Maintenance disclosures
AT 11,000 FEET & 40 MILES FROM HOU. THERE WAS A POP SOUND WITH AN AMBER CAS MESSAGE OF ACM BLEED LEAK & A BLUE CAS MESSAGE BAGGAGE HEAT OFF. A FEW SECONDS LATER THERE WAS THE SMELL OF SMOKE IN THE COCKPIT. LOOKING BACK INTO THE CABIN I COULD SEE IT FILLING WITH BLUE/GREY SMOKE. IT HAD A RUBBER BURNING SMELL. TURNED OFF BOTH BLEED AIR SOURCES & THE EMERGENCY PRESSURIZATION ON, THIS SEEM TO GIVE SOME RELIEF TO THE SMOKE. WE WERE GIVEN EMERGENCY HANDLING, LANDED RUNWAY 12 RIGHT AT HOU. EXITED THE RUNWAY & EVACUATED THE AIRCRAFT ON THE TAXIWAY. NEVER SAW THE CAS MESSAGE FOR EMERGENCY PRESSURIZATION ON. EVIDENCE OF OVERHEATING ON THE PIPES COMING OFF THE ACM. WE RAN THE APU FIRST TO TRY TO FIND ANY FAILURES, NONE WERE FOUND. STARTED EACH ENG DEPENDENTLY TO TRY & FIND A PROBLEM, NOTHING FOUND. STARTED BOTH ENGS & RAN IT UP TO CRUISE POWER, DUPLICATING AS CLOSE AS POSSIBLE ON THE GND THE CONDITION THE AIRCRAFT WAS IN WHEN THE EVENT HAPPENED, NO FAULT FOUND.
ON POST FLIGHT INSPECTION A/C HAD SMOKE COMING FROM E & E COMPARTMENT DRAIN HOLES. REMOVED AND REPLACED BOTH MAIN BATTERIES AND BATTERY CHARGER IAW AMM 24-30-01 AND 24-30-22 OPS CHECK IAW WITH AMM.
LT INBOARD FLIGHT SPOILER DRIVE LINK SHOWS CRACKED DURING ULTRASONIC INSPECTION. REMOVED AND REPLACED LEFT HAND INBOARD FLIGHT SPOILER DRIVE LINK PER EO DC9-826--91 REV A.

REMOVED CRACKED SECTION OF SKIN, FABRICATED REPAIR DOUBLER, (2) FILLERS, (2) FINGER DOUBLERS, ALODINED, PRIMED & INSTALLED PER DC9 SRM 53-04 FIGURE 12B, PG 54, 56, 60 BV, 60JC, 53-30-0, PG 18-21, 51-10-3, PG 20, & 23-24. REPETITIVE INSPECTION REQUIRED EVERY 8,000 CYCLES PER DC9 SRM 53-04 FIGURE 12B, PG 33, PAR B.

MZJ - FOUND SCREW HOLE WAS FOUND TO HAVE METAL TORN OUT AND EDGE OF METAL BENT ON LEFT WING INBOARD LEADING EDGE 19TH HOLE FROM OUTBOARD END. TRIMMED DAMAGE, HFEC AREA, FABRICATED FILLER AND DOUBLER. INSTALLED PER ECRA 915-57-057-2. (M)
MZJ - FORWARD PASSENGER DOOR UPPER FORWARD CORNER HAS A CRACK IN INNER MOST DOUBLER. REPAIRED PER ECRA 915-53-058-2 BY REPLACEMENT OF DOUBLER AFTER TRIMMING OUT DAMAGE. (M)
DURING HIGH SPEED PHASE OF TAKEOFF ROLL, LOUD BANGING NOISE HEARD FROM NOSEGEAR. AFTER TAKEOFF AIRCRAFT REMAINED IN GROUND MODE. INSPECTION AND SERVICED NOSE STRUT, NO DEFECTS/LEAKS NOTED. AT DEPARTURE, NLG SHOCK STRUT LEAKED FLUID AND DEFLATED. SERVICED NOSE STRUT PER ECRA 915-32-048-1 PART 1. (M)
CAPTAINS ATTITUDE INDICATOR FAILED, 90 DEG NOSE UP PITCH. CIRCUIT BREAKER RESET ONCE AND FAILED AGAIN. AFTER GYRO FAILURE, AN ELECTRICAL BURNING SMELL IN COCKPIT. REPLACED CAPTAINS ATTITUDE INDICATOR. OPS CHECK NORMAL. UPON FURTHER INSPECTIN FOUND NO DEFECTS WIEHT A/C ELEC SYSTEM AND WIRING. ELECTRICAL SMELL DETERMINED TO BE CAUSED BY INTERNAL FAILURE OF ADI INSTRUMENT. (M)

DFAR - FLT EIA 912 - IN CRUISE FROM FSD TO FAR AT 22,000 FEET AND CABIN PRESSURE AT 7 PSI, HEARD A POP AND CABIN STARTED CLIMBING AT 1,500 FEET. DESCENDED TO 10,000 FEET AND CABIN STABILIZED AT 2 PSI AND 8,000 FEET. LANDED IN FAR. LOCATED BLOWN HOSE IN RT TAIL COMPARTMENT AREA. REINSTALLED AND PRESSURIZED AIRCRAFT TO 4.0 PSI. ALL SYSTEMS CHECKED NORMAL. (X)

REPLACED NR 1 AND NR 2 MAIN AIRCRAFT BATTERIES. REPLACED DC TRANSFER BUS RELAY. TESTED BATTERY CHARGER TEMP SENSOR. PERFORMED DC EMERGENCY POWER SYSTEM AND BATTERY CHARGER OPERATION TEST.
