ON 22 APRIL 2026 AT 1:56PM, THE CREW REPORTED WHEN TRANSITIONING INTO HOVER A "POP" SOUND WAS HEARD. SUBSEQUENTLY MULTIPLE VERTICAL CRACKS WERE OBSERVED ON THE R/H HEATED GLASS WINDSHIELD COMMENCING FROM THE BOTTOM LEFT CORNER OF THE WINDSHIELD TO THE TOP. NO EVIDENCE WAS OBSERVED OF ANY FOREIGN OBJECT IMPACTING THE WINDSHIELD. UPON NOTICE OF THIS OCCURRENCE, CREW INFORMED FLIGHT OPERATIONS CENTRE OF INCIDENT. RETURN TO BASE, WITH NO NOTED FURTHER DISCREPANCIES. TIME IN SERVICE OF THE R/H HEATED GLASS WINDSHIELD COULD NOT BE DETERMINED AT THIS TIME. (AIRCRAFT TIME AND CYCLES USED, UNTIL CONFIRMATION OF INSTALLATION) OEM INFORMED OF THE OCCURRENCE AND A TECHNICAL QUERY RAISED. REFERENCE PSE/TQ/2026/239540. REPLACEMENT RH HEATED WINDSHIELD ORDERED. R/H HEATED GLASS WINDSHIELD CRACKED IN HOVER, SAME TO BE REPLACED. REFERENCE LOGBOOK ENTRY (LBE) 087/26. OPERATIONAL TEST CARRIED OUT SATISFACTORY. ON 16TH MARCH 2026, AFTER 9Y-TKD RETURNED TO THE HANGAR, A HEAVY DOWNPOUR OF JET FUEL EMANATING UNDERNEATH THE AIRCRAFT WAS OBSERVED. A DAILY INSPECTION WAS CONDUCTED; FUEL TANK LEAK WAS OBSERVED AT THE MAIN RIGHT FUEL TANK SUMP AREA. AIRCRAFT DEFUELED TO FACILITATE ASSESSMENT. FUEL SUMP GASKET WAS REPLACED WITH A NEW GASKET. AIRCRAFT REFUELED IN STAGES. AT ABOUT 1000LBS FUEL LEAKAGE WAS OBSERVED. UNSATISFACTORY. AIRCRAFT DEFUELED AND FUEL TANK REMOVED TO FACILITATE PHYSICAL INSPECTION OF AREA FOR SHARP EDGES AND FOD. NO FINDINGS. NR 2 MAIN RIGHT FUEL TANK REPLACED. AIRCRAFT REFUELED IN STAGES. QUANTITY INDICATION SYSTEM OPERATIONAL TEST CARRIED OUT. SATISFACTORY. AIRCRAFT RETURNED TO SERVICE.
DURING DAILY INSPECTION, THREE TAIL ROTOR BLADES WERE FOUND TO HAVE CRACKS NEAR THE EROSION SHIELD ,
DURING 500 HR INSPECTION, THE TAIL ROTOR BLADE WAS FOUND CRACKED NEAR THE EROSION SHIELD.
RADOME HAS MULTIPLE AREAS OF CORROSION ON LT AND RT SIDES OF RADOME. A MAJOR REPAIR WAS PERFORMED.
DURING ROUTINE INSPECTION NUMBER 1 TAIL ROTOR DRIVESHAFT HANGAR BEARING MOUNT WAS FOUND TO BE CRACKED ON INSIDE OF TAILBOOM, LEFT SIDE. OPEN
