THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 3572 THE CREW REPORTED THE FLIGHT ATTENDANT FORWARD FLASHLIGHT HOLDER WAS BROKEN. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE FLIGHT ATTENDANT FORWARD FLASHLIGHT ASSEMBLY IAW AMM 33-50-16. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING MAINTENANCE INSPECTION FOUND THE OUTSIDE PASSENGER MAIN DOOR HANDLE WOULD NOT OPEN DOOR. INSIDE HANDLE OPERATES NORMALLY. THE AIRCRAFT WAS REMOVED FROM SERVICE. LUBRICATED MAIN PAX DOOR ACTUATING AND LOCKING MECHANISM SYSTEM, REF EMB AMM 52-11-00. DOOR OPENS OK FROM OUTSIDE, REF EMB AMM 1 / 52-18-00. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 4234 THE CREW REPORTED A VERY STRONG ELECTRICAL BURNING ODOR IN THE COCKPIT AND FORWARD CABIN. THE AIRCRAFT WAS REMOVED FROM SERVICE. PERFORMED FIM TASK 21-20-00-810-801-A. FOUND NR 1 RECIRCULATION FAN INOP. REMOVED AND REPLACED THE NR 1 RECIRCULATION FAN IAW EMB 145 AMM 21-24-01. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 4281 THE CREW REPORTED THAT JUST PRIOR TO LANDING AII CREW MEMBERS BEGAN SMELLING A FAINT ABNORMAL ODOR. THE ODOR BECAME PROGRESSIVELY MORE PUNGENT AND WAS VERY STRONG. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE NR 2 WINDSHIELD TEMP CONTROLLER IAW AMM 30-42-01. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.

ON FLIGHT 3329 THE CREW REPORTED THAT ON TAKEOFF HAD A RED FLAG ON THE INTEGRATED STANDBY INSTRUMENT SYSTEM (ISIS). THE INTEGRATED STANDBY INSTRUMENT SYSTEM (ISIS) WAS INOPERABLE. THE CREW RETURNED TO DFW. THE AIRCRAFT LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS REMOVED FROM SERVICE. RESET ISIS, THE TEST WAS GOOD IAW AMM 34-01-01 AND NO FAULTS NOTED. THE AIRCRAFT WAS RETURNED TO SERVICE.
FLIGHT 4312, THE CREW REPORTED THE FORWARD FLIGHT ATTENDANT FLASHLIGHT CRADLE WAS NOT INDICATING A CHARGING MODE. AIRCRAFT REMOVED FROM SERVICE. REMOVED AND REPLACED THE FLASHLIGHT ASSEMBLY IAW AMM 33-50-16. OPS CHECKED GOOD. AIRCRAFT RETURNED TO SERVICE.

ON FLIGHT 3971 THE CREW REPORTED THE CAPTAINâS FLASHLIGHT WAS INOPERABLE. ITEM DEFERRED PER MEL 25-53. REMOVED AND REPLACED THE CAPTAIN'S FLASHLIGHT IAW EMB 145 AMM 33-50-16. THE OPERATIONAL CHECK WAS GOOD AT THIS TIME. MEL RESTORED. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 3946 DURING TAKEOFF THE CREW REPORTED THEY RECEIVED AN EICAS LANDING/GROUND AIR FAIL MESSAGE. THE CREW RAN THE CHECKLIST AND DECLARED AN EMERGENCY. THE CREW RETUNRED TO ORD AND LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS REMOVED FROM SERVICE. THE OPERATIONAL CHECK WAS GOOD ON ALL WEIGHT ON WHEELS PROXIMITY SENSORS IAW AMM 32-63-05. REMOVED AND REPLACED LANDING ELETRONIC GEAR UNIT IAW AMM 32-32-01. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING MAINTENANCE BASE VISIT FOUND THE EMERGENCY LIGHT IN CELING PANEL 243DLC TO BE INOPERABLE. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED LIGHT BULB AT CELING PANEL 243DLC IAW EMB 145 AMM 33-50-08. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING MAINTENANCE BASE VISIT FOUND THE PASSENGER DOOR INNER HANDLE BOX HAS CRACKED ANGLE. THE AIRCRAFT WAS REMOVED FROM SERVICE. STOP DRILLED CRACK AND FABRICATED REINFORCEMENT IAW EO EM-53-MR-250. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 3614 DURING CRUISE THE CREW REPORTED, RECEIVED A BATTERY OVERTEMP MESSAGE. THE BATTERY TEMPERATURE WENT TO 88 DEGREES CELSIUS. THE CREW DECLARED AN EMERGENCY AND ELECTED TO DIVERT TO RSW. THE AIRCRAFT LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED BOTH MAIN BATTERIES IAW AMM 24-36-01. THE OPERATIONAL CHECK WAS GOOD AT THIS TIME AND THE AIRCRAFT WAS RETURNED TO SERVICE.

LAVATORY SMOKE EICAS MESSAGE CAME ON IMMEDIATELY AFTER TAKEOFF. PERFORMED ENGINE RUNS AND LAVATORY SMOKE DETECTOR TEST WITH OPERATIONAL CHECK GOOD. AIRCRAFT RETURNED TO SERVICE. (K)
BNA - FLT 4539 - PILOT REPORTS: ON TAKEOFF, THE LEFT ENGINE FAILED, CAPTAIN LOOKED OUT THE WINDOW AND VERIFIED FLAMES. CREW ABORTED TAKEOFF AND FIRED BOTTLES. UPON TAXIING BACK IN, CFR STOPPED THE A/C AND TOLD THEM THERE WERE STILL RESIDUAL FLAMES. PASSENGER DEPLANED ON THE RAMP WITH NO INJURIES.
LIT - UPON REMOVAL OF RUDDER ASSY, DURING HEAVY MAIN BASE VISIT, UPPER VERTICAL STABILIZER TO RUDDER HINGE HAD FATIGUE CRACKING AROUND THE CENTER LUG OF THE HINGE. REMOVED AND REPLACED HINGE.
