Probable cause
THE MECHANIC'S FAILURE TO ASSEMBLE THE TAIL ROTOR DRIVE SHAFT CORRECTLY AND THE SUBSEQUENT FAILURE OF THE TAIL ROTOR DRIVE SHAFT. A FACTOR WAS THE LACK OF INSPECTION PROCEDURES FOR OFFSHORE MAINTENANCE.
