SHORTLY AFTER TAKEOFF OF A TOUR FLIGHT, AT APPROX 9:40 AM, THE PILOT NOTICED THE ENGINE NG SPEED HUNTING AND THE TORQUE FLUCTUATING (LESS THAN 20 PERCENT), THE AC ALSO EXHIBITED SLIGHT YAWING IN CONJUNCTION WITH THE FLUCTUATING NG AND TORQUE. THE PILOT PERFORMED A LOW POWER PRECAUTIONARY LANDING AT AIRPORT. INSPECTION BY MAINTENANCE REVEALED NO DEFECTS WITH THE FCU RIGGING OR WITH THE FUEL FILTERING AND FUEL DELIVERY SYSTEM. A CHECK OF THE FCU WAS FOUND TO BE DEFECTIVE (THE FCU CHARACTERISTICS CHECKS FAILED). THE DEFECTIVE FCU WAS REMOVED AND SERVICEABLE UNIT INSTALLED. NO FURTHER DEFECTS HAVE OCCURRED. (K)
ON MARCH 20, 2004 WHILE ON A SIGHTSEEING FLIGHT, THE PILOT EXPERIENCED AN ENGINE CHIP LIGHT INDICATION. HE MADE A PRECAUTIONARY LANDING AT APPROXIMATELY 12:50 PM. A MECHANIC WAS DISPATCHED TO THE SITE AND INSPECTED ALL CHIP PLUGS. A SMALL SLIVER WAS FOUND AT THE OUTPUT OF THE OIL PUMP. NO FURTHER OCCURENCES WERE NOTED.

WHITE PAINT IS PEELING OFF BLADE AND IS EXTREMELY HARD TO BALANCE. PLEASE REPAIR UNDER FULL WARRANTY.

HELICOPTER WAS ON A FINAL APPROACH TO LANDING SITE ON ISLAND OF KAHOOLAWE, HAWAII. PILOT EXPERIENCED A YAW TO THE ACFT. HE MADE A NORMAL LANDING ON THE HELIPAD AND SHUT DOWN ENGINE. POST-FLIGHT CHECK REVEALED THE POWER TURBINE BLADES WERE DAMAGED. UPON DISASSEMBLY AND INSPECTION OF ENGINE, DETERMINED THE SECOND STAGE GAS GENERATOR TURBINE BLADE BROKE AWAY AT THE ROOT AND CAUSED ADDITIONAL DAMAGE TO THE SECOND STAGE GAS GENERATOR BLADES, POWER TURBINE NOZZLE, AND POWER TURBINE BLADES. SIGNIFICANT RUBBING WAS EVIDENT ON COMPRESSOR, GAS GENERATOR, AND POWER TURBINE COMPONENTS.
DURING A TOUR FLIGHT, PILOT REPORTED A TOTAL ELECTRICAL FAILURE. PILOT PROCEEDED TO LAND AT THE COMPANY HELIPORT WITHOUT INCIDENT. FOUND ALL CAUTION LIGHTS, RADIOS, GAUGES, AND AIR CONDITIONING OPERATED NORMALLY. WHEN DEPRESSING THE START OR 'CRANK' SWITCHES FOUND STARTER WOULD NOT ROTATE. REMOVED AND INSTALLED SERVICEABLE BATTERY AND STARTER/GENERATOR AIRCRAFT WAS OPS CHECKED AND RETURNED TO SERVICE. UPON INSPECTION OF THE STARTER/GENERATOR BRUSHES, FOUND 2 OF THE 4 INSIDE BRUSHES WERE WORN.
