FLT 1018. F/A REPORTED ODOR IN CABIN ACCOMPANIED BY HAZE, POSSIBLE AIR CONTAMINATION, IN CRUISE & DESCENT. NO CONTAMINATION OR ODOR NOTICED IN COCKPIT. LANDED WITHOUT FURTHER INCIDENT. ACFT INSPECTED & RELEASED FOR A MX FLT. APPROX 2 HOURS INTO FLT, 2L LAV SMOKE DETECTOR CAME ON, ALONG WITH SLIGHT ODOR IN VICINITY OF 2L LAV. SMOKE DETECTOR WENT OUT AFTER FEW MINUTES, & SMOKE CLEARED. REPEATED 3 TO 4 TIMES. TURNED OFF LT PACK & RT PACK FOR AWHILE. NO AFFECT ON SMOKY ODOR. FLT CONTINUED & LANDED WITHOUT FURTHER INCIDENT. FOUND APU OIL COOLING FAN PLENUM TO HAVE DAMAGE. PERFORMED REMOVAL OF OIL CONTAMINATION FROM THE AC & PNEUMATIC SYS TASK, USING BOTH ENGINES & HUFFER. REPLACED APU COOLING FAN ASSY.
CLT - PILOT REPORT - MAJOR REPAIR - LOG 8480307 - CRACK IN FUSELAGE SKIN AT BS1340, S27L. REPAIRED FUSELAGE SKIN AT BS1340, S27L IAW US AIRWAYS EA 101205B.
PHL - THE L4 DOOR GIRT BAR ENGAGEMENT LIGHT WAS EXTINGUISHED WITH THE EVACUATION SLIDE ARMING LEVER IN THE ARMED POSITION. MAINTENANCE RELAMPED THE 4L DOOR EVACUATION SLIDE LIGHT. OPERATIONALLY CHECKS OK.
FLT 1752. THE CREW REPORTED THAT THE HORIZONTAL STABILIZER TRIM RAN TO FULL NOSE UP UNCOMMANDED, RESULTING IN DIFFICULTY IN KEEPING THE NOSE OF THE AIRCRAFT DOWN DURING LANDING ROLL OUT. MAINTENANCE REPLACED THE RIGHT FLT CONTROL COMPUTER (FCC). CAT SYSTEM TESTED SATISFACTORY.
CLT - THE L1 DOOR EMERGENCY ESCAPE SLIDE WAS FOUND NOT COMPLETELY ATTACHED TO THE DOOR. MAINTENANCE REPLACED THE L1 DOOR EMERGENCY ESCAPE SLIDE. OK FOR SERVICE. (S)
BFM - MAJOR REPAIR - LEFT WING INBOARD FLAP TRAILING EDGE CRACKED. REPAIRED LEFT WING INBOARD FLAP TRAILING EDGE CRACKED IAW B757 SRM 57-53-01.
PHL FLT 1250 MCO TO PHL. AFTER A JOLT FROM APPARENT WAKE TURBULENCE, APPROXIMATELY 45 MILES FROM BWI, AT FL 270 AND 320 KIAS, A PANEL ON THE UPPER LEFT WING SEPARATED FROM THE AIRCRAFT IN FLIGHT. THE PANEL WAS LOCATED AFT OF THE REAR SPAR BUT FORWARD OF THE TRAILING EDGE FLAPS, AND FROM ABOUT WBL 79.2 TO ABOUT WBL 130.5. UPON SEPARATION, A PAIR OF HYDRAULIC LINES WERE SEVERED AND A PASSENGER WINDOW AT SEAT ROW 19, LEFT SIDE, WAS CRACKED. THE LIBERATED LEFT WING INBOARD TRAILING EDGE FIXED PANEL AND SUPPORT BEAM CLIPS WERE REPLACED. SEVERAL REPAIRS WERE MADE TO THE LEFT WING TRAILING EDGE FLAPS WHERE THE PANEL CONTACTED THEM AS IT SEPARATED FROM THE AIRCRAFT. FINALLY, AD 91-14-21 WAS REACCOMPLISHED TO ENSURE ALL LEFT WING INBOARD TRAILING EDGE FIXED PANEL SUPPORT BEAM CLIPS, THE CAUSE OF THE PANEL LIBERATION, WERE IN COMPLIANCE.

PIT - MAJOR REPAIR - C3 CHECK, CRACKED CANTED FRAME CHANNEL, BS 1068. REPAIRED CANTED FRAME, BS 1068 IAW B757 SRM 53-00-07.
FLIGHT 1297: ENROUTE, THE WEATHER RADAR FAILED. FLIGHT RETURNED. MAINTENANCE FOUND AND REMOVED A BLOCKAGE IN THE WAVE GUIDE. NO FURTHER FAULTS.
FLIGHT 714, CLT-PVD: AFTER TAKEOFF, EICAS MESSAGES -GEAR DISAGREE- AND -GEAR DOORS- REMAINED ILLUMINATED. THE FLIGHT DIVERTED TO PHL. LANDING WAS NORMAL. MAINTENANCE ACCOMPLISHED TROUBLESHOOTING PER FIM 32-30-00 WITH NO FAULTS FOUND. NO PSEU FAULTS WERE LOGGED. LUBED GEAR AS PRECAUTIONARY AND RELEASED FOR SERVICE.
