Probable cause
INCORRECT INSTALLATION OF CRANKSHAFT COUNTERWWEIGHT ROLLERS BY THE MANUFACTURER DURING REMANUFACTURE OF THE ENGINE. FACTOR WAS THE INADEQUATE SURVEILLANCE OF THE ENGINE ASSEMBLY PROCESS BY THE MANUFACTURER.
