DURING CRUISE PORTION OF FLIGHT, #2 ENGINE FIRE WARNING LIGHT ILLUMINATED. NO FURTHER INDICATIONS OF FIRE NOTED, CREW ELECTED TO ABORT MISSION AND RTB. MAINTENANCE CREW CLEANED CANNON PLUGS , OPS CHECK OK.NO FURTHER PROBLEMS NOTED.
AFTER TAKEOFF, CLIMBING TO 1750 FEET AGL, BAGGAGE COMPARTMENT SMOKE DETECTOR WARNING LIGHT ILLUMINATED. NO INDICATION OF SMOKE WAS PRESENT, ACFT TURNED AROUND & LANDED WITHOUT FURTHER INCIDENT. TROUBLE SHOT SYS & REPLACED SMOKE DETECTOR. OPERATIONS CHECKED SYS, NO FURTHER PROBLEMS NOTED.
UPON LANDING FROM A HOVER, COCKPIT FILLED WITH SMOKE & AFT BAG FIRE WARNING LIGHT ILLUMINATED. ACFT LANDED & SHUTDOWN, INSPECTED BAGGAGE COMPARTMENT & FOUND THAT IT WAS FILLED WITH SMOKE.
AFTER LANDING, AFT BAG FIRE LIGHT ILLUMINATED. LIGHT EXTINGUISHED WHEN BLEED AIR & BLOWER SWITCHES WERE TURNED OFF. FOUND OIL ON HEATER DISTRIBUTION BOX & HEATER DUCT UNSEALED. CLEANED HEATER BOX & RESEALED DUCT.
DURING ACFT RUN UP AFT BAGGAGE COMPARTMENT SMOKE LIGHT ILLUMINATED. CLEANED SMOKE DETECTOR CONNECTOR OPERATIONS CHECKED SYS, SYS CHECKED GOOD.
DURING 500 HR ROUTINE INSPECTION, FOUND BLADE SPAR SPLINTERED ON "B" SIDE AT APPROXIMATE STATION 10.5
MAIN GEARBOX HOUSING HAS CORROSION.
SHORTLY AFTER TAKEOFF, NOTICED THE NR 2 ENGINE OIL TEMP HAD CLIMBED ABOVE THE RED LINE ALL THE WAY TO 118 C. IMMEDIATELY REDUCED THE ENGINE TO IDLE, AS IT APPROACHED IDLE THE ENGINE OIL PRESSURE WENT TO 0. SECURED THE ENGINE AND RETURNED TO DEPARTURE.
UPON MAJOR INSPECTION OF AFT SPAR AREA, FOUND CRACK IN AFT EDGE OF SPAR THROUGH HIGH LOC HOLE, .500 IN. LONG AT WL 111, STA. 103.4.
DURING FLIGHT THE NR 1 ENGINE ILLUMINATED TEN MINUTES AFTER DEPARTURE. THE NR 1 ENGINE WAS PLACED IN GROUND IDLE IAW EMERGENCY CHECK LIST AND THE FLIGHT CONTINUED. AT ARRIVAL, THE NR 1 ENGINE WAS PLACED IN FLY POSITION IAW CHECK LIST FOR NORMAL LANDING. THERE WERE NO OTHER IRREGULAR CONDITIONS NOTED DURING THE FLIGHT. MAINTENANCE DISCOVERED 1 SMALL DARK PARTICLE ON THE NR 1 ENGINE CHIP DETECTOR. PERFORMED SOA SAMPLE AND CHECKED RUNDOWN TIME OF THE ENGINE. CHIP PLUGS MUST BE PULLED AFTER LAST FLIGHT AND INSPECTED FOR A PERIOD OF 25 HOURS. (K)
ON DAILY SERVICE CHECK FOUND 4 INCH CRACK ON LT SIDE FOR VERTICAL PYLON, UPPER FORWARD CORNER OF THE INSPECTION PANEL AT VS STATION 123.071. MAINTENANCE REPAIRED THE CRACK IAW WITH SRM. (K)
ON A DAILY SERVICE CHECK, FOUND 4 INCH CRACK ON LT SIDE FOR VERTICAL PYLON, UPPER FORWARD CORNER OF THE INSPECTION PANEL AT VS STATION 123.071. MAINTENANCE REPAIRED THE CRACK IAW S76 SRM.
WHILE HOVERING TO REPOSITION FOR TAKEOFF MAIN GEAR BOX CHIP LIGHT ILLUMINATED. RETURNED TO PARKING, NOTIFIED CUSTOMER, BASE MANAGER AND MAINTENANCE. MAINTENANCE REMOVED BOTH CHIP PLUGS TO FIND 2 SMALL HAIR LIKE SLIVERS IN CHIP PLUG SCREEN, FOUND TO BE WITHIN LIMITS IAW S76C+ MM. REPLACED TRANSMISSION OIL AND FILTER, GROUND RAN LEAK CHECK GOOD.
DURING TAKEOFF, INTERMEDIATE/TAIL ROTOR GEAR BOX LIGHT CAME ON DURING TAKEOFF, CIRCUIT BREAKER WAS PULLED, LIGHT WENT OUT, RESET CIRCUIT BREAKER, LIGHT CAME BACK ON. AIRCRAFT WAS LANDED BACK ON RIGHT AT ST164. MAINTENANCE REMOVED INTERMEDIATE AND TAIL ROTOR GEAR BOX CHIP DETECTORS, CLEANED AND REINSTALLED, GROUND RAN OPS CHECK WAS GOOD, NO FURTHER DEFECTS NOTED. TRANSMISSION DISASSEMBLED FOR METAL CONTAMINATION, FOUND ROLLER BEARING BREAKING DOWN. REMOVED AND REPLACED BEARING.
AT A HOVER READY FOR TAKEOFF, AFTER INITIAL RUN UP AND PREPARING FOR TAKEOFF MAIN GEAR BOX LIGHT CAME ON. MAINTENANCE DISCOVERED METAL CHIP CONTAMINATION ON LEFT AND RIGHT MAIN GEAR BOX CHIP DETECTORS. TRANSMISSION REMOVED FROM SERVICE. (M)
