ON DEPARTURE FROM (KGWB), HOSPITAL THE PILOT SIDE SEARCH LIGHT BEGAN PANNING TO THE RIGHT IN A 360-DEGREE ROTATION THAT COULD NOT BE STOPPED O, CONTROL BY THE PILOT. IMMEDIATELY TURNED THE SEARCH TIGHT OFF AND ATTEMPTED TO SLOW THE SEARCH LIGHT TO RESET AND TRY AGAIN. I WAS UNABLE TO LOCATE THE PILOT SIDE SEARCH LIGHT OR VERIFY THAT II HAD TURNED ON. ASSUMING THE BULB MAY HAVE BURNED OUT OR THE LIGHT WAS STUD< IN A RANDOM DIRECTION. I SWITCHED TO THE CO-PILOT SEARCH LIGHT AND BEGAN TO MOVE IT INTO VIEW WHEN IT ALSO BEGAN AN UNCONTROLLED RIGHT 360 ?? DEGREE TUM. IT WAS AT THIS POINT BOTH SEARCH LIGHTS WERE DETERMINED TO BE UNRELIABLE AND UNSAFE TO CONTINUE THE NIGHT. CALLED DISPATCH AND ADVISED WE WERE ABORTING THE FLIGHT LO RETURN TO KGWB FOR MAINTENACE. ON CALL MECHANIC AND TIER 1 WHERE NOTIFIED OF THE SERVICE INTERRUPTION AND A WRITE UP WAS ENTERED INTO THE (CAN) MAINTENANCE LOGBOOK. TROUBLESHOT SEARCHLIGHT SYSTEM FOR UN-COMMANDED ROTATION OF BOTH SEARCH LIGHTS IN FLIGHT. UPON FURTHER TROUBLESHOOTING, FOUND RELAYS ON SEARCHLIGHT SELECTOR SWITCH TO BE INOP. REPLACED BOTH RELAYS ON SELECTOR SWITCH AND REPLACED BOTH 4-WAY POSITION SWITCHES. OPS CHECK GOOD.
DURING INSPECTION OF ACFT AFTER REMOVAL OF TAILROTOR AFT DRIVE SHAFT FOUND FLEX COUPLING BROKEN STRAPS AND GROOVE MARKS IN TAILROTOR GEARBOX HSG. NO DAMAGE ALLOWED IAW AEC INSTRUCTIONS. REMOVED FOR REPAIR.
MECHANIC WAS PERFORMING A VISUAL INSP OF THE MAIN ROTOR BLADES WHEN HE DETECTED A CRACK AT STA 2900 ON THE TOP OF THE BLADE. THE CRACK IS 45MM LONG WITH THE OTBD EDGE OF THE CRACK APPROX 735MM FROM THE INBD EDGE OF THE TRIM TAB. THE OTBD END OF THE CRACK IS ALSO 60MM FROM THE T/E OF THE BLADE CHORDWISE. THE INBD EDGE OF THE CRACK IS 90MM FROM THE T/E OF THE BLADE CHORDWISE. THIS VISUAL INSP WAS DONE BECAUSE OF ANOTHER WITHIN OUR FLEET HAD EXPERIENCED A SIMILIAR CRACK IN ONE OF ITS BLADES EARLIER IN THE WEEK. THAT PARTICULAR BLADE CRACK WAS DOCUMENTED ON SDR QMLA2011012411014.
DURING START UP, AS HYDRAULIC PRESSURE INCREASED THE TAIL ROTOR SERVO WENT HARD OVER TO THE LEFT.

MANUFACTURER TEARDOWN OF GEARBOX, FOUND PITCH CHANGE SHAFT DUPLEX BEARING FAILURE AND SHAFT SHEAR T/R BLADES IN-NEG-PITCH WITH NO CONTROL OF TAIL ROTOR.NOTE: NO T/R GEARBOXES INSTALLED ON ANY 365 IN OUR FLEET HAVE ELECTRONIC CHIP DETECTOR.
