THE PILOT REPORTED SMOKE IN THE CABIN DURING CRUISE WHILE THE HEATER WAS ON. MAINTENANCE INSPECTED, FOUND THE HEATER BLOWER INOPERABLE, AND REPLACED IT WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED BURNING ODOR IN CABIN. UPON INSPECTION MAINTENANCE, FOUND THE CABIN FAN BURNT OUT. REMOVED AND REPLACED THE CABIN FAN WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED ENGINE SHUDDERED IN FLIGHT AND THEN STOPPED SHORTLY AFTER. THE AIRCRAFT LANDED WITHOUT INCIDENT. INITIAL INSPECTION REVEALED AN ENGINE CASE PENETRATION AT THE TOP REAR OF THE CASE AT THE NR 1 CYLINDER IN THE AREA UNDER LEFT MAGNETO, AND THE MAGNETO FLANGE BROKEN. THE ENGINE IS IN PROCESS OF BEING REPLACED, FURTHER DETAILS TO BE SUPPLIED UPON TEARDOWN INSPECTION.
DURING NDT TESTING DONE TO SATISFY AD 2015-07-03, A CRACK WAS DISCOVERED ON THE LEFT NACELLE INBOARD ENGINE BEAM P/N: 5654109-6. THIS CRACK IS TRAVELING LEFT TO RIGHT ACROSS THE TOP OF THE BEAM ORIGINATING FROM A FASTENER HOLE ON THE TOP OF THE BEAM TRAVELLING TOWARD THE UPPER OUTBOARD RADIUS. THIS BEAM ASSEMBLY WILL BE REMOVED AND REPLACED WITH A NEW ASSEMBLY. PICTURES AND MEASUREMENTS HAVE BEEN ATTACHED TO THIS SDR.
PILOT REPORTED RIGHT ENGINE VIBRATING ON FINAL APPROACH AND TAXI. MAINTENANCE INSPECTED AND FOUND NO COMPRESSION ON THE RIGHT ENGINE NR 4 CYLINDER. UPON REMOVAL DISCOVERED THE EXHAUST VALVE HAD CRACKED AND BROKEN. THE CYLINDER WAS REPLACED WITH NO FURTHER DEFECT NOTED.
DURING A SID 57-10-16 NDT INSPECTION, FOUR CRACKS WERE FOUND IN THE LT OUTBOARD FITTING. TWO OF THESE CRACKS WERE FOUND ON THE INBOARD SIDE OF THE FITTING, JUST FORWARD OF THE SPAR STRAP. THEY MEASURED .143 AND .129 IN LENGTH. THE OTHER TWO CRACKS WERE FOUND ON THE OUTBOARD SIDE OF THE FITTING, JUST FORWARD OF THE SPAR STRAP. THESE CRACKS MEASURED .081 AND .114 IN LENGTH.
CYLINDER REMOVED DUE TO LOW COMPRESSION AT INSPECTION. EXHAUST VALVE CRACKED AND DISPLAYS EVIDENCE OF HEAT DAMAGE.
PILOT REPORTED LEFT BRAKE INOPERATIVE AND STEERING NOT AVAILABLE. MAINTENANCE INSPECTED AND FOUND THE BRAKE LINE FROM THE PARKING BRAKE VALVE ELBOW, TO MASTER CYLINDER FLEX HOSE LEAKING. REMOVED AND REPLACED THE LINE.
PILOT REPORTED ELEC SMELL IN THE COCKPIT 15 MINUTES INTO FLIGHT. INSPECTED & FOUND THE AUTOPILOT COMPUTER TO BE AT FAULT. R & R AUTOPILOT COMPUTER.
PILOT REPORTED A STRONG FUEL SMELL IN CABIN, NOTING THAT THE HEATER WAS OFF DURING FLIGHT. PERFORMED AN OPERATIONAL CHECK OF THE HEATER, INSPECTED THE FUEL LINES & COMPONENTS, NO FUEL LEAKS OR SMELLS NOTED AT THIS TIME. APPLIED TDMI OF RT WING FUEL PANEL SEEP, UPON FIX OF PANEL SEEP, INSPECTED & FOUND LT ENG FUEL LINE LOOSE. SECURED THE FUEL LINE & LEAK CHECKED WITH NO FURTHER DISCREPANCIES NOTED.
DURING SCHEDULED INSPECTION EVENT, THE RT NACELLE OTBD ENG BEAM WAS FOUND TO BE CRACKED AT AREA EXTENDING FWD & LWR OF OTBD PORTION OF AFT MOUNT BEAM OVERLAYS. A SUPPLEMENT TO THIS SDR WILL BE SUBMITTED TO ILLUSTRATE DAMAGE POINT. CORRECTIVE ACTION WILL BE COMPLETE DISASSEMBLY OF OTBD ENG BEAM ASSEMBLY FOR INSPECTION & REPLACEMENT OF ALL DAMAGED PARTS.
THE FRAYED AILERON CONTROL CABLE WAS FOUND DURING A FINAL INSPECTION AFTER THE ACFT WAS STRIPPED & PAINTED. THE CABLE WAS LOCATED, VISUALLY, IN THE RT FLAP WELL INBD OF THE AILERON BELL-CRANK BETWEEN TWO RIBS, WS 133.29 & 151.04. THE FRAYED CABLE WAS IN PLAIN SIGHT & HAD SIGNS OF RUBBING. BEFORE REPLACING DAMAGED CABLE, FOUND LT & RT AILERON CONTROL CABLES IN WINGS WITH TOO LOW OF CONTROL CABLE TENSIONS MEASURED WITH A CALIBRATED CABLE TENSION METER. R & R DAMAGED CABLE.
STRONG SMELL OF FUEL IN COCKPIT DURING FLIGHT. R & R HEATER FUEL PUMP IAW SERVICE MANUAL, LEAK CHECKED GOOD.

PILOT WHO WAS ACTING AS INSTRUCTOR FOR THIS TRAINING FLIGHT REPORTED THAT WHILE LEVELING OFF, THE FO NOTED THAT THE ELEVATOR TRIM SEEMED TO BE WORKING BACKWARDS. WHEN THE FO TRIMMED FOR NOSE DOWN, THE NOSE PITCHED UP. THE CAPTAIN TOOK THE CONTROLS AND VERIFIED THAT THE TRIM WAS OPERATING BACKWARDS. THEY RETURNED TO DEPARTURE TO RETURN THE ACFT TO MX. WHEN MX INSPECTED THE ACFT, THEY RE-RIGGED THE ELEVATOR TRIM AND CHECKED IT IAW C402C MM CHAPTER 27.
AT SCHEDULED INSPECTION, NLG DRAG BRACE OEM PN 5142002-5 WAS FOUND TO BE CRACKED IN THE AREA OF NLG ACTUATOR ATTACH POINT. METHOD OF INSPECTION WAS DYE PENETRANT. THIS CRACK WAS FOUND TO BE WITHIN ALLOWABLE LIMIT. THIS UNIT HAS BEEN REPAIRED IAW PROCEDURE OUTLINED IN MEB941-11R1 & RETURNED TO SERVICE.
AIRCRAFT WAS IN CRUISE AND THE RIGHT ENGINE SHUT DOWN WITHOUT WARNING. PILOT PERFORMED EMERGENCY CHECK LIST, DECLARED AND EMERGENCY AND DIVERTED TO MVY AIRPORT. AIRCRAFT LANDED WITH NO FURTHER INCIDENT, TAXIED TO GATE ON 1 ENGINE, AND DEPLANED 7 PASSENGERS. MAINTENANCE INSPECTED AND FOUND THE RIGHT ENGINE DRIVEN PUMP TO BE SEIZED. MAINTENANCE REMOVED AND REPLACED PUMP, AND OPERATIONALLY CHECKED THE PUMP SATISFACTORY.
THE RT AXLE OF N88833 CRACKED DURING LANDING. THE LOWER STRUT LEG WAS REPLACED WITH SERVICEABLE ASSY AND A/C RETURNED TO SERVICE.
BATTERY SHOWED HIGH RATE OF CHARGE. BOTH ALTERNATORS, AMPERE OUTPUT, TOTAL AVIONICS FAILURE, UTILIZED AVIONICS BYPASS, REGAINED PARTIAL AVIONICS. AIRCRAFT LANDED WITHOUT INCIDENT. MECHANIC REMOVED AND REPLACED RIGHT STARTER RELAY (UNIT HAD FAILED) AND RIGHT STARTER MOTOR AS A PRECAUTIONARY MEASURE. AIRCRAFT WAS RETURNED TO SERVICE.
