HIGH VIBRATION ON CLIMB CONTINUED FOR REMAINDER OF FLIGHT. FOUND RT WING NR 2 SPOILER DELAMINATED.
RETURNED TO DEPARTURE DUE TO HIGH STRUCTURE VIBRATION ON CLIMB PHASE PASSING 16,000 AND REMAINED FOR REST OF FLIGHT.
DURING POSTFLIGHT WALKAROUND, FOUND A DENT ON FUSELAGE FWD OF THE FWD CARGO DOOR. REPAIRED IAW STS EO 100-2014-114. ALL WORK DOCUMENTED ON NR 26850, 002651, 003002.
THE CREW REPORTED THAT DURING TAKEOFF, THERE WAS AN AIRSPEED DIFFERENCE AFTER 80KTS. THE F/O AIRSPEED INDICATOR DROPPED TO 55 KTS WHILE THE CAPTAINâS AIRSPEED INDICATOR READ 95 KTS. REJECTED TAKEOFF AT 95 KIAS AND TAXIED BACK TO THE RAMP WITHOUT ANY FURTHER INCIDENT. FIM 34-11-02, PURGED LINES OF THE FIRST OFFICERâS PITOT SYSTEM IAW AMM 34-11-00. PERFORMED PITOT SYSTEM LEAK TEST IAW AMM 34-11-00. NO LEAKS NOTED.
ON REACHING TOP OF CLIMB, A LEVEL 2 OIL PRESSURE LOW DISPLAYED. CREW FOLLOWED THE QRH PROCEDURES SIMULTANEOUSLY FOLLOWING THE QRH PROCEDURES "ENGINE SHUTDOWN IN FLIGHT" FOR ENG NR 2. THE CREW ACCOMPLISHED THE CHECKLIST THEN PROCEEDED TO DUMP APPROXIMATELY 50 KGS OF FUEL. THE CREW DECLARED AN EMERGENCY LANDING, BUT RETURNED BACK TO DEPARTURE WITHOUT ANY FURTHER INCIDENTS OR DELAYS. VISUALLY INSPECTED ENG NR 2, FOUND METAL SEDIMENT AT CHIP DETECTORS AND IN MAIN OIL FILTER. REPLACED NR 2 ENGINE.
DURING HEAVY CHECK, FLIGHT COMPARTMENT, MAIN DECK, AND LOWER CARGO COMPARTMENT FLOOR PANELS WERE FOUND DAMAGED. REPAIRED FLOOR PANELS IAW EO SLC-140313.
DURING HEAVY MAINTENANCE, MAINTENANCE FOUND UPPER PLATING AROUND BS 1229 BETWEEN RBL 57 TO 78 CORRODED. REPAIRED IAW EO SLC-140609.
DURING HEAVY CHECK, LT WING UPPER PLATE AT LWS 160 AND S15L WAS FOUND TO BE CORRODED. DAMAGE REPAIRED IAW EO SLC-140317.
DURING HEAVY CHECK, MAIN DECK CARGO HANDLING SYSTEM, CENTERLINE RESTRAINT ASSY SIDE FITTINGS WERE FOUND CRACKED. R & R 38 EACH CENTERLINE RESTRAINTS SIDE FITTINGS. REPAIR OF SIDE FITTINGS ACCOMPLISHED IAW EO SLC-121214.
DURING HEAVY CHECK, UPPER PLATING AROUND BS 1229 AROUND RBL 8 FOUND CORRODED. REPAIRED IAW EO SLC-140611.
HYD NR 3 FAIL DURING CRUISE, FL380 20 MIN FROM SVMI. AIRCRAFT RETURNED TO AIRFIELD, LANDING NORMAL. MX FOUND PIPE ASSY BROKEN, TO HORIZONTAL STAB TRIM MOTOR NR 1. REMOVED AND REPLACED HYD LINE IAW 27-43-04. PERFORMED SYSTEM TEST AND FUNCTIONAL TEST OF HORIZ STAB HYD OP'S AND TRIM IAW 27-40-00. NO LEAKS NOTED. AIRCRAFT RETURNED TO SERVICE.
NR 1 SYSTEM HYD FLUID LOSS. RETURNED TO AIRFIELD. LANDING NORMAL. REPLACED NR 1 ENGINE LEFT AND RIGHT EDHP'S IAW 29-11-20, HYD MANIFOLD IAW 29-12-30. RUN UP AND LEAK CHECKED SYSTEM FOUND SATISFACTORY IAW 71-00-00. AIRCRAFT RETURNED TO SERVICE.
DURING CRUISE, LEVEL 2 ALERT 'HYD SYSTEM NR 1 LOW" ILLUMINATED. A/C RETURNED TO DEPARTURE SITE, WITH NORMAL LANDING. MX SERVICED HYD SYSTEM NR 1 IAW 12-13-00, OP'S CHECK GOOD. NO LEAKS NOTED. A/C RETURNED TO SERVICE.
DURING CLIMB CREW CALLED AND REPORTED NR 3 ENG HAD HIGH OIL TEMP. AIRCRAFT RETURNED TO AIRFIELD, LANDING NORMAL. MX T/S OIL TEMP WRITE UP IAW MD11 FIM 79-00-03 PG 108,109. PERFORMED INSP UP TO STEP 4 AND STEP 9, 14. REMOVED AND REPLACED MAIN OIL FILTER IAW 79-22-05. SERVICE NR 3 ENG OIL LEVEL, RUN UP AND LEAK CHECK SATISFACTORY, A/C RETURNED TO SERVICE.
DURING CRUISE HYD 3 ELEV "OFF" LIGHT ON. HYD QTY CHECKED 4.2 GAL AND KEPT ON DROPPING TO BELOW 2.5 GALS. HYD 3 FAIL. DIVERTED TO VCP, LANDING NORMAL. MX FOUND O-RING CUT ON HYD FILTER MODULE. REMOVED AND REPLACED O-RING IAW 20-40-02. SERVICED NR 3 HYD IAW 12-13-02, RUN UP AND LEAK CHECK GOOD IAW 29-00-00 PG 216. A/C RELEASED FOR SERVICE.
AFTER TAKEOFF, DEFLECTED AILERON DISAGREE LIGHT CAME ON. FUEL DUMPED TO REACH APPROVED LANDING WT. AIRCRAFT RETURNED TO AIRFILED. LANDING NORMAL. MX FOUND FLAP HANDLE POSITION XMITTER AT FAULT. UNIT REMOVED AND REPLACED IAW 27-54-03. OP'S CHECK SATISFACTORY, AIRCRAFT RETURNED TO SERVICE.
AFTER TAKEOFF CAPT'S PRIMARY FLT DISPLAY LOST ALL AIRSPEED AND ALTITUDE INDICATIONS. ELEVATOR LOAD FEEL AND FLAP LIMIT OVERRIDE ILLUMINATED, CAUSED BY THE LOSS OF CADC-1. THEN LOST CADC-2, AS WELL. PERFORMED APPROPRIATE CHECKLIST, REGAINED CADC-2. CREW CONTACTED COMPANY VIA PHONE PATCH, INQUIRED ABOUT WEATHER IN AMS. AFTER DOUBLE CHECKING FUEL AND ROUTE INFO, DETERMINED THAT AMS WAS THE BEST OPTION FOR DIVERSION. AIRCRAFT LANDED NORMAL. REMOVED AND REPLACED CADC NR 1 IAW 34-16-01. OP'S CHECKED NORMAL, AIRCRAFT RETURNED TO SERVICE.
DURING CLIMB/CRUISE LEVEL 2 ALERT "AIL DEFLECTOR DISAGREE" ILLUMINATED. QRH (QUICK REF HANDBOOK) PROCEDURES FOLLOWED, LIGHT REMAINED ON. AIRCRAFT DUMPED APPROX. 48,400 LBS IF FUEL FOR APPROVED LANDING WEIGHT. A/C RETURNED TO FIELD. LANDING NORMAL. MX FOUND BAD AILERON ACTUATOR, LEFT O/B ACTUATOR REPLACED. OP'S CHECKED SATISFACTORY, A/C RETURNED TO SERVICE.
