Probable cause

INADEQUATE MAINTENANCE INSPECTION OF THE MAIN LANDING GEAR ACTUATOR ASSEMBLY BY COMPANY MAINTENANCE PERSONNEL. A FACTOR IN THE INCIDENT IS INADEQUATE DESIGN OF THE LANDING GEAR ACTUATOR BY THE MANUFACTURER.
