DURING A SCHEDULED MAINTENANCE EVENT, MAINTENANCE FOUND THE STRUCTURAL WEB UNDER THE COPILOTS SEAT RAIL SHELF CRACKED. THE CRACK WAS APPROXIMATELY 1.5 INCHES LONG LOCATED FORWARD OF FRAME 7 ON THE SEAT RAIL WEB. THE CRACK WAS REPAIRED BY REMOVING THE DAMAGED AREA AND INSTALLING A DOUBLER IAW MFG STANDARD REPAIR DRAWING NR SR-HK-0034 REV 1.

THE CREW REPORTED THAT DURING CLIMBOUT AT FL 210 THE CABIN ALTITUDE COULD NOT BE CONTROLLED. THE ALTITUDE HORN SOUNDED AND THE CREW DECLARED AN EMERGENCY. DURING DESCENT, CABIN ALTITUDE COULD NOT BE CONTROLLED USING THE CABIN RATE CONTROL KNOB OR MANUAL RATE CONTROL KNOB. THE AIRCRAFT CABIN ALTITUDE NEVER EXCEEDED 9,500 FT. WHEN RETURNING TO KLAS THE AIRCRAFT WAS LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE REPLACED THE OUTFLOW / SAFETY VALVE CONTROLLER. OPERATIONAL CHECKS WERE GOOD, AND THE AIRCRAFT WAS RETURNED TO SERVICE.

THE CREW REPORTED, EXPERIENCED A MAIN HYDRAULIC SYSTEM FAILURE DURING LANDING GEAR AND FLAP EXTENSION WHILE ON A VISUAL APPROACH INTO KCKV. A GO-AROUND WAS EXECUTED AND A SUBSEQUENT DIVERSION WAS MADE TO KBNA. THE LANDING GEAR WAS EXTENDED AND FLAPS WERE AT 15 DEGREES WHEN THE FAILURE OCCURRED. THE AUXILIARY HYDRAULIC SYSTEM WAS USED TO COMPLETE FLAP EXTENSION. AN EMERGENCY WAS DECLARED BY THE CREW AND THE AIRCRAFT WAS LANDED IN KBNA WITHOUT FURTHER INCIDENT.
