ENG1 ON APPROACH FLAME OUT. PROCEEDED BY QRH - AFTER TROUBLESHOOTING, 5 TRIPS TO THE BLAST FENCE FOR GROUND HIGH POWER RUNS, AND 2 FCFâS TO CHECK THE TORQUE PROBLEM. WE REPLACED THE TORQUE SENSOR AND SWAPPED ALTERNATORS FOR TROUBLESHOOTING WITH NO HELP. THE LAST THING TO CHANGE WAS THE ECU AND THAT WAS ACCOMPLISHED. COMPLETED HIGH POWER GROUND RUNS AND RELEASED THE AIRCRAFT. THERE HAS NOT BEEN A REPEAT WRITE-UP SINCE THE ECU WAS CHANGED.
SMOKEY ODOR DURING THE FIRST HOUR, AFTER T/O CLIMB AND CRUISE BUT NO VISIBLE SMOKE OR SMOKE DETECTORS DIDN`T ACTIVATE.
AT APPROXIMATELY 6 PM THE AIRCRAFT DEPARTED FROM MSY. AFTER APPROXIMATELY 30 MINUTES INTO THE FLIGHT, THE CREW REPORTED A FLAP FAULT CENTRAL WARNING PANEL INDICATION AND SPLIT FLAP INDICATION IN THE FLAP INDICATOR. FLAP FAULT DOES NOT CLEAR UNTIL FLAPS 7 WAS SELECTED. FLIGHT CREW RETURNED THE AIRCRAFT TO MSY. THE FLIGHT CREW DECLARED AN EMERGENCY BEFORE LANDING AND LANDED WITHOUT ANY FURTHER ISSUES. MAINTENANCE PERFORMED A COMPLETE RIG OF THE FLAP CONTROL SYSTEM AND MADE ADJUSTMENTS AS REQUIRED IN ACCORDANCE WITH THE MAINTENANCE MANUAL PROCEDURES. AIRCRAFT THEN OPERATED SATISFACTORILY ON A FUNCTIONAL CHECK FLIGHT FOR APPROXIMATELY AN HOUR. AIRCRAFT WAS RETURNED TO SERVICE.
DURING THE AFTERNOON FLIGHT FROM LIT TO MSY, THE CREW SELECTED FLAPS PRIOR THE LANDING. THE AIRCRAFT HAD A FLAP FAULT INDICATION ON THE CENTRAL WARNING PANEL WITH A SPLIT FAULT INDICATION ON THE FLAP POSITION INDICATOR. CREW PROCEEDED TO MSY. THEY DECLARED AN EMERGENCY AND LANDED WITH ZERO FLAPS WITHOUT FURTHER INCIDENT. MAINTENANCE DISCOVERED THE OUTPUT VOLTAGE READING FOR THE POSITION TRANSMITTER ON THE FLAP CONTROL LEVER WAS WELL OUT OF TOLERANCE. MAINTENANCE ADJUSTED THE POSITION OF THE TRANSMITTER ON THE FLAP CONTROL LEVER UNTIL VOLTAGE READING WAS IN SPECIFIED RANGE IN ACCORDANCE WITH THE MAINTENANCE MANUAL. OPERATIONAL CHECK WAS SATISFACTORY. DUE TO THE DEFINITIVE FIND, THE AIRCRAFT WAS RETURNED TO SERVICE.
ON APPROACH, SELECTED GEAR DOWN. NLG DOWN INDICATOR DID NOT ILLUMINATE. THE IN TRANSIT LIGHT DID ILLUMINATE DURING EXTENSION & EXTINGUISHED. CAPTAIN DECLARED AN EMER PRIOR TO LANDING. THE ACFT LANDED WITHOUT FURTHER INCIDENT. MX INSPECTED THE ACFT & REPLACED THE NLG DOWN INDICATOR LIGHT BULB. OPERATIONAL CHECK WAS SATISFACTORY. ACFT WAS RETURNED TO SERVICE.
PRESSURIZATION AT FL200 WENT OUT. OUTFLOW CABIN ALTITUDE CLIMBED. TROUBLESHOT SYSTEM IAW MM 21-30-00 AND FOUND PRIMARY OUTFLOW VALVE DOES NOT CLOSE FULLY. R & R OUTFLOW VALVE. OPS CHECKED GOOD.
WHILE COMPLYING WITH AD 2011-09-02 MULTIPLE SPOTS OF CORROSION WERE DISCOVERED BETWEEN THE LOWER TCAS ADAPTOR PLATE AND THE FUSELAGE SKIN. THE CONTOUR PLATE CALLED OUT IN THE STC WAS REPLACED AND THE CORROSION WAS BLENDED AND MAPPED OUT. INFORMATION WAS SENT TO DAN BUZZ AND ASSOCIATES FOR EVALUATION AND A REPAIR SCHEME DEVELOPED WHICH MET THE DAMAGE TOLERANCE REQUIREMENTS. FAA WAS CONTACTED PER THE AIRWORTHINESS DIRECTIVE AND REPAIRS TO THE CORRODED FUSELAGE SKIN MADE BY ADDITION OF AN EXTERNAL DOUBLER ASSEMBLY. INSTALLED DOUBLER ASSEMBLY, CONTOUR PLATE AND TCAS ANTENNA. RETURNED AIRCRAFT BACK INTO SERVICE.
AVIONICS SMOKE LIGHT CAME ON DURING FLIGHT. REMOVED SMOKE DETECTOR FROM AVIONICS BAY AND CLEANED CONNECTORS. OPS CHECK GOOD.
DURING TAKE-OFF ROLL LEFT ENGINE TAIL-PIPE HOT LIGHT ILLUMINATED AND SUBSEQUENTLY ABORTED TAKE-OFF. OPS CHECK IAW AMM 26-11-00-05, PROCEDURE 1, NO DEFECTS NOTED.
LT TAIL PIPE HOT. ILLUMINATED DURING TAKEOFF AT 100 KTS. PERFORMED LT TAIL PIPE HOT (LIGHT) DETECTION SYSTEM AND RAN LT ENG AT FULL POWER SETTING IAW SAAB 340 AMM 71-00-00 AND NO DISCREPANCY NOTED AT THIS TIME. (S)
RETURN TO GATE DUE TO IGNITION LIGHT LT, DURING TAXI, BEFORE TAKEOFF. REMOVED AND REPLACED LT DECU. OPS CK GOOD.
ON TAKEOFF PILOT REPORTED THAT CONFIGURATION WARNING LIGHT ILLUMINATED TO DISPATCH AND MAINTENANCE CONTROL. THE PILOT THEN REALIZED THAT HE DID NOT HAVE THE POWER LEVERS ALL THE WAY UP TO THE TAKEOFF POSITION CONSEQUENTLY; CONFIG WARNING LIGHT ILLUMINATED.
LOW SPEED ABORT DUE TO CONFIG WARNING. PERFORMED HIGH SPEED TAXI WITH NEW, COULD NOT DUPLICATE PROBLEM.
LT EMERGENCY EXIT HANDLE COVER FELL OFF IN FLIGHT. SECURED AND RELEASED FOR SERVICE IAW AMM 52-22-00.
DURING TAKEOFF BEFORE 80 KNOTS LEFT OIL PSI CWP WARNING LIGHT ILLUMINATED. FOUND OIL FILLER CAP OFF. SERVICED LT ENGINE W/ 2.75 QTS BP2380. SECURED FILLER CAP AND TEST RAN ENGINE. NO FURTHER DISCREPANCIES NOTED.
FOUND EMERGENCY EXIT SIGN BELOW WING EXIT DOOR NOT ILLUMINATING. REMOVED AND REPLACED FUSE 21LN IAW 33-50-00, OPS CHECK GOOD.
BOTH EFIS SCREENS BLANK ON THE FO'S SIDE. PWR'D DOWN ACFT REST THE LT MAIN ESS C/B, PERFORMED OPERATIONAL TEST, OP'S CHECK GOOD IAW AMM 34-12-00. FOUND MAIN STAR C/B ON RT ELEC-DC BUSSES POPPED REST BREAKER WITH EXTERNAL POWER ON EFIS, SCREENS OPERATE NORMAL AT THIS TIME, OK FOR SERVICE.
CARGO SMOKE LIGHT IN THE COCKPIT REMAINED ON. REMOVED AND REPLACED AFT CARGO BAY SMOKE DETECTOR IAW 26-13-15. OPS CHECK WAS SATISFACTORY, PN ON/OFF CGAB1201-01, SN OFF 467, SN ON 475.
LT ENGINE FIRE DETECT FAIL LIGHT ON THE TEE-HANDLE WOULD NOT ILLUMINATE. AFTER TROUBLESHOOTING SYSTEM, FOUND THE LT ENGINE FORWARD FIRE DETECTION LOOP WAS INOP. REMOVED AND REPLACED LT ENGINE FORWARD FIRE DETECTION LOOP IAW 26-11-05, PN ON/OFF 703072.
LT DUCT OVERTEMP LIGHT ILLUMINATED ON TAXI. REMOVED AND REPLACED LT HIGH PRSOV PN ON/OFF 321935 SN OFF 557A, SN ON 982CD, SN OFF 557A.
RT AFT EMERGENCY EXIT WOULD NOT OPEN FROM INSIDE OF A/C. REPLACED SHAFT, COUPLING AND LOWER BEARINGS ON RT AFT EMERGENCY EXIT LATCH MECHANISM IAW 52-20-01 OPS CHECK GOOD. SHAFT PN 129-514033-2, COUPLING PN 129-514067-2, SPRING PN MS9048-170, BEARING PN FF718-1 CAM PN 129-514065-2
CONFIG WARNING LIGHT ILLUMINATED AT 20 KNOTS. REMOVED AND REPLACED CONTROL QUANDRANT IAW AMM 76-10-05 OPS CHECK GOOD CONFIG WARNING OPS CHECKS IAW AMM 31-50-00 OPS CHECK GOOD PN ON 53162-4, PN OFF 53170-4, SN ON 005, SN OFF 031.
CONFIGURATION WARNING LIGHT ON FOR TAKEOFF. PERFORMED PITCH TRIM ADJT/TEST IAW 31-51-00 NO FAULTS FOUND. COULD NOT DUPLICATE.
FLAPS DEPLOY VIOLENTLY FROM 0 TO 15 DEGREES. REMOVED AND REPLACED FLAP VALVE.
EMERGENCY GEAR EXTENSION REQUIRED DUE TO NOSE LANDING GEAR LIGHT NOT ILLUMINATED AND IN TRANSIT LIGHT STILL ILLUMINATED
NO MOVEMENTS WHEN FLAPS SELECTED TO 15 DEGREES
RIGHT OIL PRESSURE ANNUNCIATOR ILLUMINATED DURING CLIMB FROM IAD WITH SLIGHT DROP IN OIL PRESSURE. CREW SHUTDOWN ENGINE IN FLIGHT PER EMERGENCY CHECKLIST E-13 AND RETURNED TO IAD. MAINTENANCE WAS DISPATCHED AND FOUND OIL LEAKING FROM THE BB-AXIS SEAL. THE SEAL WAS REPLACED AND A OPERATIONAL CHECK AND LEAK WAS GOOD. AIRCRAFT WAS RETURNED TO SERVICE. (M)
EN ROUTE FROM PIT TO HTS, LEFT PROPELLER OIL PRESSURE WAS ABOVE 140 PSI WITH ASOCIATED VIBRATION, AND FLUCTUATING PROPELLER RPM. AS A RESULT, LEFT ENGINE WAS SHUTDOWN ON APPROACH INTO HTS. AIRCRAFT LANDED WITHOUT INCIDENT. CREW REPORTED THAT ONCE ON GROUND, PROPELLER COULD NOT BE ROTATED BY HAND. MAINTENANCE WAS DISPATCHED AND FOUND THAT THE PROPELLER GEAR BOX HAD SEIZED. THAT GEARBOX WAS CHANGED AND THE AIRCRAFT WAS RETURNED TO SERVICE. (M)
LAVATORY SMOKE WARNING DETECTOR LIGHT ILLUMINATED INTERMITTENTLY DURING FLIGHT. FLIGHT PROGRESSED WITHOUT INCIDENT. LAVATORY FIRE EXTINGUISHER WAS FOUND TO BE SERVICEABLE WITH NO SIGNS OF SMOKE IN LAVATORY. REMOVED AND REPLACED LAVATORY SMOKE DETECTOR. OPERATIONALLY CHECKED GOOD. (M)
EMERGENCY LIGHTS AT ROWS 5A, 9A AND 9D OUT. MAINTENANCE RELAMPED BULBS, ALL CHECKS GOOD.
ON CLIMBOUT, NR 2 VERTICAL AIR SPEED INDICATOR FAILED. AIRCRAFT RETURNED TO MSP. MAINTENANCE REPLACED NR 2 VERTICAL AIR SPEED INDICATOR. OPS CHECK GOOD.
RIGHT AC GEN LIGHT ILLUMINATED IN FLIGHT, UNABLE TO REST PER QRH. AIRCRAFT RETURNED TO DTW. MAINTENANCE REPLACED RT AC GEN CONTROL UNIT, OPS CHECK GOOD. ALL WORK IAW SAAB AMM.
EXIT SIGN OVER SEAT 6A FELL. MAINTENANCE RESECURED SIGN, ALL CHECKS GOOD.
CARGO SMOKE DETECTOR DID NOT TEST. MAINTENANCE FOUND CONNECTOR PLUG LOOSE. INSTALLED PLUG AND TESTED SYSTEM, CHECKS GOOD.
DURING FLIGHT, O2 HOSE OVER SEAT 5D FELL OFF FROM ABOVE. AIRCRAFT RETURNED TO DTW. MAINTENANCE RESECURED PANEL AND HOSE ABOVE 5D, OVERHEAD PSU PANEL. SECURITY CHECK GOOD.
AVIONICS YELLOW FLAG ON EHSI APPEARED ONCE ON GROUND TAXING OUT, REST THEN THREE TIMES IN THE AIR, RESET TWICE, 3RD TIME WOULDN'T RESET, THEN SHUT OFF BY ITSELF. AIRCRAFT RETURNED TO MSP. MAINTENANCE REPLACED NR 1 AHRS COMPUTER. OPS CHECK GOOD IAW SF3 340 MM, CHAP 34.
DURING ACCOMPLISHMENT OF AD 93-19-01 NON-DESTRUCTIVE TEST OF PROPELLER HUB FOR CRACKS, ONE CRACK WAS FOUND. THIS CRACK IS APPROXIMATELY .50 INCH LONG AND LOCATED BETWEEN PROPELLER MOUNTING BOLT HOLES. THE PROPELLER HUB HAS BEEN REMOVED FROM SERVICE PERMANENTLY. (X)
