THE PILOT'S INADEQUATE PRE FLIGHT PLANNING AND DISREGARDING THE AIRCRAFT PERFORMANCE DATA. CONTRIBUTING FACTORS WERE THE EXTREME OUTSIDE AIR TEMPERATURES, THE ICY RUNWAY, AND THE TAILWIND.
