ON FLIGHT 2733 THE CREW REPORTED THEY HAD AN ENGINE 1 ANTI-ICE FAIL DURING CLIMB IN NON-ICING CONDS: TAT +11C 9000 MSL. ICE DET1 FAIL MESSAGE ALSO ON. THE CREW FOLLOWED AOM PROCEDURES AND DID NOT REMEDY THE SITUATION. THE CREW DECLARED AN EMERGENCY AND REMAINED CLEAR OF ICING CONDITIONS. THE AIRCRAFT RETURNED TO DFW AND LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED ENGINE 1 ANTI ICE VALVE IAW EMB-140 AMM 30-21-01. THE OPERATIONAL CHECK WAS GOOD IAW AMM 30-00-00-700-802-A. REMOVED AND REPLACED NR 1 ICE DETECTOR IAW EMB-140 AMM 30-80-01. PART NUMBER 0871HT31A, S/N 00728. THE OPERATIONAL CHECK WAS GOOD IAW AMM 30-80-00. NO FURTHER DEFECTS NOTED AT THIS TIME. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING MBV INSPECTION, QC FOUND FRAME 23 REPAIR DOUBLER CRACKED BETWEEN STRINGERS 15R AND 16R. REMOVED PREVIOUS REPAIR PARTS, FABRICATED AND INSTALLED NEW REPAIR PARTS IAW EO EM-53MR-1808.
DURING B INSPECTION FOUND THE PROXIMITY LIGHT IN GALLEY, ONE WITH ARROWS AND ONE WITH OUT ARROWS TO HAVE BROKEN WIRES. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE PROXIMITY LIGHT IN GALLEY, ONE WITH ARROWS AND ONE WITH OUT ARROWS IAW EMB 140 AMM 33-50-12. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING B INSPECTION FOUND THE LEFT PROXIMITY LIGHT HOLDER 2EA AND RIGHT FLOOR TRIM 2EA CRACKED. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE LEFT PROXIMITY LIGHT HOLDER 2EA AND RIGHT FLOOR TRIM 2EA IAW EMB 140 AMM 33-50-12. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING MAINTENANCE INSPECTION FOUND THE ELECTRICAL WIRE FOR THE NR 1 EMERGENCY LIGHT STRIP BROKEN AT CONNECTOR. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE CONNECTOR FOR THE NR 1 EMERGENCY LIGHT STRIP IAW EMB 140 SWPM 20-21-00. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING B INSPECTION FOUND THE LEFT OVERWING FAIRING EMERGENCY LIGHT INOPERABLE. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE NR 3 EMERGENCY BATTERY AND BATTERY CHARGER IAW EMB 140 AMM 33-50-04. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING B INSPECTION FOUND THE SERVICE DOOR CENTER SILL HAS CORROSION AROUND ALL 3 NUT PLATE HOLES (MARKED). THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE SERVICE DOOR CENTER SILL WITH SERVICEABLE CENTER SILL AND RESTORED PROTECTIVE COATING IAW EMB SRM 51-40-02, CPM 51-21-05, CPM 51-21-06, AEA ETN-GN-51-016R2, ETN-EM-51-075 AND ETN-EM-51-091R2. THE AIRCRAFT WAS RETURNED TO SERVICE.

ON FLIGHT 3039 THE CREW REPORTED THAT BOTH FORWARD & AFT EMERGENCY FLASHLIGHT "CHARGE" LIGHTS [RED] WERE NOT ILLUMINATED. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE FORWARD AND AFT EMERGENCY FLASHLIGHT ASSEMBLY IAW EMB 140 AMM CHAPT 25-60-00. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.

DURING MAINTENANCE BASE VISIT FOUND THE THIRD AFT EMERGENCY LIGHT FLOOR STRIP INOPERABLE. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE THIRD AFT EMERGENCY LIGHT FLOOR STRIP IAW EMB 140 AMM 33-50-12. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING PREFLIGHT INSPECTION FOR FLIGHT 4060 THE CREW FOUND THAT THE LEFT AND RIGHT OVERWING EXTERIOR LIGHTS FAILED TO ILLUMINATE. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE NR 4 BATTERY AND POWER SUPPLY IAW EMB AMM 33-50-04. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 3918 THE CREW REPORTED THAT DURING THE CLIMBOUT THE AIRCRAFT PRESSURIZATION FAILED, CLIMBED TO 10,000FT, CABIN ALTITUDE AND SWITCHED TO MANUAL MODE, SYSTEM STABILIZED AND PRESSURIZATION STABILIZED, SHORTLY AFTER, CABIN ALTITUDE DISPLAY WENT BLANK (AMBER DASHES) THEN CABIN PRESSURIZED TO 7.9 PSID AT 8,000FT MSL. THE CAPTAIN DECLARED AN EMERGENCY AND RETURED BACK TO ORD. THE CREW TURNED OFF THE PACKS TO DEPRESSURIZE AND LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE ELECTROPNEUMATIC OUTFLOW VALVE IAW AMM 21-31-03. REMOVED AND REPLACED THE MANUAL CONTROLLER IAW AMM 21-31-02. PERFORMED OUTFLOW VALVE FUNCTION IAW AMM 21-31-00. PERFORMED PRESSURE CONTROL SYSTEM FUNCTION TEST IN AUTOMATIC AND MANUAL IAW AMM 21-31-00, AMM 21-31-00. THE OPERATIONAL TEST WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 3974 THE CREW REPORTED THE UPPER BRACKET ON THE FLASHLIGHT CRADLE BEHIND THE CAPTAINâS SEAT TO BE BROKEN. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE FLASHLIGHT CRADLE IAW 33-50-16. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 4108 CREW REPORTED A FLUTTER/VIBRATION IN THE FLIGHT CONTROLS DURING ALL PHASES OF FLIGHT. STARTED AT 150KTS, VIBRATION FELT WITH GEAR IN UP OR DOWN POSITION. VIBRATION FELT WITH AUTO PILOT ON AND OFF, PACKS ON, CREW NOTED THE VIBRATION THROUGH FLIGHT CONTROLS AS A CONSTANT SHIMMY. CREW DECLARED AN EMERGENCY AND LANDED WITHOUT INCIDENT. ACFT REMOVED FROM SERVICE. AWAITING FOLLOW UP.
AFTER DEPARTURE, NR 1 PACK VALVE FAIL FOLLOWED SHORTLY BY NR 1 BLEED VALVE CLOSED FOLLOWED CHECKLIST AND AFTER RESET ONCE. FUMES APPEARED IN COCKPIT. RETURNED TO BOS NO OTHER ABNORMALITIES NOTED. NO FUMES IN CABIN. FUMES DISSIPATED AS SOON AS BLEED VALVE CLOSED. FOUND COUPLING BETWEEN DUAL TEMP CONTROL AND NR 1 ACM TUBE BLOWN REMOVED AND REPLACED COUPLING IAW EMB AMM 21-60-01. RUN PACK OPS CHECK GOOD IAW AMM 21-51-00. AIRCRAFT RETURNED TO SERVICE.
ALSO, F/A REPORTED AN OIL SMELL IN THE CABIN. THIS WAS DURING FLIGHT. NO EMERGENCY DECLARED AND A/C LANDED WITHOUT INCIDENT. RAI NR 2 ENGINE, OPS CHECK GOOD, FCF PASSED. AIRCRAFT RETURNED TO SERVICE. (M)
ABI - DURING AN MBV INSPECTION, THE LEFT OMEGA BEAM AT FRAME 25 WAS FOUND CORRODED. PERFORMED A CUTOUT OF THE BEAM TO REMOVED CORROSION. FABRICATED A REPAIR DOUBLER, FILLER AND TWO SHIMS. REPAIR DONE IAW EMB 145 SRM, SRM EMB-145/1142 REV 28, CHAPTER 53-21-10 FIG 201. (M)
