DISCREPANCY - LEFT HAND SIDE FUSELAGE JUST AFT OF ENTRY DOOR AND FORWARD OF REPAIR HAS "OIL CANNING" AND DISTORTION THAT IS APPROXIMATELY 2 INCH'S SQUARE. CORRECTIVE ACTION - REPAIRED LEFT HAND SIDE FUSELAGE JUST AFT OF ENTRY DOOR IN ACCORDANCE WITH EMB 145 SRM 51-75-00. THE DAMAGE REPAIR WAS CLASSIFIED AS A MAJOR REPAIR AND IS LOCATED ON THE CENTER FUSELAGE I, LEFT HAND FUSELAGE SKIN BETWEEN FRAMES FR19L & FR20L AND STRINGERS STR11L & STR12L.
PAC #1 VALVE CLOSED IN FLIGHT. FOUND DISCONNECTED COUPLING AT DUEL TEMP CONTROLLER. RECONNECTED COUPLING AND PERFORMED OPS CHECK. LEAK CHECK OF PACK 1 OPS CHECK GOOD AT THIS TIME.

THE CREW REPORTED RECEIVING A NR 2 ENGINE BLEED LEAK EICAS MESSAGE IN FLIGHT. MAINTENANCE FOUND THREE CLAMPS BLOWN OFF AND RE-SECURED THE CLAMPS. FOLLOW ON OPERATIONAL CHECKS OF THE BLEED SYSTEM WERE GOOD.
DURING CRUISE FLIGHT THE FLIGHT CREW GOT A LANDING GEAR AIR GROUND FAIL MESSAGE. AIRCRAFT MADE AN EMERGENCY LANDING AT STL. MAINTENANCE INSPECTED AND REMOVED AND REPLACED THE RIGHT MLG NR 2 WOW SWITCH. SYSTEM OPS CHECKED GOOD.
DURING CRUISE, THE FLIGHT CREW RECEIVED A LANDING GEAR AIR GROUND FAILURE MESSAGE ON THE EICAS. MAINTENANCE REMOVED AND REPLACED THE RIGHT MLG NR 2 WOW SWITCH. OPS CHECKED GOOD.
DURING MAINTENANCE INSPECTION, FOUND PASSENGER ENTRY CEILING EMERGENCY LIGHT INOP. MAINTENANCE REPLACED THE BULB IN THE PASSENGER ENTRY CEILING LIGHT, OPS CHECK GOOD.
CREW COULD NOT OPEN MAIN PASSENGER DOOR DURING POST FLIGHT. MAINTENANCE INSPECTED AND FOUND A MISSING SCREW FROM A DOOR PANEL AND INSTALLED SCREW FOR DOOR PANEL AND OPS CHECK GOOD, DEFERRED PANEL DUE TO CRACKING FROM DOOR OPENING.
DURING CLIMB THE AIRCRAFT WOULD NOT PRESSURIZE. AIRCRAFT RETURNED TO STL. MAINTENANCE INSPECTED AND REPLACED THE PNEUMATIC OUTFLOW VALVE. SYSTEM OPS CHECKED GOOD.

THE CREW REPORTED RECEIVING A BLEED LEAK MESSAGE ON THE EICAS DURING CLIMB. MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND THE LEAK TO BE AT THE NR 1 ENGINE PRE-COOLER. THE CLAMP WAS REPOSITIONED AND THE SYSTEM WAS OPS CHECKED WITH NO ADDITIONAL ISSUES.
THE CREW REPORTED RECEIVING A LANDING GEAR DISAGREE MESSAGE ON THE EICAS AFTER TAKEOFF AND THAT THE EICAS INDICATED THAT THE LANDING GEAR WAS DOWN AND RED WITH THE HANDLE IN THE UP POSITION. MAINTENANCE INSPECTED AND REPAIRED A CORRODED PIN AT THE LANDING GEAR SOLENOID VALVE CONNECTOR. FOLLOW-ON SYSTEM CHECK PROVED A SATISFACTORY FIX AT THAT TIME.
WHILE IN CRUISE, THE CREW REPORTED THAT THE LAVATORY SMOKE DETECTOR WENT OFF AND THAT A STRONG SMELL WAS COMING FROM PACK NR 2. INITIALLY, THE NR 2 PACK WAS DEFERRED AND THE SMOKE DETECTOR WAS OPERATIONALLY CHECK GOOD. THE DEFERRAL WAS CORRECTED WHEN MAINTENANCE REMOVED AND REPLACED THE NR 2 ENGINE FOR LEAKING OIL INTERNALLY WHICH WAS CAUSING THE SMELL AND POSSIBLY THE SMOKE.
DURING THE LANDING, THE CREW REPORTED A LAV SMOKE WARNING AND THE F/A REPORTED THAT THE NR 2 PACK WAS BLOWING WARM AIR AND HAD ODOR. MAINTENANCE INSPECTED THE AIRCRAFT AND PERFORMED AN OPERATIONAL CHECK OF THE LAV SMOKE WITH NO DEFECTS NOTED. MAINTENANCE DEFERRED THE NR 2 PACK AS THE SOURCE OF THE ODOR.
WHILE IN CRUISE, THE CREW REPORTED RECEIVING A BLEED 1 LEAK MESSAGE ON THE EICAS. MAINTENANCE INSPECTED THE AIRCRAFT AND PRIOR TO RELEASING IT BACK INTO SERVICE. REMOVED AND REPLACED THE BLEED O-RING SEALS AT THE AFT CARGO BAY. AN OPERATIONAL CHECK OF THE BLEED SYSTEM WAS PERFORMED WITH NO DEFECTS NOTED. THE AIRCRAFT WAS RETURNED TO SERVICE.
AFTER FLIGHT, THE CREW REPORTED THAT THE MAIN DOOR WAS DIFFICULT TO OPEN. MAINTENANCE INSPECTED THE MAIN DOOR PRIOR TO THE NEXT FLIGHT AND LUBED THE DOOR OPERATING MECHANISM. OPERATIONAL CHECK OF THE DOOR DID NOT REVEAL ADDITIONAL DEFECTS.
CREW REPORTED THAT THE AIRCRAFT EXHIBITED SOME VIBRATIONS IN THE FLOORBOARD WHILE IN FLIGHT. MAINTENANCE INSPECTED THE AIRCRAFT, REMOVED AND REPLACED THE LT AND RT HORIZONTAL STABILIZER FAIRING TIPS. ALSO REMOVED AND REPLACED THE LT AND RT ELEVATOR TIPS. THE RIGHT LANDING GEAR WHEEL FAIRING WAS ALSO FOUND TO HAVE A CRACK. IT WAS REMOVED AND PLACED ON CDL. NO OTHER DEFECTS NOTED AT THE TIME.
THE CREW REPORTED THE MAIN DOOR AS BEING DIFFICULT TO OPEN FROM THE INSIDE. MAINTENANCE LUBED THE DOOR ROLLERS. OPS CHECKED GOOD.
THE FLIGHT ATTENDANT REPORTED THAT THE MAIN DOOR WAS DIFFICULT TO OPEN. MAINTENANCE ADJUSTED THE MAIN DOOR ROLLERS AND OPS CHECKED THE DOOR GOOD.
DURING GROUND HANDLING, THE CREW REPORTED THAT THE MAIN DOOR LATCH WAS DIFFICULT TO OPEN AND CLOSE. MAINTENANCE CLEANED AND LUBRICATED THE MAIN DOOR TRACKS AND HANDLE. OPERATIONAL CHECK OF THE DOOR WAS GOOD.
CREW REPORTED RECEIVING A BLEED 1 LEAK MESSAGE ON THE EICAS DURING FLIGHT. MAINTENANCE INSPECTED AND FOUND THE CLAMP AT THE DUAL TEMP CONTROL VALVE LOOSE. THE CLAMP WAS TIGHTENED AND AN OPERATIONAL CHECK OF THE SYSTEM WAS PERFORMED WITH NO LEAKS FOUND.
THE CREW REPORTED THAT THE LAVATORY SMOKE DETECTOR WAS PRODUCING INTERMITTENT FAILURES WHILE IN FLIGHT. MAINTENANCE CLEANED THE SMOKE DETECTOR AND CONNECTOR AND PERFORMED A OPERATIONAL CHECK OF THE SMOKE DETECTOR. OPS CHECKED GOOD.
CREW REPORTED, RECEIVED A ELECTRICAL EMERGENCY ABNORMAL INDICATION ON THE EICAS DURING CRUISE. MAINTENANCE REMOVED AND REPLACED THE NUMBER 1 BATTERY SWITCH IAW AMM PROCEDURES. OPS CHECKED GOOD.
