ON TAKEOFF ROLL, ENGINES ABOVE 40 PERCENT NG, AUTOTHROTTLES JUST STARTING TO TAKE CONTROL, THERE WAS A BANG AND NR 2 ENGINE STARTED TO ROLL BACK. FLIGHT WAS UNDER 80 KTS AND THE CREW ABORTED THE TAKEOFF AND RETURNED TO THE GATE.
R & R FORWARD AREA POWER SUPPLY AND BATTERY PACK. OPS CHECK GOOD.
FS 540, BUTT LINE 0 FLOOR SUPPORT CRACKED. REMOVED FLOOR SUPPORTS AND INSTALLED NEW FLOOR SUPPORT.
DURING OTHER MAINTENANCE IN LT WHEEL WELL, FOUND VAPOR BARRIER CRACKED APPROX 14" LT STA 663.73, LBL 30. THIS WAS PREVIOUSLY REPAIRED WITH CRACK RUNNING THROUGH REPAIR AREA AND BEYOND. REPAIRED DAMAGE.

ON ARRIVAL AT KCHO, L1 DOOR SLIDE DEPLOYED BUT DID NOT INFLATE. REMOVED AND REPLACED L1 ESCAPE SLIDE.
ON TAKEOFF ROLL, RIGHT AFT OVERWING EXIT LIGHTS ILLUMINATED.
BIRD STRIKE ON ARRIVAL INTO SEA. INSPECTED AND FOUND DAMAGE TO THE LT FUSELAGE NOSE SKIN AND COCKPIT WINDOW FRAME JUST INBOARD OF THE C-D WINDOW POST AND BELOW THE NR 1 WINDOW. DAMAGE CONSISTS OF A DENT APPROX 4 X 4 1/2 X 7/16 DEEP WITH A FURTHER AREA 9 X 5.5 INCHES SHOWING EVIDENCE OF DEFORMATION. UPON REMOVAL OF THE DAMAGED SKIN, THE INBOARD FLANGE OF THE WINDOW FRAME SKIN ATTACHMENT FLANGE WAS FOUND BENT APPROX 1 5/8 INCH X 0.075 INCH DEEP. ACCOMPLISHED PERMANENT REPAIR PER BOEING INSTRUCTION & SCA EO 53-6599-1 TO INSTALL NEW SKIN PANEL AND DOUBLER.
ONE EMERGENCY LIGHT WAS FOUND INOPERATIVE ON THE RT SIDE BETWEEN OVER WING EXITS (EXTERIOR). R & R EXTERIOR EMERGENCY LIGHT BULB FROM ASSY ON RT SIDE BETWEEN HATCHES AND CHECKED NORMAL. CONDITION FOUND TO BE OK FOR SERVICE IAW AMM 33-51-04.
