ON CLIMBOUT AFTER FRA, RECEIVED PRI STAB TRIM FAIL CAS MESSAGE. ACCOMPLISHED ASSOCIATED QRH, AND ALSO RECEIVED SEC STAB TRIM FAIL CAS MESSAGE WHEN SECONDARY TRIM WAS TRIED. RAPID DECOMPRESSION OCCURRED AT 35,000 FEET CABIN ROSE TO 12,000 +\~ FEET MASKS WERE DONNED CHECKLIST RUN EMERGENCY DESCENT EXECUTED WITH PASSENGER MASKS DEPLOYED EMERGENCY DESCENT EXECUTED PAX MASKS DEPLOYED AND RETURNED SAFELY TO LAS VEGAS.
GEAR POSITION & WARNING: LANDING GEAR WOULD NOT RETRACT AFTER TAKEOFF. THE GEAR HANDLE DID NOT GO INTO THE UPLOCK DETENT. WE DID NOT GET THE GEAR DISAGREE CAS. AFTER PUSHING ON THE HANDLE A SECOND TIME THE GEAR RETRACTED. LANDING GEAR EXTENSION WAS NORMAL. REPEAT WRITEUP.// SIGNOFF // GAINED ACCESS TO THE CMC VIEWER AND CHECKED FOR ANY RECORDED FAULT RELATED TO THE LANDING GEAR MALFUNCTION, NO FAULT INFORMATION WERE RECORDED ON THE SYSTEM. FOUNDED THE LANDING GEAR CONTROL UNIT LEVER KNOB TO BE LOOSE DUE TO ELONGATED HOLES, NEW PARTS ON ORDER IAW CESSNA 680A MM 45-40-00, 32-00-00, AND WDM 32-60-01. PER NETJETS MAINTENANCE CONTROL REQUEST RECORDED VIDEO OF LANDING GEAR SWINGS AND GEAR HANDLE ACTUATION, EMAILED VIDEO TO MAINTENANCE CONTROL AND PER THEIR RECOMMENDATION RE-SECURED AND TIGHTEN THE GEAR HANDLE KNOB AND PERFORMED 10 GEAR SWINGS WITH SATISFACTORY RESULTS, DOWN JACKED AIRCRAFT AND SERVICED THE HYDRAULIC ACCUMULATOR RESERVOIR IAW CESSNA 680A MM 32-00-00,12-10-30 AND 32-30-30.

CHECK DOOR AND CABIN DOOR AMBER ANNUNCIATOR ILLUMINATED AFTER MULTIPLE DOOR CLOSINGS. N2 CHECK DOOR QRH HAD TO BE USED. MAIN CABIN INTERIOR AND EXTERIOR DOOR HANDLE IS STIFF AND HARD TO ACTUATE. COMPLETED MAIN CABIN DOOR WARNING SYSTEM RESET PROCEDURE IAW AMM 52-70-00. COMPLETED MULTIPLE OPS CHECKS WITH NO FURTHER DISCREPANCIES NOTED.
WING TANK O'FULL R AMBER CAS ILLUMINATED - WING O' FILL AMBER CAS MSG ILLUMINATED EN ROUTE. ACCOMPLISHED CHECK LIST AND VERIFIED WE WERE LOSING FUEL. TRIED VARIOUS ATTITUDE AND PITCH CHANGES AS WELL AS A 360 DEG TURN BUT NO CHANGE. THERE WAS VERY LITTLE FUEL IN THE CENTER TANK WHEN IT STARTED AND EVEN AFTER THE CENTER WAS EMPTY IT WAS STILL BLOWING OUT. ////SIGN-OFF////REMOVED AND REPLACED THE RH WING OUTBOARD FLOAT VALVE. FUELED CENTER TANK, PERFORMED FUEL TRANSFER TO RH WING SEVERAL TIMES, OPS CHECK GOOD. LEAK CHECK OF THE FLOAT VALVE ACCESS FUEL PANEL GOOD. WORKED IAW 750 MM 28-10-04
DURING GEAR EXTENSION ON FINAL APPROACH, THE NLG GREEN LIGHT DID NOT ILLUMINATE AND RED UNLOCK LIGHT REMAINED ILLUMINATED WITH GEAR HANDLE SELECTED DOWN. BOTH MAIN GEAR LIGHTS ILLUMINATED GREEN. AFTER ROTARY TEST CONFIRMED ALL LIGHTS WORKING, BLEW DOWN THE GEAR IAW QRC. THE GEAR UNSAFE INDICATION CONTINUED AND PULLED THE NLG UPLOCK IAW QRC. THE UNSAFE INDICATION PERSISTED (RED UNLOCK LIGHT, GREEN MLG LIGHTS, NO NLG LIGHT). AFTER 5 MINUTES OF MANEUVERING, THE GEAR INDICATING LIGHTS INDICATED NORMAL OPERATION (THREE GREEN, NO RED). LANDED AND CONFIRMED NORMAL NOSE WHEEL STEERING AND NORMAL BRAKE OPERATION. TAXIING CLEAR OF THE RUNWAY THE AIRCRAFT HANDLED AND INDICATED NORMAL OPERATION. RESET PNEUMATIC SYSTEM AND COMPLETED HYDRAULIC SYSTEM BLEED IAW MM 12-10-15. REMOVED AND REPLACED NOSE LANDING GEAR ACTUATOR IAW MM 32-30-03. COMPLETED SYSTEM BLEED, LEAK AND OPERATIONAL CHECKS WERE GOOD.

BLEND AND REMOVE CORROSION AND EVALUATE 1 EACH PIN HOLE ON THE R/H SIDE OF THE FUSELAGE, JUST AFT OF THE NOSE ACCESS DOOR. PERFORMED REPAIR TO RT NOSE SKIN AFT OF AVIONICS DOOR.
BLEND AND EVALUATE CORROSION ON UPPER OUTBOARD SURFACE OF LT WING SKIN AT AFT SPAR LOCATION.BLENDED AND REMOVED ALL VISIBLE CORROSION FROM UPPER OUTBOARD SURFACE OF LT WING SKIN AT AFT SPAR LOCATION UNTIL VISUALLY ELIMINATED.PERFORMED REPAIR TO LT UPPER WING SKIN CORROSION.
DURING DESCENT THROUGH 20,000', -A- HYDRAULIC QUANTITY DROPPED BELOW 16%. DROPPED TO 13% BEFORE UNLOADED. PNEUMATIC GEAR BLOWDOWN AND EMERGENCY BRAKE WERE USED. AFTER SHUTDOWN, HYDRAULIC FLUID COULD BE SEEN FROM THE CENTER OF THE FUSELAGE NEAR THE MAIN GEAR UNDER THE FUSELAGE TO THE TAILCONE. TROUBLESHOT AND FOUND THE HYDRAULIC PRESSURE TRANSDUCER TO BE LEAKING. REPLACED THE O-RING WITHIN TRANSDUCER. OPS AND LEAK CHECKED GOOD IAW AMM 29-20-01, & 29-30-01. SERVICED THE -A- HYDRAULIC RESERVOIR, PERFORMED A HYDRAULIC BLEED. LEAK AND OPS CHECKED GOOD.
DURING TAKEOFF THROUGH 3000 FEET WITH THROTTLE IN TAKE OFF, THE RT ENGINE N2 WENT TO 102 & INDICATED RED. THE FLYING PILOT FELT A POWER BUMP & THEN SLOWLY RETARDED THE RT ENG TO IDLE. THE ITT ON THE RT ENG SLOWLY SURGED BETWEEN AROUND 500 & 700 DEGREES A COUPLE TIMES. SHORTLY AFTER THE THROTTLE WAS AT IDLE WE RECEIVED THE AMBER CAS MESSAGE FAN DAMAGE RT. ACCOMPLISHED INSPECTION IAW MM 75-33-10 AND ACCOMPLISHED BORESCOPE OF THE COMPRESSOR SECTION IAW MM 72-30-00. NO DEFECTS AT TIME OF INSPECTION. REMOVED AND INSTALLED FPMU IAW MM 73-21-10. LEAK AND OP CHECKED GOOD.
DURING THE REMOVAL OF RELAYS K379V AND K378V MOUNTING PANEL, NOTICED CORROSION UNDER THE AFT CLOSET FLOOR, LOWER FUSELAGE BELLY SKIN. REPAIRED IAW S-750-0210/19RO AND THE SRM 51-40-03 AND 51-20-06. OPS CHECKED NORMAL.
AFTER TAKEOFF, AMBER FUEL BOOST ON LIGHT ILLUMINATED. SELECTED LT FUEL BOOST SWITCH TO ON, CAS MESSAGE TURNED WHITE. PLACED SWITCH BACK TO NORM, AMBER CAS STAYED OUT REMAINDER OF FLIGHT. RAN AIRCRAFT AT IDLE & MANIPULATED WIRES AND CONNECTOR PLUGS, NO DEFECT NOTED. RAN AIRCRAFT AT IDLE, 73% N2 AND TAKE OFF WHILE MONITORING MOTIVE FLOW PRESSURES IAW MM 28-20-00, NO DEFECTS NOTED. DURING RUNS MONITORED PRESSURES AT BOOST PUMP PRESSURE SWITCH & FOUND PRESSURE AT IDLE 15 PSI, AT 73% 19PSI, AT TAKE OFF 20 PSI. CUSTOMER STATED SAME ISSUE ON RIGHT. ELECTED TO REPOSITION BOOST PUMP PRESSURE SWITCHES. REPLACED SWITCH. LEAK AND OPS CHECK GOOD. WORK DONE IAW MM 28-20-05.
DISTINCTIVE EXHAUST ODOR NOTICED ABOUT 45MIN INTO CRUISE FLIGHT AT FL400. SMELL BECAME MORE NOTICEABLE AND STRONGER JUST AFTER ADJUSTING COCKPIT PAC TO WARMER TEMPERATURE. ALL ATTEMPTS TO ISOLATE PACS AND BLEEDS UNSUCCESSFUL. EACH PACK AND BLEED WERE TURNED TO OFF BUT NO SIGNIFICANT CHANGE WAS NOTICED. OPERATED APU ON GROUND AND VERIFIED SLIGHT OIL ODOR ON START, BUT FADES AFTER RUNNING A WHILE. INSPECTED APU INLET AND FOUND OIL PRESENT. R & R APU IAW MM 49-10-00. OPS AND LEAK CHECKS GOOD.
WHILE ACFT WAS CLIMBING WE EXPERIENCED A HYD VOLUME LOW CAS MESSAGE. THE VOLUME DROPPED TO AS LOW AS 8 PERCENT. IAW CHECKLIST WE UNLOADED THE "A" HYD SYS AND PULLED THE PTU CIRCUIT BREAKER AND THE SITUATION STABILIZED. PRESSURE DROPPED TO AS LOW AS 2500 PSI. AFTER LANDING, QTY INDICATED APPROX 30 PERCENT. POST FLIGHT INSP REVEALED EVIDENCE OF HYD LEAK BETWEEN MAIN GEAR WELLS. R & R "A" SYS ACCUMULATOR FITTING "O-RING IAW MM 29-10-04. SYS OPS AND LEAK CHECKED GOOD.
GEAR WOULD NOT RETRACT AFTER TAKEOFF. GEAR HANDLE WAS LOCKED IN DOWN POSITION. 3 GREEN GEAR DOWN LIGHTS WERE ILLUMINATED, NO RED UNSAFE LIGHTS ILLUMINATED. FOUND NOSE GEAR DID NOT FULLY EXTEND. PUSHED NOSE GEAR INTO POSITION. FOUND UPPER AND LOWER CHAMBERS 25 PSI LOW. SERVICED STRUT IAW MM 12-10-38, REV 41. OPS CHECKED GOOD.
JUST PRIOR TO BEING CLEARED FOR TAKEOFF, EXPERIENCED A VISIBLE WHITE SMOKE IN THE COCKPIT. THE ODOR APPEARED TO BE SULFUR TYPE ODOR. AFTER ABOUT 10 SECONDS IT DISSIPATED AND COULD NOT BE ISOLATED AGAIN WITH PAC OR ENGINE BLEED SYSTEMS. VERIFIED ODOR. REMOVED COCKPIT ACM. FOUND HEAVY OIL AND SOOT IN ACM AND DUCTING. CLEANED ALL DUCTING. REPLACED COCKPIT ACM. RE-INSTALLED ALL LINES AND PLUMBING. R & R COCKPIT COALESCER SOCK. ALL SYS OPERATIONAL, LEAK AND SNIFF CHECKED SATISFACTORY. NO ODOR OR SMOKE IN ACFT AFTER 30 MINUTES OF RUNNING SYSTEMS. COMPLETED IAW MM 21-51-00.

ON TAKEOFF FROM F70 THE GEAR WOULD NOT RETRACT. BURNED OFF FUEL & LANDED BELOW LANDING WEIGHT MINIMUMS. JACKED ACFT & INSPECTED THE NLG & MLG SQUAT SWITCHES. NO DEFECTS NOTED. CHECKED NLG STRUT NITROGEN SERVICE FOUND NLG STRUT UPPER BARREL TO BE BELOW REQUIRED SERVICE. SERVICED UPPER BARREL TO 420 PSI & DEPLEATED TO 380 PSI. ALSO FOUND LOWER NLG BARREL TO BE BELOW REQUIRED SERVICE. SERVICED NITROGEN ON NLG STRUT. CHECK BOTH MLG STRUT & FOUND TO BE WITHIN LIMITS. SWUNG GEAR SEVERAL TIMES WITH NO DEFECTS. IAW MCC LEAK CHECKED THE NLG STRUT & CHECKED NITROGEN SERVICE NO DEFECTS NOTED. SWUNG GEAR SEVERAL MORE TIMES. NO DEFECTS NOTED. WORK DONE IAW MM 27-20-00.
ON TAKE OFF, GEAR WOULD NOT RETRACT. REMOVED THE LANDING GEAR CONTROL HANDLE ASSY AND INSTALLED NEW GEAR CONTROL HANDLE. OPS CHECK GOOD. WORK PERFORMED IAW CHAP 32-30-01 OF THE MM.

SELECTING FLAPS 5 DEGREES, GOT AN AMBER FLAPS FAIL CAS ( SLATE EXTENDED & FLAPS SHOWED 1 DEGREE-2 DEGREES DOWN) , SELECTED THE SLATS POSITION AND PRESSED FLAP RESET, THE FLAPS CAME BACK UP THE 1 DEGREE OR 2 DEGREES BUT FAILED AGAIN. NR 2 INDICATOR FOUND ON PROP CONTROLLER. PERFORMED BITE SYS TROUBLESHOOTING IAW NR 2 BITE, PWR DINECENT FAULT, AND DELETED TROUBLESHOOTING FOR BITE NR 2. RESET BITE ON FLAP CONTROL BOX. MEASUREMENTS FOR THE FLAP RIGGING WERE FOUND TO BE WITHIN LIMITS OF THIS TIMES. PDU AMPERAGE WAS ALSO FOUND TO BE WITHIN LIMITS. INSTALLED FLAP BRAKEOUT BOX AND READ COMMAND PARTS WHILE FLAPS WERE IN OPERATIONAL. OPS CHECKS NORMAL. NO FAULS NOTED. FLAP OPERATION NORMAL AT THIS TIME IAW MM 27-50-00

INSTALLED 2 NEW TIRES AND OVERHAULED WHEEL ASSY. SERVICED AND SAFTIED NOSE WHEEL TIRES. INSPECTED RT MAIN BRAKES. NO LEAKS NOTED NO HEAT OR WEAR NOTED. DATA REFERENCE: MM 32-41-01.

ALSO REMOVED AND REPLACED NOSE GEAR TORQUE LINK BUSHINGS P/N 6742051-5 AND 6742150-3 IAW CE750 CMM CH 32-20-02, OPS CHECKED GOOD.

RECYCLED DURING FLIGHT FROM TEB TO DAL NOTICED BURNING ODOR COMING FROM COCKPIT GASPERS WHEN DUCT TEMP (COCKPIT) REACHES APPROX 120*. ESPECIALLY NOTICABLE WHEN POWER IS REDUCED ON INITIAL DESCENT. SUSPECT COCKPIT PAC MIGHT BE BAD, CONTINUED TO DAL. INSPECTED APU AND FOUND APU OVER SERVICED. SERVICED APU TO CORRECTED LEVEL, INSPECTED ACU AND DUCTS, RAN ENGINES AND APU NO DEFECTS OR BURNING ODOR NOTED AT THIS TIME REF CESSNA 750 M/M 21-51-00.

LT GEAR UNSAFE ON EXTENSION, FOLLOWING PNEUMATIC AND MANUAL EXTENSION STILL UNSAFE. INTERMITTENT UNLOCK LIGHT. AFTER LANDING DURING TURN OFF OF RUNWAY, STEADY UNLOCK LIGHT AND GEAR HORN CAME ON SHUTDOWN IN PLACE. REMOVED & REPLACED L/H MAIN LANDING GEAR ACTUATOR DOWN LOCK MICRO SWITCH IAW CESSNA 750 MM 32-30-04. OPS CHECKS GOOD IAW CESSNA 750 MM TASK # 32-00-01-720
AT APPROX 80 KTS ON TAKEOFF ROLL THE MACH TRIM CAS MESSAGE ILLUMINATED, FOLLOWED BY THE LOWER RUDDER FAIL CAS MESSAGE. FOLLOWED CHECKLIST FOR LOWER RUDDER FAIL, CAS MESSAGE WERE NORMAL. FOUND BUGS IN PITOT LINES FROM ADC. CLEANED LINES. C/W PITOT SYTEM LEAK CHECK IAW FAR 91-411 11-17-04. CHECKS NORMAL.
ON TAKEOFF ROLL AT 80 KNOTS FELT STRONG VIBRATION AT THE NOSE GEAR AREA, ABORTED TAKEOFF. REMOVED AND REPLACED BOTH NOSE WHEEL ASSEMBLIES DUE TO OUT OF BALANCE. OPS CHECK GOOD, WORK DONE IAW 750 MAINTENANCE MANUAL CHAPTER 32-41-00.
YAW DAMPER & MACH TRIM DISENGAGED DURING TAKEOFF ROLL, ABORTED TAKEOFF. FOUND WATER SITTING IN RT PITOT TUBE. REMOVED RT PITOT TUBE AND CLEANED AND REINSTALLED PITOT TUBE, PERFORMED OPERATIONAL CHECK OF AIR DATA SYSTEMS AND YAW DAMPENER SYSTEMS IAW HONEYWELL PRIMUS 2000 OPERATING MANUAL. PITOT/STATIC LEAK CHECK GOOD ON LT AND RT PITOT/STATIC SYSTEMS IAW FAR 91.411.
