSTEM. AFTER APPROXIMATELY 30 HOURS OF OPERATION FOLLOWING THE INSTALLATION, AN LOI MESSAGE DISPLAYED ON THE INTEGRATED DISPLAY UNIT (IDU). THE RECEIVER WAS REPLACED, AND THE ERROR MESSAGE WAS NO LONGER DISPLAYED, AND THE SYSTEM FUNCTIONED NORMALLY. GPS/WAAS RECEIVER, SN 4048218.
WHILE AT CRUISE RPM, PILOT HEARD LOUD BANG FOLLOWED BY A SEVERE VIBRATION. REDUCING RPM MADE VIBRATION WORSE. A HIGHER RPM SETTING WAS USED AND AIRCRAFT RETURNED TO LAND SAFELY. UPON EXAMINATION, FOUND APPROXIMATELY 8 INCHES OF ONE PROPELLER BLADE SEPARATED FROM BLADE. SUBMITTER STATED THE BREAK WAS CLEAN AND SHOWED NO SIGN OF A PRE-EXISTING CRACK.
SUBMITTER FOUND IMPULSE COUPLING RETAINER NUT LOOSE ON SHAFT WITH COTTER PIN STILL IN PLACE. THE IMPULSE ASSY COULD BE PULLED FROM TAPERED SHAFT WITH ONLY SLIGHT PRESSURE. A BLUEING COMPOUND WAS APPLIED TO THE TAPERED SHAFT AND THE IMPULSE ASSY WAS RESEATED. IT WAS FOUND THE SHAFT HAD A HIGH AREA THAT WOULD NOT ALLOW THE IMPULSE ASSY TO SEAT PROPERLY. A DIAL INDICATOR READING SHOWED APPROX .002 INCH - .003 INCH HIGH AREA IN A SEMI-CIRCLE SHAPE EXTENDING FROM THE KEY SLOT ABOUT .3750 INCH HIGH AROUND SHAFT TAPER. SECOND INSTANCE FOUND OF AN IMPULSE ASSY LOOSENING ON SHAFT WITH LESS THAN 100 HRS IN SERVICE. SUBMITTER RECOMMENDED TO SET ASSY WITH NUT TORQUE, REMOVE NUT AND TEST FIT BY PULLING WITH PULLER.
REMOVED MAGNETO FOR 400-HOUR IMPULSE INSPECTION. FOUND BOTH PAWL PINS HAD BECOME LOOSE ON IMPULSE BODY WITH ONE PIN NEARLY READY TO COME OUT OF ITS MOUNT HOLE. THIS WAS A NEW MAGNETO ASSY INSTALLED ON ENGINE AS A UNIT.
MAGNETO REMOVED AT 500 HOURS TIS DUE TO IMPULSE COUPLING DEFECT.
