THE IN-FLIGHT SEPARATION OF THE MAIN ROTOR BECAUSE OF THE FAILURE OF A PILLOW BLOCK BOLT, AND THE FAILURE OF THE OPERATOR/REPAIR STATION TO FOLLOW THE OVERHAUL PROCEDURES.
