THE LH NLG WHEEL/TIRE ASSEMBLY SEPARATED FROM THE AIRCRAFT ON TAKEOFF AND LANDED ON AIRPORT PROPERTY.
THE OXYGEN CYLINDER VALVE WAS FOUND TO HAVE A GROOVE AT THE REGULATOR CONNECTION CAUSING AN OXYGEN LEAK. MAINTENANCE REPLACED THE OXYGEN CYLINDER/VALVE ASSEMBLY WITH A SERVICEABLE UNIT. THE LAST HYDROSTATIC INSPECTION/OVERHAUL OF THE DEFECTIVE UNIT WAS PERFORMED ON 5/13/2019. THE NEXT OVERHAUL DUE IS 5/13/2022
AFT CARGO DOOR LOWER FRAME PAN DENTED. FRAME PAN WAS COLD WORKED TO ORIGINAL CONTOUR AND INSPECTED USING HIGH-FREQUENCY EDDY CURRENT INSPECTION METHOD. ALL WORK ACCOMPLISHED IAW M7 AEROSPACE EO 42R47000-14 DATED MAY 19, 2021, FAA FORM 8110-3.
LANDING GEAR DID NOT EXTEND. HAD TO USE ALTERNATE GEAR EXTENSION. MAINTENANCE FOUND GEAR SELECTOR VALVE INOPERATIVE. REMOVED AND REPLACED GEAR SELECTOR VALVE.
RECIRCULATION FAN #1 EMITTING BURNING SMELL IN FLIGHT. MAINTENANCE REPLACED #1 RECIRCULATION FAN.
AFTER TAKEOFF, ACFT HAD A FAILURE OF THE GREEN HYDRAULIC SYSTEM. THE CREW LEVELED OFF AT 4000 FT AND WENT THROUGH APPROPRIATE CHECK LISTS. DUE TO THE LANDING GEAR DID NOT FULLY RETRACT THE CREW DECLARED AN EMERGENCY. A MANUAL GEAR EXTENSION WAS PERFORMED AND THE ACFT LANDED UTILIZING EMERGENCY BRAKING. R & R LEAKING HYDRAULIC LINE ON RT MLG IAW IPC 32-31-50, FIG 1A. LEAK CHECK HYDRAULIC SYSTEM IAW JIC 29-00-00, LEAK CK 10010.
AFTER DEPARTURE THE NR 1 DC GEN FAILED INTERMITTENTLY RESULTING IN THE LOSS OF THE PILOTS & COPILOTS ADI, HSI & THE PILOTS AIR DATA INSTRUMENTS ACCOMPANIED BY CRC & GPWS WARNINGS & REPETITIVE CHIME. AN EMERGENCY WAS DECLARED. AIR TURN BACK WAS EXECUTED, THE FLIGHT LANDED WITHOUT FURTHER INCIDENT. AFTER TROUBLE SHOOTING THE GCU WAS FOUND TO BE BAD. THE GCU WAS REPLACED IAW JIC 24-32-81 RAI 10000. THE SYS CHECKED GOOD & THE ACFT WAS RETURNED TO SERVICE.
ON LANDING THE COPILOTS LT BRAKE DID NOT OPERATE ON LANDING. TROUBLESHOT THE BRAKE SYS IAW JIC 32-42-00 FUT 10000. NO FAULTS FOUND. BLED THE COPILOTS LOW PRESSURE BRAKE SYS IAW JIC 32-42-00 BLD 10000. SYS CHECKED GOOD & THE AIRCRAFT WAS RETURNED TO SERVICE.
COPILOTS RIGHT WHEEL BRAKE INOPERATIVE, LITTLE EFFECT, PEDAL GOES TO THE FLOOR. TROUBLESHOOT THE SYSTEM. BLED LOW PRESSURE LINE IAW JIC 32-42-00, BLED 10000 AND BLED THE HIGH PRESSURE LINE IAW JIC 32-42-00 BLD 10010. SYSTEM CHECKED GOOD. AIRCRAFT RETURN TO SERVICE.

ON LANDING OF FLIGHT 7369 DHN-MEM THE PILOT HAD NO BRAKES. AFTER THE PILOT TAXIED THE AIRCRAFT TO THE RAMP THE MECHANIC TROUBLESHOOT THE SYSTEM, PERFORMED OPS CHECK OF THE PROX SWITCH ELECTRIC UNIT IAW ATR42 MM JIC 32-61-83OPT10000. PERFORMED NORMAL BRAKING OPERATIONAL TEST IAW ATR 42 JIC 32-42-00 OPT10000. TAXIED AIRCRAFT TO CHECK STEERING AND BRAKING SYSTEM OF AIRCRAFT IAW MAC ATR 42 72 MAINTENANCE ENGINE RUN AND TAXI CHECK LIST. ALSO INSTALLED NEW RELAY AT FIN 27GB FOR PRECAUTIONARY REASONS IAW ATR 42 SPM 20-45-50. THE SYSTEM CHECK GOOD AND THE AIRCRAFT WAS RETURN TO SERVICE.
AFTER TAKEOFF, THE LANDING GEAR DID NOT RETRACT. THE PILOT DECIDED TO RETURN TO DEPARTURE. MAINT DETERMINED THAT A UPLOCK WAS OUT OF SEQUENCE. UPLOCK WAS RECYCLED AND ACFT WAS RETURNED TO SERVICE. ACFT BLOCKED OUT FOR A SECOND TIME. AFTER TAKEOFF, LANDING GEAR DID NOT RETRACT FOR THE SECOND TIME. PILOT RETURNED THE ACFT TO DEPARTURE. ACFT WAS THEN TAKEN FOR A GEAR RETRACTION AND TROUBLESHOOTING. MX FOUND A BAD LANDING GEAR SELECTOR. THE SELECTOR WAS REMOVED AND REPLACED AND THE ACFT RETURNED TO SERVICE.
REPAIRED CORROSION ON THE LEFT WING REAR SPAR UPPER EXTRUSION VERTICAL FLANGE BETWEEN RIBS 24-27 AS FOLLOWS. BLENDED OUT CORROSION IN ACCORDANCE WITH ATR STRUCTURAL REPAIR INSTRUCTIONS DO/TS-107/09-11167 ISSUE A. THE DEVIATIONS WERE THAT HOLES WERE OVERSIZED TO THE 2ND OVERSIZE. EDDY CURRENT OF THE BLENDED AREAS AND ROTO TEST OF THE FASTENER HOLES. NO CRACK INDICATIONS WERE FOUND. APPROVED DATA FOR THIS REPAIR IS ATR STRUCTURAL REPAIR APPROVAL SHEET (SRAS) DO/TS-107/09.
DURING INSPECTION LEVEL 2 CORROSION WAS FOUND ON A TEE FITTING INSIDE THE RIGHT WING AILERON. THE AILERON WAS REPLACED.
