(EMERGENCY DECLARED) ON APPROACH INTO DAY. AT 14000 FT & 280 KTS, PILOTS RECEIVED A SLAT OVERSPEED WARNING. THE PILOTS RAN A CHECK LIST AND SUDDENLY THE FIRE WARNING CAME UP FOR THE NR 2 ENGINE. THE CAPTAIN DEPLOYED 1ST FIRE BOTTLE AND AFTER 30 SECONDS THE 2ND BUT FIRE DID NOT EXTINGUISH UNTIL RIGHT BEFORE TOUCHDOWN. R&R NR 2 ENGINE.
NO PAXS IN FLIGHT, FERRY FLIGHT. PACKS SHUT DOWN INFLIGHT, LOST PRESSURIZATION, INITIATED RAPID DECENT. HOWEVER AT 14,000 FT CABIN ALT THE PX O2 MASKS DEPLOYED. LT PACK SHOWS 14 PSI AT CRUISE POWER, EVEN WHEN PACK IS OFF. REF WO 290982 ITEMS 1 & 3 FOR TROUBLESHOOTING OF THE ABOVE DISCREPANCY. ISSUES WERE FOUND WITH THE RT PACK INLET TEMP SENSOR WIRE, BROKEN PLUG FOR THIS SENSOR WAS REPINNED. ISSUES WERE ALSO FOUND WITH RT PACK COMPRESSOR DISCHARGE CONNECTOR BEING LOOSE. REPLACED CONNECTOR IAW SWPM 20-31-00 & PACK COMP DISCHARGE TEMP SENSOR IAW MM 21-31-05. OPERATIONS CHECKED GOOD. A SINGLE PACK CONFIDENCE CHECK WAS PERFORMED REF WO 290982, CONFIDENCE PASSED. ACFT IS OK FOR RTS.
JUST LEVELED OFF AT FL360, A FEW MINS LATER, THE PACKS BASICALLY QUIT WORKING. HAD A CABIN ALT MASTER WARNING. THE CABIN WAS AT 10,000 & CLIMBING. STARTED ON THE QRH PROCEDURE FOR, CABIN ALT & RAPID PRESSURIZATION, ON PAGE 21.2. THEY WERE NOT ABLE TO CONTROL THE CABIN PRESS & IT CONTINUED TO GO UP IN ALTITUDE. WENT ON TO THE QRH PROCEDURE ON PAGE 21.3 FOR, RAPID DESCENT. DURING THE RAPID DESCENT THE O2 MASKS DEPLOYED IN THE CABIN. AFTER LEVELING AT 10,000 FEET & REMOVING THEIR O2 MASKS THE CREW NOTICED A BURNING & SMOKY SMELL. THE NEEDLE ON THE LT AIR CONDITIONING PRESS GAGE WAS JUMPING UP & DOWN ERRATICALLY. THE BURNING & SMOKY SMELL WENT AWAY AFTER THEY TURNED IT OFF. THE ACFT WAS NOT OVERWEIGHT FO

THEY ASKED ATC FOR LOWER & WERE GIVEN FL 320. THEY STARTED ON THE QRH PROCEDURE FOR, CABIN ALT & RAPID PRESSURIZATION, ON PAGE 21.2. THEY WERE NOT ABLE TO CONTROL THE CABIN PRESS & IT CONTINUED TO GO UP IN ALTITUDE. THE CREW ASKED FOR & WERE GIVEN A DESCENT TO 10,000 FEET & THEY WENT ON TO THE QRH PROCEDURE ON PAGE 21.3 FOR, RAPID DESCENT. DURING THE RAPID DESCENT THE O2 MASKS DEPLOYED IN THE CABIN. AFTER LEVELING AT 10,000 FEET & REMOVING THEIR O2 MASKS THE CREW NOTICED A BURNING & SMOKY SMELL. THEY NOTICED THAT THE NEEDLE ON THE LT AIR CONDITIONING PRESS GAUGE WAS JUMPING UP & DOWN ERRATICALLY. THE BURNING & SMOKY SMELL WENT AWAY AFTER THEY TURNED OFF THE LT AIR CONDITIONING PACK. AFTER LEVELING OFF AT 10,000 FEET THEY CREW WAS ADVISED THAT THE WEATHER WAS GOOD & IT WAS JUST OFF TO THE NW OF THEIR FLIGHT TRACK. THE ACFT WAS NOT OVERWEIGHT FOR THE LANDING. THE RIGHT TEMP CONTROLLER WAS IN AUTO, BUT THE TEMP GAUGE INDICATED FULL HOT. THE CREW WAS NEVER ABLE TO RE-PRESSURIZE THE ACFT.

THE AIRCRAFT HAS BEEN RETURNED TO SERVICE.
DURING A ROUTINE OVERNIGHT SERVICE CHECK INSPECTION MAINTENANCE DEPARTMENT PERSONNEL REPORTED A PORTION OF THE EMERGENCY EVACUATION LIGHTING SYSTEM WOULD NOT TEST. MAINTENANCE PERSONNEL REMOVED AND REPLACED THE APPROPRIATE EMERGENCY FLASHLIGHT ASSY AND MADE ALL REQUIRED CORRECTIONS AND OPERATIONAL CHECK WAS ACCOMPLISHED PER AMM 33-50-00, SYSTEM OPERATIONAL CHECK WAS NORMAL. THE AIRCRAFT HAS BEEN RETURNED TO SERVICE.

DURING BOW 1C/2C/S1/S2/S4, THERE WAS CORROSION OUT OF LIMITS ON THE LT HORIZONTAL TO VERTICAL LUG PLATES, ALSO CORROSION ON THE RT LUG PLATES WERE CLEANED UP IN LIMITS. ADDRESSED AS REQUIRED.

FLIGHT CREW REPORTED SMOKE IN CABIN. AIRCRAFT WAS DIVERTED TO SEA-TAC AND AN EMERGENCY WAS DECLARED. MAINTENANCE DEPARTMENT PERSONNEL INSPECTED THE AIRCRAFT AND REPORTED THE FOLLOWING; NO FIRE WAS NOTED BY FIRE CREW, FLIGHT CREW OR MAINTENANCE PERSONNEL. SMOKE WAS REPORTED TO SMELL OF BURNING WOOD WITH FOREST FIRES IN THE AREA. INSPECTED AIRCRAFT, ENGINES, APU AND AIR CONDITIONING AND PRESSURIZATION SYSTEM. NO FAULTS OR DEFECTS NOTED IAW AMM 76-00-00, 21-00-00. ALL SYSTEM CHECK NORMAL AND AIRCRAFT WAS RETURNED TO SERVICE.

FLIGHT 475 FSD/LAS - DURING THE DE-ICING OPERATION THE EQUIPMENT OPERATOR STRUCK THE AIRPLANE ON THE LEFT ELEVATOR AT THE AFT OUTBOARD CORNER. AMERICAN AIRLINES PERSONNEL REPORT THAT THEY REMOVED AND REPLACED THE LEFT ELEVATOR TRAILING EDGE. REF SRM 55-01-00. PERFORMED MEGGER CHECK FOR REMOVE/REPLACE OF STATIC WICK PER AMM 23-60-00-5. ELEVATOR BALANCE CHECKED AND WEIGHT REMOVED AT NOSE CAP STATION 41.5. VALANCE TAG UPDATED PER SRM 55-20-01, PARAGRAPH 18.A.(9)(C) AND AMERICAN AIRLINES ERO 5520-01. THE AIRPLANE HAS RETURNED TO SERVICE.
DURING A ROUTINE OVERNIGHT SERVICE CHECK LAS VEGAS, MAINTENANCE REPORTED THAT THE EMERGENCY EXIT OVERHEAD SIGNS WOULD NOT TEST. REPLACED THE BATTERY PACK IN B5-27 POWER SUPPLY. A SATISFACTORY OPERATIONAL CHECK WAS ACCOMPLISHED PER AMM 33-51-12. THE AIRPLANE HAS RETURNED TO SERVICE.
