DURING THE DAILY AIRWORTHINESS CHECK, A MISSING RIVET WAS FOUND IN THE AREA OF THE TAIL GEARBOX. AFTER A CLOSER INSPECTION, NUMEROUS RIVETS WERE FOUND TO HAVE THE APPEARANCE OF STARTING TO BE "WORKING". THE TAIL ROTOR HEAD/BLADE ASSY WAS REMOVED ALONG WITH THE TAIL GEARBOX FOR FURTHER INSPECTION. INSPECTION REVELED UP TO 37 RIVETS REQUIRING REPLACEMENT, RIVETS WERE REPLACED ACCORDING TO MFG DIRECTIONS AND STANDARD MX PRACTICES. AFTER RIVET REPLACEMENT AND DURING REASSEMBLY A CHECK OF THE TAIL ROTOR HEAD BEARINGS RESULTED IN FINDING A DEFECTIVE TEETER BOLT BEARING BUSHING REQUIRING REPLACEMENT OF THE TAIL ROTOR SHAFT. WHEN THE SHAFT WAS REMOVED AN EXCESSIVE AMOUNT OF DAMAGE WAS DISCOVERED ON THE SPLINE AREA OF THE SHAFT AND A SIGNIFICANT AMOUNT OF METAL WAS FOUND IN THE SPLINES OF THE TAIL GEARBOX REQUIRING THE REPLACEMENT OF THE TAIL GEARBOX.
TAIL ROTOR GEARBOX CHIP LIGHT OCCURED DURING ENGINE SHUTDOWN. A VISUAL INSPECTION FOUND A CHIP OUT OF LIMITS. REMOVED FAILED GEARBOX. GEARBOX AND SHAFT ASSY, ASSEMBLED AND INSTALLED ON ACFT, PERFORMED VIBRATION & BALANCE CHECK AND RETURNED ACFT TO SERVICE.
TAILROTOR GEAR BOX MOUNT FITTING AT TOP OF VERTICAL FIN CRACKED AND TAIL ROTOR GEAR BOX MOUNTING STUD ROSSET FITTING LOOSE.
A CRACK WAS FOUND AT THE TOP OF THE TAILBOOM VERTICAL FIN WHILE TROUBLESHOOTING A TAIL ROTOR VIBRATION ISSUE. THE CRACK WAS FOUND ON THE LT SIDE OF THE ACCESS SIDE OF THE OPENING FOR ACCESS TO THE TAILROTOR GEARBOX MOUNTING HARDWARE.

LAST REPAIR REF RO 9-090006-18 MMIR CLAIM NR 91640 (ALSO A WARRANTY REPAIR FOR UNIT FAILING OUT OF BOX). THIS WILL BE THE THIRD REPAIR ON THIS UNIT IN 8 MONTHS.

BLADE HAS ACCUMULATED 106.1 HOURS SINCE 10/23/2001 AND HAS BEEN REPAIRED THREE TIMES IN THAT TIME SPAN WITH THIS BEING THE FOURTH. AIR METHODS HAD BLADE INSTALLED ON A/C 7111 ON 02/26/2003 FOR A TOTAL OF .3 HOURS BEFORE RECEIVED.

SINCE FEBRUARY 21,200 BLADE HAS ACCUMULATED ONLY 279.8 HOURS BUT HAS BEEN REMOVED FROM SIX DIFFERENT AIRCRAFT FOR REPAIR WITH THIS BEING THE SEVENTH TIME. THIS TIME BLADE WAS INSTALLED FOR A TOTAL OF 1.8 HOURS ON N911TG/7506. LAST REPAIR 9-090000-65 S/N 2254 ; BLADE WILL NOT TRACK, CLIMBS AND DIVES FROM GROUND TO HOVER TO FLIGHT. WE RECEIVED BLADE ON EXCHANGE UNDER OUR PO 8-080107-53. BLADE WAS INSTALLED FOR A TOTAL OF 1.8 HOURS ON N911TG/7506. BLADE HAS ACCUMULATED ONLY 19.3 HOURS BUT HAS BEEN REPAIRED THREE TIMES SINCE 11/13/2001.

BLADE HAS ACCUMULATED ONLY 733.4 HOURS ON IT SINCE 09/15/1997 BUT HAS BEEN IN FOR REPAIR EIGHT TIMES WITH THIS BEING THE NINTH.
BOLTS JUST PURCHASED, BUT THE COTTER PINS HOLES ARE DRILLED WRONG, MAKING THE BOLTS UNUSEABLE. REQUEST WARRANTY CREDIT ON THESE BOLTS.
MAIN ROTOR BLADE HAS MINOR LEAD LAG PLAY. BLADE LASTED ONLY 421.6 HOURS SINCE LAST REPAIR. PART IS ALSO DUE LOOP INSPECTION.

DURING A PRE-FLIGHT INSPECTION, THE MECHANIC FOUND A 3 INCH CRACK AT THE ROOT END OF TAIL ROTOR BLADE. THIS IS THE THIRD EVENT. 11-18-94, 198.4 HRS, S/N 116/118. 5-3-96, 1,050.1 HRS, S/N 103/104. 858.5 HOURS ON THE BLADES, SAME P/N.
FORWARD AND REAR MOUNT BRACKETS CRACKED DUE TO VIBRATION AND SB 133-1 HAD NOT BEEN ISSUED YET AND HAD NOT BEEN C/W AT THE TIME. INSTALLED NEW COIL ASSY, PN JBS3001-3, AND C/W SIB 133-1.
IN CRUISE FLIGHT, 60 PERCENT TORQUE WHEN ENGINE IS BEEPED TO 100 PERCENT, N2 TORQUE DROPS OFF 40 PERCENT, AND N1 DROPS OFF 10-15 PERCENT. SEEMS TO OPERATE NORMALLY WHEN N2 IS AT 98-99 PERCENT.
DURING ROUTINE BATTERY CHANGE, MECH NOTICED 3 OUT OF 4 MOUNTS ON A/C EVAPORATOR ASSY CRACKED. UNIT JUST INSPECTED 5 DAYS PRIOR WITH NO DEFECTS. UNIT LOCATED BETWEEN ENG EXHAUST EJECTORS. THESE AFT COWLING ARE NOT NORMALLY REMOVED BETWEEN ENG CHANGES OR MAJOR INSPECTIONS WITH EXCEPTION OF 1 COWLING REMOVED TO SERVICE BATTERY. IT IS KNOWN BY MFG THIS PROBLEM EXISTS. REF A.E.C. LETTER DATED MAY 2, 1995. AN INTERVAL TIME NEEDS TO BE ESTABLISHED SO UNIT CAN BE REPEATEDLY INSPECTED, 25 FLIGHT HRS SEEMS ADEQUATE. SB OR TECH BULLETIN NEEDS TO BE ISSUED. UNIT SITS ABOVE T/R DRIVE SHAFT & REQUIRES A CERTIFIED TECH TO EVACUATE FREON SYS & SERVICE UNIT ACCORDING TO FED REGULATIONS, A SERVICE NOT READILY AVAILABLE.
