Probable cause
THE CATASTROPHIC FAILURE OF THE NO.12 BLADE ON THE 4TH STAGE TURBINE WHEEL, AND FAILURE OF COMPANY MAINTENANCE PERSONNEL TO IDENTIFY THE PREEXISTING CRACK DURING MAINTENANCE INSPECTION.
