Probable cause

FUEL EXHAUSTION AS A RESULT OF THE PILOT IN COMMAND'S NOT REFUELING. A FACTOR IN THE ACCIDENT WAS THE OPERATOR/MANAGEMENT INADEQUATE FLIGHT FOLLOWING AND OPERATIONAL CONTROL OVER THE FLIGHT.
