OXYGEN SYSTEM TRANSMITTER / REGULATOR SLOWLY LEAKING.
DURING CRUISE OF FLIGHT 8106 BQN-BGI, AIRCRAFT 909FX INCURRED A HIGH ITT ON THE RIGHT ENGINE WHICH RESULTED IN A DECLARED EMERGENCY AND DIVERSION INTO STX. AIRCRAFT 909FX LANDED UNEVENTFULLY. REMOVED AND REPLACED THE #2 ENGINE FUEL NOZZLE KIT.
WHILE CONFIGURING THE AIRCRAFT TO LAND, THE CREW SELECTED THE LANDING GEAR FOR LANDING. BOTH THE FORWARD AND OVERHEAD PANELâS LANDING GEAR INDICATORS REMAINED RED, INDICATING AN UNSAFE GEAR SITUATION. THE CREW DISCONTINUED THE APPROACH TO LAND, DECLARED AN EMERGENCY, AND RAN THE QRH. THE FLIGHT THEN LANDED WITHOUT ANY FURTHER INCIDENT. MAINTENANCE TROUBLESHOT SYSTEM AND FOUND A BAD LANDING GEAR SELECTOR VALVE AND REPLACED. OPS CHECK GOOD.
DURING DESCENT, THE CREW DETECTED A BURNING ODOR. NO WARNING WAS TRIGGERED. THE CREW DECLARED AN EMERGENCY, AND LANDED WITHOUT INCIDENT. AN OPERATIONAL CHECK OF THE CABIN & FLIGHT COMPARTMENT AIR DISTRIBUTION, SMOKE DETECTION SYSTEM, AVIONICS WINDSHIELD HEAT, AND SIDE WINDOW HEAT WAS PERFORMED. A GVI OF THE AVIONICS RACKS, COCKPIT, CARGO AREAS, AND COWLING WAS PERFORMED, AS WELL AS A GVI FOR OIL LEAKS. OPERATIONAL CHECKS WERE GOOD, NO DEFECTS WERE NOTED, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING TAXI, THE AIRCRAFT EXPERIENCED A COMPLETE BRAKE FAILURE. THE AIRCRAFT WAS STOPPED USING THE EMERGENCY BRAKE, AND RETURNED TO GATE WITHOUT INCIDENT. THE FO'S MASTER CYLINDER WAS REPLACED, ALONG WITH THE LOW-PRESSURE RESERVOIR AND THE METERING VALVE. THE LOW-PRESSURE SYSTEM WAS BLED, THE OPERATIONAL CHECK WAS GOOD, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
FOUND CORROSION ON FUSELAGE SKIN BETWEEN FRAMES 17 AND 20 AND BETWEEN STRINGERS 13L AND 14L TO BE OUT OF SRM LIMITS. REPAIRED SKIN IAW SRAS S535R0272. 7876.
FOUND CORROSION ON FLOOR PANEL 214BF, OUT OF LIMITS. REPLACED FLOOR PANEL IAW SRM.
FOUND CORROSION ON RT INBD FLAP, OTBD END RIB, OUT OF LIMITS. REPLACED END RIB IAW SRM.
FOUND CORROSION ON LT INBD FLAP, OTBD END RIB, OUT OF LIMITS. REPLACED END RIB IAW SRM.
FOUND CORROSION ON CREASE BEAM WEB, OUT OF LIMITS. REPLACED WEB IAW SRM.
FOUND CORROSION ON CREASE BEAM WEB, OUT OF LIMITS. REPLACED WEB IAW SRM.
60 MILES OUT OF MYR THE NUMBER ONE ENGINE LOST OIL PRESSURE. THE ENGINE WAS SHUT DOWN THE QRH PROCEDURES WERE APPLIED. AN EMERGENCY WAS DECLARED. THE AIRCRAFT LANDED WITHOUT OTHER PROBLEMS.
DURING INSPECTION LEVEL 2 EXFOLIATION CORROSION WAS FOUND ALONG EDGE OF BEARING OUTER FITTING I/B ON THE RT AILERON OUTBOARD HINGE/SHACKLE ASSEMBLY. THE BEARING OUTER HINGE WAS REPLACED. INSPECTION REASON: MRB 576108-01 REFERENCE ATR 42 SRM 57-61-10 P115-116 FIG 1.
DURING INSPECTION LEVEL 2 CORROSION WAS FOUND ON THE AILERON "TEE PICKUPS" (TEE CLIPS) ON THE LEFT AILERON ASSEMBLY. THE EXFOLIATION CORROSION WAS ALONG THE EDGES AND SIDES OF THE FITTINGS. REFERENCE: ATR 42 SRM 57-61-10 P 115/116 FIG 1. REASON FOR INSPECTION MRB 576108-01.
THE LONGITUDINAL FLOORBEAM BL 0 AT FRAME 15 CRACKED NEAR ATTACH POINT TO THE LATERAL FLOORBEAM. CRACK WAS DISCOVERED DURING TROUBLESHOOTING ANOTHER UNRELATED PROBLEM. REPAIR WAS ACCOMPLISHED PER EO 4225001-42-53-32. THIS PART WAS INSTALLED BY STC ST01189W1.
