Probable cause
THE PILOT IN COMMAND'S FAILURE TO MAINTAIN VISUAL LOOKOUT AND CLEARANCE FROM TERRAIN. A FACTOR WAS THE PILOT DIVERTED HIS ATTENTION TO SECURE A LOOSE FIRE EXTINGUISHER.
