ROOF ANGLE P.N. L533M1017266 HAS A APPORX. 3.0 INCH CRACK TO THE RIGHT OUTBOARD PROTION.
MAIN ROTOR TRANSMISSION DECK P.N. L533M1017293 HAS CRACKS THAT MEASURE 8.50 INCHES AND 1.0 INCHES IN LENGTH.
TAIL CONE SKIN CRACKED WITH APPROXIMATELY 3.3 INCH CRACK TO RIGHT TAILCONE SKIN P.N. L533M1022216.
LEFT HAND AND RIGHT HAND FUSELAGE FRAME 5 STRUCTURE P.N. L533M1105219 (RIGHT) AND P.N. L533M1088219 (LEFT) HAVE CRACKS TO THE INSIDE RADIUS OF THE FRAME DOUBLERS.
THE LOWER ANGLE P.N. L533M1052318 FOR THE ATTACHMENT OF THE MAIN ROTOR CONTROL ROD CENTER POST CORRODED BEYOND SERVICEABLE LIMITS REQUIRING REPLACEMENT.
THE RIGHT HAND ENGINE DECK P.N. L533M1018246 HAS A 4.5" AND A 5.5 INCH CRACK 4 INCHES AND 1 INCH AFT OF THE INBOARD ENGINE MOUNT.
WHILE ON FINAL APPROX 200-250 FEET WHEN THE NUMBER 1 ENGINE CHIP AND OIL PRESSURE LIGHT CAME ON CONFORMATION OF THESE INDICATIONS CONFIRMED WITH OIL PRESSURE INDICATION AT 0 PSI. THE #1 ENGINE WAS SECURED, AN EMERGENCY WAS DECLARED WITH A RUN ON LANDING PERFORMED SUCCESSFULLY. THE #1 ENGINE TROUBLESHOT AT THIS TIME TO A CRACK IN THE ACCESSORY GEAR BOX NEAR THE STARTER-GENERATOR. THE AFFECTED ENGINE P.N. 311720001 S.N. BB0097 AND AFFECTED STARTER-GENERATOR P.N. 23081-073 S.N. P2165 WERE REMOVED AND REPLACED WITH OVERHAULED SERVICEABLE ENGINE P.N. 311720001 S.N. BB0073 AND SERVICEABLE #1 STARTER-GENERATOR P.N. 23081-073 S.N. Y10271. THE ORIGINAL STARTER-GEN WAS REQUESTED CHANGE BY PRATT AND WHITNEY FOR EVALUATION. DURING THE ENGINE CHANGE ALL OF THE ENGINE OIL LINES AND OIL COOLER WERE CONFIRMED NON-CONTAMINATED.
COCKPIT DISPLAY SYS, CDS INTERMITTENTLY DISPLAYS INACCURATE INFORMATION WHEN AMBIENT AIR TEMP IS ABOVE 85 DEGREES. THIS COMPONENT WAS INSTALLED LESS THAN A YEAR AGO & 476 HRS AGO. THIS WAS AN 0.0 TSO PART & SHOULD NOT BE FAULTY THIS SOON.

METAL CONTAMINATION OF OIL SYSTEM.
DRIVE SPLINES WERE DOWN AND WOULD NOT TURN BLOWER.
TWO MINUTES AFTER TAKEOFF, POWER DECREASED TO 20 - 40 PERCENT OF NORMAL ON NR 1 ENGINE. RETURNED FOR PRECAUTIONARY LANDING. ENGINE APPEARED TO FUNCTION NORMALLY BELOW 50 PERCENT TORQUE, ABOVE 50 PERCENT, TORQUE WOULD DECREASE. NO FURTHER INCIDENT OCCURRED. AIRCRAFT SHUT DOWN. ELECTRICAL CONNECTIONS CHECKED. OPERATIONAL CHECK FLIGHT PERFORMED AND RETURNED TO SERVICE.
