Probable cause

MISMANAGEMENT OF THE FUEL SYSTEM BY THE PILOT WHICH RESULTED IN FUEL STARVATION AND A DUEL ENGINE FLAMEOUT. CONTRIBUTING TO THE ACCIDENT WAS THE PILOT'S LACK OF TOTAL EXPERIENCE IN THE SIKORSKY S58T.
