THE CREW REPORTED THAT DURING TAKEOFF ROLL AT APPROXIMATELY 98KTS, THE MASTER WARNING LIGHT ILLUMINATED MOMENTARILY. ABORTED THE TAKEOFF. PERFORMED RTS OF DEU 1, DEU 2, AND DEU 3. RTS PASS IAW MM 31-61-02. OPS TEST OF MASTER WARNING ILLUMINATED PUSH BUTTON SWITCH. NO DISCREPANCIES NOTED.
THE CREW REPORTED THAT DURING TAKEOFF ROLL AT APPROXIMATELY 90KTS, THE MASTER WARNING LIGHT ILLUMINATED MOMENTARILY. AND THE AUTO BRAKES REACTED UNEVEN PULLING THE ACFT TO THE LT WITH A JERKING MOTION. CORRECTED THE SITUATION WITH MOVEMENT OF THE RUDDER AND NOSE WHEEL STEERING IN THE OPPOSITE DIRECTION. THE CREW PROCEEDED TO REJECT TAKEOFF. AS THE ACFT CAME NEAR TO A STOP, IT JOLTED ONCE AGAIN BUT THIS TIME HARDER THAN BEFORE. PROCEEDED TO TAXI TO THE GATE WITHOUT ANY FURTHER INCIDENT. PERFORMED RTS OF DEU 1, DEU 2, AND DEU 3. RTS PASS IAW MM 31-61-02. OPS TEST OF MASTER WARNING ILLUMINATED PUSH BUTTON SWITCH. NO DISCREPANCIES NOTED.
THE CREW REPORTED THAT PRIOR TO TOP OF DESCENT THAT A LEVEL 2 ALERT FOR GEN BUSS 3 FAULT WAS DISPLAYED IN THE EAD. FOLLOWED THE QRH PROCEDURES, DEPLOYED THE ADG AND NO FUEL DUMP WAS PERFORMED. LANDED WITHOUT FURTHER INCIDENT OR DELAY. PERFORMED FIM 24-20-17, STEPS 1-5, CHECKED T1-T3 FOR IDG TO GROUND. CHECKED CONNECTIONS P1-8559A & P1-8559B. CHECKED 53-393. GR-3 TO BTR-3, B7-7, B7-8 & B7-9 WITH GROUND = 250 OHMS. CHECKED GENERATOR RELAY & BUSS TIE RELAY. FOUND ALL NORMAL. FOUND RCCB B1-138 DIFFERENTIAL FAULT DEFECTIVE, R & R RCCB AND OPERATIONALLY CHECKED NORMAL.
THE CREW REPORTED DURING CRUISE FLIGHT, APPROXIMATELY 1 HOUR FROM DESTINATION THAT A LEVEL 2 ALERT FOR GEN BUSS 3 FAULT WAS DISPLAYED ON THE EAD. FOLLOWED THE QRH PROCEDURES, DEPLOYED THE ADG AND NO FUEL DUMP WAS PERFORMED. LANDED WITHOUT ANY FURTHER INCIDENT OR DELAY. REMOVED NR 3 BUSS ASSY FEEDER CABLES, NR 3 GR & NR 3 GEN CONTROL RCCB. STOWED ADG. OPS CHECK NORMAL. ACFT RELEASED FOR SERVICE.
THE CREW REPORTED DURING CRUISE FLIGHT, APPROX. 60 MILES FROM CARTAGENA VOR THAT A LEVEL 2 ALERT: GEN BUS 3 FAULT WAS DISPLAYED IN THE EAD. THE CREW THEREBY CONTACTED DISPATCH AND FLIGHT OPERATIONS. THE CREW FOLLOWED THE QRH PROCEDURES, DEPLOYED THE ADG AND NO FUEL DUMP WAS PERFORMED. THE CREW RETURNED WITHOUT ANY FURTHER INCIDENT OR DELAY. PERFORMED FUNCTIONAL TEST OF THE GEN BUSS FAULT RESET IAW 24-21-00.ENGINE RUN WAS PERFORMED IAW 71-00-00. EPCU RTS TEST PASSED.
THE CREW REPORTED THAT DURING CRUISE FLIGHT, APPROXIMATELY 50 MINUTES AFTER TAKEOFF, A LEVEL 2 ALERT OF A GENERATOR BUSS 3 FAULT WAS DISPLAYED IN THE EAD. FOLLOWED QRH PROCEDURES, DEPLOYED THE ADG AND NO FUEL DUMP WAS PERFORMED. RETURNED TO DEPARTURE WITHOUT FURTHER INCIDENT. R & R DIFFERENTIAL PROTECTION CURRENT TRANSFORMERS, T1-9071, T1-9074 & T1- 9080. GROUND OPS NORMAL. SYSTEM CHECKED OK.
CREW REPORTED THAT DURING TAKEOFF, THRUST REVERSER UNLOCK LIGHT ILLUMINATED LESS THAN 80 KNOTS. CREW RETURNED TO THE GATE IAW MAINTENANCE PROCEDURE TO LOCK OUT THRUST REVERSER. CREW ATTEMPTED ANOTHER TAKEOFF AND THE SAME OCCURRENCE OF THE THRUST REVERSER UNLOCK LIGHT INDICATION ILLUMINATED AGAIN LESS THAN 80 KNOTS CAUSING A SECOND REJECTED TAKEOFF. CREW RETURNED TO THE RAMP WITHOUT ANY FURTHER INCIDENT.
DURING TAKEOFF ROLL, PRIOR TO V1, CREW REPORTED THEY FELT VIBRATION AND SIMULTANEOUSLY "LEVEL 2 ALERT" INDICATED "TIRE FAILURE," NR 1 TIRE FAIL DISPLAYED. THE CREW ABORTED TAKEOFF IAW PROCEDURE. ALSO, DUE TO THE HIGH-ENERGY STOP, THIS CAUSED AN OVER TEMP OF THE THERMAL PLUGS IN 4 OTHER TIRES.
DURING CRUISE, AT APPROXIMATELY 100NM SOUTH OF AIRPORT, LEVEL 2, GEN 3 BUSS FAIL ALERTED. CREW FOLLOWED THE QRH PROCEDURES, DEPLOYED THE ADG. CONTINUED FLIGHT AS PLANNED AND LANDED AT FLIGHT DESTINATION WITHOUT ANY FURTHER INCIDENTS. PERFORMED ENGINE RUN. PERFORMED FUNCTIONAL TEST OF THE GENERATOR BUSS FAULT TEST IAW AMM-24-21-00. STOWED ADG IAW AMM-24-21-00. OPS CHECKED NORMAL.
CREW REPORTED THEY LOST THE RT EMERGENCY AC BUSS. THE ALERT WAS GENERATOR BUSS 3 FAULT. HAD ONLY A SINGLE GENERATOR OPERATIVE. FOLLOWED QRH PROCEDURES, DEPLOYED ADG AND LANDED WITHOUT FURTHER INCIDENT. PERFORMED TASK IAW FIM 24-20-05. AUTO RESET OF EPCU. STOWED ADG. OPS CHECKED NORMAL.
IN FLIGHT, LOST RT EMERGENCY AC BUSS, GEN BUS 3 FAULT ALERT. FOLLOWED THE QRH PROCEDURES, DEPLOYED ADG AND RETURNED TO DEPARTURE WITHOUT FURTHER INCIDENT. FOUND IDG/INTERFACE TRIM HEAD FAULT. FOUND CONNECTOR "A" PINS 8 & 7 OPEN. REPINNED. OPS CHECKED OK. AFTER CHECK OUT, HAD NR 3 TRANSFORMER FAULT. REPLACED NR 3 TR & GEN CONT RELAY, SWAP RELAY. ALL NO HELP. R2 APU GCU, NO FAULTS.
DURING CRUISE FLIGHT, THE CREW COULD NOT SHUT OFF FUEL FLOW TO THE UPPER AUX TANK. CREW DIVERTED DUE TO THE MALFUNCTION. FUEL WAS ADDED AND THE MALFUNCTION WAS TRANSFERRED TO DMI. TRAVEL WAS RESUMED. ONCE IN DESTINATION, THE AUX FILL VALVE ACTUATOR WAS REPLACED, DMI WAS CLEARED, AND THE OPS CHECK WAS NORMAL. ACFT HAS RESUMED FLYING WITHOUT INCIDENT.
DURING MX FOR NR 1 TANK, AFT PUMP OFF, THE PUMP HOUSING PROTECTOR COVER WAS BROKEN AT EACH SCREW ATTACH POINT AND THE CONNECTOR WAS SHORTED & BURNED. THE LEFT AFT PUMP HOUSING, HARNESS, AND RCCB WERE REPLACED AND PROBLEM WAS CORRECTED (DMI CLEARED).
DURING CRUISE FROM LTX TO MIA, CREW ENCOUNTERED A NR 3 GENERATOR OFF LIGHT AND LOSS OF POWER TO BUS NR 3 AND RIGHT EMERGENCY BUS. CREW DEPLOYED THE ADG AND REGAINED POWER TO BOTH BUSES. CREW DIVERTED INTO BOG DUE TO REMAINING FUEL ON BOARD WITH NO FURTHER INCIDENT. MAINTENANCE PERFORMED EPCO RTS TEST IAW MD11 AMM 24-08-01, TEST PASSED. RE-RACKED GCU NR 3 IAW MD11 AMM 24-21-03, BUS 3 AND RT EMERGENCY BUS POWER RESTORED. ADG STOWED AND ALL BUSES SUPPLIED WITH POWER.
DURING CLIMB AT ABOUT 17000FT MANIFOLD FAILURE NR 3 ALERT AND 18500FT MANIFOLD FAILURE NR 1 ALERT. FUEL DUMPED TO MLW AND ACFT RTB (MIA) WITH NO INCIDENT. REMOVED AND REPLACED PSC NR 1 AND NR 2 IAW MD11 AMM 36-10-02. RTS ON PSC NR 1 & NR 2 PASSED IAW MD11 AMM 36-10-02. PERFORMED PNEUMATIC OP CHECK AT MAX POWER IAW MD11 AMM 36-00-03 AND NO DEFECTS NOTED. PERFORMED PNEUMATIC SYSTEM PRESSURE DECAY CHECK WITH ENGINES RUNNING IAW MD11 AMM 36-00-02 AND NO DEFECTS NOTED. ENVIRONMENTAL SYSTEM CONTROLLER AUTO MODE INOP - OK IN MANUAL. TRANSFERRED TO DMI 8368 IAW MEL 21-08-1-1 CAT C EXP 02/14/13. MAINTENANCE PROCEDURES ACCOMPLISHED.
DURING CLIMBOUT AIR SYSTEM 3 OFF ILLUMINATED WITH THE BLEED AIR 3 MANIFOLD ANNUNCIATOR LIGHT ON OVERHEAD PANEL ALSO ILLUMINATED. DURING DESCENT AIR SYSTEM 1 OFF ILLUMINATED. NO FUEL DUMPED AND ACFT RTB (MIA) WITH NO INCIDENT. PERFORMED FIM 36-23-03. REMOVED AND REPLACED MANIFOLD FAILURE CONTROL IAW MD11 AMM 36-23-01, OP CHECK NORMAL- NO FAULTS. PERFORMED FIM 36-11-01. RE-SECURED LOOSE UNION AT NR 1 HIGH STAGE VALVE AND RE-SECURED SENSE LINE IAW MD11 AMM 36-11-03, OP CHECK PASSED WITH NO FAULTS. RE-SECURED PACK COMPRESSOR DISCHARGE DUCT CLAMP IAW MD11 AMM 21-53-01, OP CHECK WITH NO LEAKS NOTED. PERFORMED PNEUMATIC SYSTEM PRESSURE DECAY CHECK WITH ENGINES NR 1 AND NR 3 IAW MD11 AMM 36-00-02 AND AMM 71-00-00. NO FAULTS NOTED. PERFORMED PNEUMATIC SYSTEM ADJUSTMENT/TEST IAW MD11 AMM 36-00-03, AMM 71-00-00, AND AMM 36-10-02. ALL PASSED WITH NO FAULTS.
DURING CLIMB CREW ENCOUNTERED A LEVEL 3 CABIN ALT ALERT ALONG WITH NR 2 AND NR 3 BLEED FAIL AND NR 3 PNEUMATIC MANIFOLD FAIL. FOLLOWED PROCEDURES, EMERGENCY DESCENT AND DUMPED FUEL (38000 K) FOR ACCEPTABLE LANDING WEIGHT. AIRCRAFT RETURNED TO SCL WITH NORMAL LANDING. NR 3 PNEUMATIC MANIFOLD FAIL. NR 3 BLEED FAIL, LVL 2 MESSAGE. NR 2 BLEED FAIL, LVL 2 MESSAGE. REPAIRED DUCT LEAK (LOOSE CLAMP) IAW MD11 AMM 20-10-11. LVL 3 CABIN ALT ALERT. RTS TO CABIN PRESSURE CONTROLLER IAW MD11 AMM 21-31-00. NO FAULTS. AIRCRAFT RETURNED TO SERVICE.
DURING CLIMB BOTH AUTOPILOT SYSTEMS FAILED. CREW NOT ABLE TO RE-ENGAGE. FOLLOWED PROCEDURES, DUMPED APPROX 50K KGS FUEL TO ACCOMPLISH ACCEPTABLE LANDING WEIGHT. AIRCRAFT RETURNED TO MIAMI WITH NORMAL LANDING. MAINTENANCE FOUND DUPLEX FLAP LIMITER ACTUATOR INOP. REMOVED AND REPLACED DUPLEX FLAP LIMITER ACTUATOR IAW MD11 AMM 27-56-01. OP CHECK NORMAL. REMOVED AND REPLACED NR 1 FCC IAW MD11 AMM 22-01-03. TEST FAILED-ENTERED ON DMI AUTOPILOT NR 1 (DMI# 8289) AS PER MEL 22-10-01 EXPIRATION 01/15/13.
IN CRUISE, OUT OF ANC, LEVEL 2 'GEN BUS 3 ALERT'. DEPLOYED ADG AND RETURNED TO FIELD. MAINTENANCE PERFORMED TROUBLESHOOTING IAW FIM 24-20-17. AIRCRAFT RELEASE TO SERVICE ON MEL 24-21-1. MEL 24-21-1 CLEARED ON 12-09-12 WITH REPLACEMENT OF IDG.
WHILE IN FLIGHT, LEVEL 2 "GEN BUS 3 FAULT" ILLUMINATED. DEPLOYED ADG AND RETURNED TO FIELD (MIA) FOR SAFE LANDING. REPLACED EPCU IAW AMM 24-08-01, GROUND RUN ENGINES, NO DEFECTS NOTED. STOWED ADG IAW AMM 24-25-01. AIRCRAFT RETURNED TO SERVICE.
DURING CLIMBOUT, LEVEL 1 ALERT HYD PUMP 3L OFF, HYD PUMP 2L OFF, HYD PUMP 1L OFF. AIRCRAFT DUMPED APPROX 3K KGS OF FUEL FOR LANDING WEIGHT ENVELOPE, AIRCRAFT RETURNED TO AIRFIELD, LANDING NORMAL. MX FOUND SHORT IN CONNECTOR ON NR 2 ENGINE. RE-PIN CONNECTOR FROM NR 2 ENGINE RIGHT EDHP, OP'S CHECK NORMAL IAW 29-11-02 AND 71-02-02. AIRCRAFT RETURNED TO SERVICE.
DURING CRUISE, NR 1 WEATHER RADAR FAILED, SELECTED NR 2 WEATHER RADAR AND FAILED. RETURNED TO AIRFIELD. APPROX, 53K KILO'S OF FUEL DUMPED. LANDING NORMAL. MX FOUND WEATHER RADAR ANTENNA FAULTY. REPLACED ANTENNA IAW 34-41-00, OP'S CHECK NORMAL. AIRCRAFT RETURNED TO SERVICE.
DURING CRUISE, CREW NOTICED AMBER ALERT LEVEL 1 HYD QTY SYS 1, QTY GOING LOWER. A/C DIVERTED TO CCS. LANDING NORMAL. MX FOUND O-RING CUT AND SQUEEZED OUT ON NR 1 EDP LOW PRESSURE SWITCH. REMOVED AND REPLACED O-RING IAW 29-12-34. SERVICED NR 1 HYD SYS IAW 12-13-02, RUN UP AND LEAK CHECK GOOD IAW 29-00-00. A/C RETURNED TO SERVICE.
ON APPROACH, LANDING GEAR DID NOT COME DOWN AFTER GEAR HANDLE WAS PLACED IN THE "DOWN" POSITION. CYCLED 2 TIMES AND ACCOMPLISHED QRH PROCEDURES. GEAR FINALLY CAME DOWN ON 3RD TRY. LANDING ACCOMPLISHED, WITH NO FAULTS. PERFORMED LANDING GEAR RETRACTION/EXTENSION TEST. LUBRICATED ALL MAIN LANDING GEARS IAW 12-21-05. PERFORMED TROUBLESHOOTING IAW FIM 32-30-14, 15. CHECKED LANDING GEAR POSITION FOLLOW UP PUSH RODS IAW 32-31-00, OPS CHECKED NORMAL. AIRCRAFT RETURNED TO SERVICE.
CREW REPORTED THEY HAD ELECTRICAL PROBLEMS. MX CONTROL ADVISED CREW TO RETURN TO BASE (MIAMI) A/C LANDED NORMAL, SECOND TIME TO RETURN TO FIELD FOR SAME ISSUES. MX REMOVED AND REPLACED AC BUS TIE RELAY, AC BUS SENSING RELAY, AND NR 1 T/R. RUN ALL 3 ENGINES, CHECKED PHASE'S ON GENERATORS, FOUND PHASE 'B' WAS UNDER VOLTS. PLACED NR 1 IDG ON MEL, IDG ASSY PLACED ON ORDER. A/C RELEASED FOR SERVICE.
