CREW REPORTED LEFT WINDSHIELD HEAT WAS INOPERATIVE AND FOGGING UP. MAINTENANCE FOUND WINDSHIELD HEAT FUSE 100FU01-080 FAILED (OPEN), THIS WAS CAUSED BY THE OVERHEATING OF HEAT CONTROL CONTACTOR 480CC01. THE CONTACTOR TERMINAL âAâ HAD OVERHEATED CAUSING MELTING OF WIRE INSULATION AND DAMAGING THE TERMINAL (SEE PHOTO). THE CREW DID NOT REPORT ANY BURNING OR UNUSUAL ODORS IN THE COCKPIT. THE CONTACTOR AND FUSE WERE REPLACED AND SYSTEM OPERATIONAL CHECKED GOOD. AIRCRAFT RETURNED TO SERVICE.
WHILE ON SHORT FINAL TO KAUS, THE FLIGHT DECK CREW AND FORWARD FLIGHT ATTENDANT REPORTED AN ELECTRICAL BURNING ODOR. AIRCRAFT CONTINUED APPROACH AND LANDED WITHOUT INCIDENT. MAINTENANCE PERFORMED CHECKS IAW FIM 21-20-00 AND 25-31-00 TASK AND INSPECTED GALLEY AND UNDER FLOOR AREA. UNABLE TO LOCATE ANY SIGNS OR A SOURCE FOR ELECTRICAL OVERHEATING ODOR. AIRCRAFT RETURNED TO SERVICE.
AFTER TAKEOFF AND PASSING THROUGH 5000 FT, CREW EXPERIENCED A STABILIZER TRIM RUNAWAY, FOLLOWED BY A FAILURE OF THE MAIN AND STANDBY TRIMS AS INDICATED BY EICAS MESSAGES. CREW COMPLETED STABILIZER TRIM RUNAWAY, AND STABILIZER TRIM FAILURE QRC PROCEDURES. RETURNED TO AIRPORT AND DECLARED AN EMERGENCY, LANDED WITHOUT INCIDENT. MAINTENANCE FOUND CAPTAINS CONTROL WHEEL TRIM SWITCH HAD FAILED. SWITCH WAS REPLACED AND AIRCRAFT RETURNED TO SERVICE.
AFTER TAKEOFF, THE CREW RECEIVED AN ENG 2 FIRE DETECTOR FAILED MESSAGE ON EICAS. FOLLOWED PROCEDURES AND RETURNED TO DEPARTURE, LANDED WITHOUT FURTHER INCIDENT. NO EMERGENCY WAS DECLARED. MAINTENANCE REPAIRED A DAMAGE WIRE THE NR 2 ENGINE FIRE DETECTOR CONNECTOR IAW SWPM 20-21-00. SYSTEM OPS CHECKED GOOD. AIRCRAFT RETURNED TO SERVICE.
DURING APPROACH, WITH THRUST AT IDLE AND ANTI-ICE SELECTED ON. A STRONG ODOR OF ELECTRICAL SMOKE IN THE COCKPIT, BUT NOT IN CABIN. APPROACH WAS CONTINUED AND AIRCRAFT LANDED WITHOUT INCIDENT. NO EMERGENCY WAS DECLARED. MAINTENANCE REMOVED POSITIVE TERMINAL BOLT, CLEANED WIRE TERMINAL AND REINSTALLED BOLT ON LEFT WINDSHIELD TERMINAL BLOCK. OPS CHECKED WINDSHIELD HEAT SYSTEM IAW AMM 30-42-00/04. NO ODOR OF SMOKE NOTED. A CHECK FLIGHT WAS PERFORMED, WITH ON NO REOCCURRENCE OF THE DISCREPANCY.
STRONG ODOR OF ELECTRICAL SMOKE DURING APPROACH WITH MOMENTARY DU4 OVERHEAT ADVISORY MESSAGE. APPROACH WAS CONTINUED WHERE AIRCRAFT LANDED WITHOUT INCIDENT. NO EMERGENCY WAS DECLARED. MAINTENANCE PERFORMED GVI OF FLIGHT COMPARTMENT, FORWARD FUSELAGE EXTERNAL AND INTERNAL ELECTRICAL COMPARTMENT IAW AMM 05-20-06 AND 05-20-07 WITH NO DISCREPANCY NOTED. SUSPECTED MFD 2 DISPLAY UNIT COOLING FANS MAY HAVE INTERMITTENTLY FAILED. PERFORMED AN OPERATIONAL CHECK OF THE EFIS SYSTEM IAW AMM 34-22-00 WITH SYSTEM CHECKING GOOD.
AFTER TAKEOFF, CREW RECEIVED A FLAP LOW SPEED WARNING ON EICAS AND A COMPARATOR WARNING ON THE EFIS CAUSED BY A 30 KIAS DISCREPANCY BETWEEN LEFT AND RIGHT AIRSPEED INDICATIONS. CREW DECLARED AN EMERGENCY, RETURNED TO DEPARTURE AND LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE REPORTED FINDING THE PITOT SYSTEM DRAIN LINE HOLES PLUGGED WITH INSECT NEST. THE TUBE WAS CLEANED IN ACCORDANCE WITH AMM 34-13-00.
AFTER TAKEOFF, ON A RE-POSITION FIGHT WITH NO PAXS, THE CREW REPORTED AN OIL ODOR IN THE CABIN. THE CREW FOLLOWED PROCEDURES RETURNING & LANDED WITHOUT INCIDENT. AFTER SHUTDOWN A VISUAL INSPECTION OF THE RT ENG REVEALED TURBINE OIL IN THE INTAKE & EXHAUST AREAS. TROUBLESHOOTING IS CONTINUING. CORRECTIVE ACTION TO FOLLOW.
FLT 5535 - STL - LANDING GEAR AIR/GROUND FAILURE MESSAGE ON EICAS. MAINTENANCE INSPECTED AND FOUND THE NLG PROXIMITY SENSOR TARGET OUT OF ADJUSTMENT. TARGET AND SENSOR WHERE RE-ALIGNED AND AN OPERATIONAL CHECK WAS PERFORMED WITH NO DEFECTS NOTED.
FLT NR 5490 - IAD - AFTER TAKEOFF, GOING THRU 15,000 FT, ALL PRESSURE INDICATIONS WENT TO DASHES. MAINTENANCE REMOVED AND REPLACED DAU NR 2, OPS CHECK GOOD.
PRE FLT NR 5515 - STL - HANDRAIL EMERGENCY LIGHT IS INOP. MAINTENANCE RELAMPED LIGHT AS REQUIRED, OPS TEST GOOD.
POST FLT NR 5542 - STL - AISLE FLOOD LIGHT EMERGENCY LIGHT INOP. MAINTENANCE RE-BULBED EMERGENCY CEILING WASH LIGHT, OPS CHECK GOOD.
PRE FLT NR 5467 - STL - EMERGENCY LIGHT (FWD SIDE AFT CEILING EXIT LOCATOR) IS OUT. MAINTENANCE REPLACED LIGHT, OPS CHECK GOOD.
PRE FLT NR 5526 - STL - EMERGENCY LIGHTS FROM FRONT ROW OF AIRCRAFT TO ROW 8 INOP. MAINTENANCE CHARGED THE EMERGENCY LIGHTS SYSTEM BY RUNNING THE APU. LIGHTS OPS CHECKED GOOD.
PRE FLT NR 5596 - STL - EMERGENCY LIGHT IN CEILING AT 12BC INTERMITTENT. MAINTENANCE REMOVED AND REPLACE THE BULB.
PRE FLIGHT 5450 - STL - EMERGENCY LIGHT OUT AT ROW 12. MAINTENANCE REMOVED AND REPLACED THE EMERGENCY LIGHT INVERTER. OPS CHECKED GOOD.

PRE-FLIGHT 5575-STL - EMERGENCY LIGHT OVER ROW 12 INOP. MAINTENANCE RESEATED THE CONNECTOR AT THE ROW 12 OVERHEAD EMERGENCY EXIT SIGN.
FLT 5566 - RIC - PITCH TRIM MAIN AND BACKUP INOP ON APPROACH. MAINTENANCE RESET AND RESTORED POWER TO THE AIRCRAFT AND RESET PITCH TRIM CIRCUIT BREAKERS.
FLT 5520 - AUS - DURING APPROACH, LOST HYDRAULIC SYSTEM 2 QUANTITY AND THEN E2 ENGINE HYDRAULIC PUMP FAIL MESSAGE. MAINTENANCE REMOVED AND REPLACE THE NR 2 ENGINE DRIVEN HYDRAULIC PUMP.
