A NINE INCH LONG CRACK ON MAIN ROTOR BLADE BETWEEN STATIONS 166 - 173.
12/26/2014 AT APPROX 09:45, ON TAKEOFF, THE FIRE WARNING LIGHT ILLUMINATED. ACFT WAS IMMEDIATELY RETURNED & LANDED WITHOUT EVENT. MX FOUND A BAD FLAME DETECTOR, PN 92310-04802-105, SN REMOVED 5853, INSTALLED SN 4565. THERE WAS NO INDICATION THAT A FIRE HAD OCCURRED, ACFT RETURNED TO SERVICE.
AFTER TAKEOFF, THE FIRE WARNING LIGHT ILLUMINATED. ACFT WAS IMMEDIATELY RETURNED AND LANDED WITHOUT EVENT. MAINTENANCE FOUND A BAD FLAME DETECTOR, PN: 92310-04802-105, SN:5853 REMOVED, INSTALLED SN: 4565. THERE WERE NO INDICATIONS THAT A FIRE HAD OCCURRED, ACFT RETURNED TO SERVICE.
DURING 1250 HOUR INSPECTION, REMOVED BAGGAGE COMPARTMENT HEADLINER AND FOUND STRINGER CRACKED ABOUT 3 INCH LONG. SPOT DRILLED CRACK. FABRICATED DOUBLER. INSTALLED IAW DRB 2013-5A-92-059.
WHILE PILOT WAS PERFORMING PREFLIGHT CHECKS, THE LIFE RAFT DEPLOYED. VERIFIED THAT SWITCHES WERE IN CORRECT POSITIONS ACCORDING TO ACFT CHECKLIST. S92 TECH REP REVEALED THAT THIS IS AN ISSUE THAT IS KNOWN AND IS CURRENTLY BEING INVESTIGATED. ACCORDING TO THE REPS THE PROBLEM STEMS FROM ELECTRICAL CONNECTORS TO THE SQUIBS WHICH ARE UNACCESSIBLE TO MX TECH. FURTHER INFORMATION HAS BEEN REQUESTED FROM TECH REPS WHEN INFORMATION BECOMES AVAILABLE.
LEFT MAIN GEAR STRUT COLLAPSED AND FLUID ON GROUND.
