Maintenance disclosures
THE PILOT REPORTED DURING INITIAL CLIMB OUT FROM KPHX THAT HE OBSERVED FUEL LEAKING FROM THE INSTRUMENT PANEL. THE PILOT INFORMED ATC AND REQUESTED TO RETURN TO THE AIRPORT. UPON LANDING ATC DISPATCHED FIRE AND RESCUE BUT WAS NEEDED AND THE PILOT WAS ABLE TO TAXI TO PARKING WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT INSTRUMENT PLUMBING ASSOCIATED WITH THE FUEL FLOW AND FUEL PRESSURE GAUGES. MAINTENANCE SECURED AND TIGHTENED THE LINES AND PERFORMED GROUND RUNS. NO LEAKS WERE NOTED AND THE AIRCRAFT WAS RETURNED TO SERVICE AND NO SUBSEQUENT ISSUES HAVE BEEN REPORTED.
DURING FLIGHT LT ENG LOST MANIFOLD PRESS DROPPED TO 27 INCHES, PILOT DECLARED AN EMER & DIVERTED & LANDED AT WITHOUT FURTHER INCIDENT. TROUBLESHOOTING REVEALED FAULTY TURBOCHARGER ON LT ENG. R & R TURBOCHARGER & PERFORMED OPERATIONAL CHECK OF ENG. ALL PERFORMANCE CHECKS WERE NORMAL.
ON APPROACH THE PILOT SELECTED GEAR DOWN WITH NO MOVEMENT OF THE LG. THE PILOT ATTEMPTED TO CYCLE THE LG WITH THE SAME RESULTS. PERFORMED AN EMER EXTENSION OF THE LG & RECEIVED THREE DOWN & LOCKED INDICATION. PILOT LANDED WITHOUT FURTHER INCIDENT. TROUBLESHOOTING REVEALED THE RT ENG HYD FILTER BOWL HAS A CRACK IN THE BOWL THREADS WHICH RESULTED IN THE LOSS OF HYD FLUID TO THE POINT WHERE THE LG WOULD NOT CYCLE NORMALLY. R & R HYD FILTER BOWL AN6234-1 IAW MM SECTION 6-147 & 6-148. LEAK CHECK PERFORMED BY GND RUNNING THE ENGS, LEAK CHECKED SATISFACTORY & BOTH ENG HYD PUMPS CHECKED NORMAL.
ON DEPARTURE, THE PILOT REPORTED THAT ALL ENGINE INDICATIONS WERE NORMAL PRIOR TO ROTATION. CLIMBING OUT OF 4000 FT, THE RT ENGINE LOST POWER. PILOT PROCEEDED WITH THE ENGINE OUT PROCEDURES AND DETERMINED THAT THE RT ENGINE WAS ONLY PRODUCING 30 INCHES OF MANIFOLD PRESSURE AND THAT THE FUEL FLOW WAS 8 PSI. RETURNED TO DEPARTURE AND LANDED WITHOUT INCIDENT. WITH THE ENGINE STILL PRODUCING POWER DID NOT SHUTDOWN ENGINE. MX INSPECTED THE RT ENGINE AND FOUND THE NR 1 CYLINDER INJECTOR LINE BROKEN. AD2011-26-04 LAST COMPLIED WITH ON 05-24-2012 AIRCRAFT TT: 20555.3 TIME SINCE LAST INSPECTION 67.7 HOURS. NO DEFECTS WERE FOUND DURING THE INSPECTION.
EN ROUTE TO KBUR THE PILOT REPORTED THAT HE OBSERVED A FAINT CRACKLING SOUND ACCOMPANIED BY A FLICKERING OF THE CENTER CONSOLE LIGHTS AND A SMALL AMOUNT OF SMOKE ARISING FROM THE SAME PANEL. THE PILOT IMMEDIATELY SHUT OFF ELECTRICAL POWER AND EXECUTED THE EMERGENCY CHECKLIST. THE SMOKE CEASED AND UPON COMPLETION OF THE CHECKLIST THE PILOT RE-APPLIED ELECTRICAL POWER AND NOTIFIED ATC OF THE SITUATION AND AS A PRECAUTION REQUESTED VECTORS TOWARDS KVGT. THE PILOT FURTHER ASSESSED THE SITUATION; THE MALFUNCTION APPEARED TO BE ISOLATED TO THE RIGHT FUEL BOOST PUMP CIRCUIT BREAKER, WITH THE CIRCUIT BREAKER PULLED THE SITUATION SEEMED TO BE ISOLATED. THE PILOT PROCEEDED TO KBUR WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND THE RIGHT FUEL BOOST PUMP AND CIRCUIT BREAKER FUNCTIONED NORMALLY. FURTHER INVESTIGATION REVEALED THAT A RESISTOR AND CAPACITOR IN THE MAIN AUDIO PANEL WAS SHORTED. THIS WAS THE CAUSE OF THE SMOKE AND THE LIGHTS FLICKERING. THE AUDIO PANEL WAS REPLACED.
