ELONGATED BOLT HOLES FOUND IN SPLICE-UPPER LH FWD #: 2611118-7 ON MAIN LANDING GEAR BULKHEAD STRUCTURE, FOUND DURING DOCUMENT 16, TASK 53-10-00-256. REQUIRES REPLACEMENT OF SPLICE.
ON START UP OF THE AIRCRAFT, AN OIL LEAK FROM THE FORWARD ENGINE COMPARTMENT WAS NOTICED. THE PILOT IMMEDIATELY SHUT DOWN THE ENGINE. MAINTENANCE REPLACED THE OIL RETURN TUBE ELBOW AND APPLICABLE O-RINGS IAW PT6-114A MM 79-20-01. GROUND RUN UP FOR OPS OF LEAK CHECK GOOD. AIRCRAFT RETURNED TO SERVICE.
FLIGHT 8786 AIR TURN BACK, WO 208-156061 & DISCREPANCY. INLET TURBINE TEMPERATURE GAUGE FLUCTUATED IN FLIGHT. CREW ELECTED TO RETURN, NO EMER DECLARED & ACFT LANDED WITHOUT INCIDENT. CRIMPED TERMINAL LUGS AT T5 BLOCK. OPERATIONS CHECKED & ENG PERFORMANCE RUNS GOOD. ACFT RETURNED TO SERVICE.
DURING ROUTINE FUEL PUMP GARLOC SEAL REPLACEMENT FOR LEAKAGE, THE MECHANIC INSPECTED THE FUEL PUMP DRIVE GEAR AND IT BECAME DECOUPLED FROM THE FUEL PUMP. THIS IS NOT NORMAL FOR THIS MODEL PUMP. THE ACCESSORY DRIVE GEAR IS HOLLOW AND HAD THE FUEL PUMP DRIVE MIGRATED FORWARD AND DECOUPLED FROM THE PUMP, THE FCU WOULD HAVE LOST REFERENCE AND CAUSED AN INFLIGHT SHUTDOWN. INSPECTION OF THE DRIVE GEAR APPEARS THE RETAINER IS DEFORMED AND INEFFECTIVE.
JUST AFTER TAKEOFF, ON FLIGHT NR 7797 THE PILOT RECEIVED A FIRE WARNING. HE DECLARED AND EMERGECY AND RETURNED. AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE DISCOVERED THE FIRE WARNING TEST BUTTON HAD FALLEN OFF CAUSING A FALSE WARNING. MAINTENANCE REINSTALLED THE FIRE WARNING TEST BUTTON. FOUND FIRE TEST BUTTON LOOSE. MAINTENANCE THEN REMOVED AND REPLACED THE FIRE TEST BUTTON. AIRCRAFT RETURNED TO SERVICE. (K)
