DURING FLIGHT, RECEIVED A LEVEL 2 ALERT HYDRAULIC SYSTEM 3 FAIL. ACFT DIVERTED AND LANDED SAFELY. PUMP CASE DRAIN LINE CRACKED LINE WAS REPLACED.
30 MINUTES INTO FLIGHT HYDRAULIC SYSTEM 3 PRESENTS HYDRAULIC SYS 3, LEVEL 1 ALERT, FOLLOWED BY QUANTITY LOW, FOLLOWED BY HYD SYSTEM 3 FAIL. ACFT RETURNED TO DEPARTURE.
DURING APPLICATION OF TAKEOFF POWER, ENGINE NR 1 DID NOT ACCELERATE. DISCONTINUED TAKEOFF.
DURING DECENT, ENG NR 2 FADEC BACK UP POWER APPEARED, THEN ENG NR 2 SHUT DOWN.
DURING DESCENT (SBKP), (02-02-2013) AIRCRAFT CAPTURED FL 180/LEVEL OFF. THROTTLES ADVANCED BUT ALL ENGINES INITIALLY FAILED TO RESPOND. LOSS OF 40 KTS BEFORE ENGINE SPOOL-UP. PERFORMED RTS FCC1 AND FCC 2, RTS PASSED OK, ACCOMPLISHED TEST SERVO AUTO THROTTLE. TEST OK IAW AMM REF. 22-00-07. AIRCRAFT RETURNED TO SERVICE. NO FURTHER SIMILAR INCIDENT REPORTED TO DATE.
DURING APPROACH (SKBO) PHASE, LEVEL 2 HYD 3 FAIL ALERT. LANDED WITHOUT INCIDENT. DURING INSPECTION FOR CORRECTION ACTION, INBOARD SLAT DRIVE MECHANISM WAS FOUND CRACKED. FURTHER INSPECTION AND REPAIRS TBD.
LEVEL 2 ALERT, HYD 1 FAILURE ON ARRIVAL MIA. DURING TS FOUND NR 1 ENGINE HYD PUMP LOWER SUPPLY LOSE LEAKING. REMOVED AND REPLACED HOSE ASSY IAW MM 20-40-01.
DISCREPANCY - DURING TAKEOFF ROLL (SEQU), BEFORE 80 KNOTS, REVERSE NR 3 UNLOCK ILLUMINATE. AFTER TROUBLESHOOTING, ACCOMPLISHED REVERSE LOCKOUT NR 3 ENGINE IAW 78-32-00 / TRANSFER TO DMI 6057, CAT "C", PLACARAD INSTALLED IAW MEL 78-30-1.
SUPPLEMENT INFORMATION - (06-13-2011)(MIA) REMOVED AND REPLACED # E TR VALVE PRESSURE SWITCH AND DIRECTIONAL PILOT ASSY IAW MM 78-31-08. OPS CHECK IAW MM 78-30-00 NORMAL. CLEARED DMI 6057, REMOVED PLACARD, AND AIRCRAFT WAS RETURNED TO SERVICE.
DURING TAKEOFF ROLL, LEVEL 1 ALERT REV 3 FAULT. TRANSFERRED TO DMI 6037 IAW MEL 78-30-1, CAT C, PLACARD INSTALLED AND DEACTIVATED IAW MM. AIRCRAFT RETURNED TO SERVICE.
REJECTED TAKEOFF, BELOW 89 KTS (MIA) DUE TO REV NR 3 PRESS FAULT. TRANSFERRED TO DMI 5705 IAW MEL 78-30-1. DE-ACTIVATED NR 3 TR. CAT "C" PLACARD INSTALLED. ON TROUBLESHOOTING OF DISCREPACY, FOUND DEFECTED PRESSURE SWITCH. ( 04-17-2011) R & R PRESSURE SWITCH ON DIRECTIONAL PILOT IAW AMM 78-31-08. OPS CHECK OK. DMI PLACARD REMOVED. ACFT RETURNED TO SERVICE.
LEVEL 2 TIRE FAIL ALERT DURING T/O ROLL. AIRCRAFT RETURNED TO FIELD, NO FUEL WAS DUMPED. LANDING NORMAL. MX FOUND NR 10 TIRE PRESSURE XDUCER DEFECTIVE. INDICATOR PLACED ON MEL 32-46-1-2. AIRCRAFT RETURNED TO SERVICE
AFTER TAKEOFF, LT MLG UNSAFE (RED) LIGHT ILLUMINATED BOTH PRIMARY AND SECONDARY. PERFORMED APPROPRIATE CHECK LIST. DUMPED FUEL,APPROX 20,380 GALLONS, RETURNED TO AIRFIELD WITH UNEVENTFUL LANDING. ACFT RETURNED TO GATE. MX PERFOMED INSP WHEN ACFT RETURNED TO GATE, FOUND LT MLG TRIM CYLINDER LEAKING (FAILED). R & R TRIM CYLINDER IAW32-37-01, OP'S AND LEAK CHECKED SATISFACTORY. ACFT RETURNED TO SERVICE.
UPON LANDING AT SBKP, DUE TO HIGH BODY ANGLE, APPROX 10 DEGREES, CAUSED BY AN EXCESSIVE CLOSURE RATE TO THE GROUND, ANTENNA WAS SCRAPPED BY ABOUT 1/8TH OF AN INCH. DURING APPROACH BY F/O, NORMAL APPROACH UNDER VMC CONDITIONS TO RWY 15, UPON ENCOUNTERING EXCESSIVE CLOSURE RATE TO THE GROUND, CORRECTIVE ACTION WAS TAKEN, UPON CONTACT WITH THE GROUND A/C BOUNCED FIRMLY, F/O RECOVERED ASSISTED BY CAPTAIN TO PREVENT EXCEEDING 10 DEGREES BODY ANGLE. FLT MECH PERFORMED A VISUAL INSPECTION AND DISCOVERED THE SCRAPPING TO NR 3 VHF ANT. TAIL STRIKE INSP ACCOMPLISHED IAW 5-51-13 WITH NO FINDINGS. HARD LANDING INSPECTION ACCOMPLISHED IAW 5-51-03 WITH NO FINDINGS. NR 3 VHF ANTENNA PLACED ON MEL. AIRCRAFT RETURNED TO SERVICE. MAINTENANCE REPLACED NR 3 VHF ANTENNA 5-6-2010, IAW 23-12-05/401. OP'S CHECK GOOD.
DURING PRE-FLIGHT, "BUS L EMERGENCY AC OFF LEVEL 2" INDICATION CAME ON. T/S IAW 24-24-03, FOUND WIRE BUNDLE AT APPROX STA 950 ON RT SIDE OF FWD BELLY O/HEAD AREA BURNED/SHORTED WITH DAMAGE TO WIRES. T/S IAW FIM 24-24-03. FOUND WIRE BUNDLE BURNED/SHORTED WITH DAMAGE TO WIRES. INSPECT AREA FOR O/HEAT CONDITIONS, AND SURROUNDING AREA AND WIRES AND PLUMBING. INSPECTION CHECKED GOOD. WIRE BUNDLE REPAIRED IAW SWPM 20-10-03. ELECTRICAL CHECK OUT C/W, OP'S CHECK GOOD. A/C RETURNED TO SERVICE.
DURING APPROACH TO SKBO, A/C STABLE ON GLIDE PATH & ON SPEED, 50 FLAPS, PRIOR TO 50' CALL OUT, "SINK RATE" CALL FROM GPWS WAS MADE. F/O REACTED POSITIVELY, WITH PITCH UP CORRECTION TO DECELERATE THE SINK RATE. IMMEDIATELY CAME 50' AND TOUCH DOWN WAS MADE. SINCE LANDING WAS FIRM & ACTION OF INCREASE IN LIFT ON WINGS CAUSED BY PITCH UP, THE AIRCRAFT BOUNCED ONCE AND SETTLED ON GROUND WITHIN TOUCHDOWN ZONE. NO HARD LANDING WAS FELT. AIRCRAFT CONTINUED ON ROLL OUT AND TAXI TO PROPER AREA. POST FLT INSPECTION REVEALED NR 3 VHF ANTENNA DAMAGED DUE TO GROUND STRIKE. TAIL STRIKE INSPECTION ACCOMPLISHED IAW 5-51-13 WITH NO FINDINGS. HARD LANDING INSPECTION ACCOMPLISHED IAW 5-51-03 WITH NO FINDINGS. NR 3 VHF ANTENNA PLACED ON MEL. AIRCRAFT CONTINUED IN SERVICE. MAINTENANCE REPLACED NR 3 VHF ANTENNA UPON ARRIVAL TO MIA, IAW 23-12-05/401 OP'S CHECK NORMAL.
