Probable cause

AN INADVERTENT DESCENT INTO THE GROUND WHILE CONDUCTING A NON-PRECISION INSTRUMENT APPROACH, AS A RESULT OF THE PILOT FAILING TO STOP HIS DESCENT AT MDA. A FACTOR IS LOW VISIBILITY DUE TO FOG.
